Stimulated by the desire to improve the quality of life, numerous multiprofessional health work teams have emerged in recent times. In this perspective, the objective of this article is to demonstrate how multidisciplinary educational actions can improve the integration between the school, the Basic Health Unit and the community, in order to make students aware of the importance of maintaining adequate sodium levels in their daily diet. It is a field research, based on action research, resulting from experience constituted from multidisciplinary practice between a school, a Basic Health Unit and a community in the neighborhood where they are located. The results indicate that the school, teachers and managers, as well as nutritionists and nurses, when working in an integrated manner, can play a fundamental role in the identification of additional health needs and in the possibility of collective interventions to improve the population's quality of life. The benefits of shared learning linking education and health, including the acquisition of team work skills, were presented not only to the teams involved but to the entire school community.
