Birth weight is one of the most significant indicators of the quality of life of children and their mothers, and the very low weight considered relevant in the infant mortality rate and maternal stress, which led the World Health Organization to identify it as the single most important factor. The objective of then research was to understand the emotional demands of mothers of very low birth weight (MBP) preterm infants admitted to Neonatal Intensive Care Units. Qualitative research with ten mothers with children attended at the intensive care unit of a referral hospital in Recife - PE. Data collection was performed through interviews between January and July 2019, which were analysed using Minayo et al. (2001) Thematic Content analysis. Ten mothers participated, aged between 19 and 36 years. The delimitation of the sample was based on the content saturation criterion. The identified categories highlight that the fact that the mother gives birth to a baby who needs special attention makes them feel helpless but use hope and spirituality to support themselves. The team can increase the quality of care as it understands mothers' emotional situations and demonstrates this understanding by minimizing risks of stress, depression, anxiety, and negative thoughts from mothers. In addition, being next to the babies proved to be a mitigating factor for possible symptoms of emotional disorganization.
