The present study aimed to conduct a narrative review of the literature about the difficulties faced by people living on the street during the COVID-19 pandemic, and to theorize them based on Ayres' three dimensions: individual, social and programmatic vulnerability. To this end, a survey of scientific articles was made in different databases - Latin American and Caribbean Literature on Health Sciences (LILACS), Publons, Web of Sciences and on the PubMed portal -, referring to the period from 2020 to 2021, 42 texts were collected. The findings were analyzed based on the conceptual framework of vulnerability, which encompasses the behaviors and characteristics of life of a specific group, its dynamics with society and access to public resources, which end contribute to the greater or lesser exposure to an injury. Among the measures applied during the pandemic, mental health care for homeless people was not a priority, with records of few services. It can be seen how a vulnerable group became even more vulnerable, and its invisibility made it difficult to create more productive responses.
