Health care failures can compromise patients' safety. At the hospital, especially in Intensive Care Unit (ICU), the operations management depends on administrative tasks, but their role in patients' safety has not been studied yet. This study has the purpose to evaluate administrative failures in ICU patients using empirical approach of exploratory and descriptive research and a research-action strategy. We have detected 653 administrative failures (6.94%) and of them, 280 adverse events (42.8%). Administrative failures increased length ICU stay in 1.84 days, (p<0.0005) and hospital stay (p=0.002). This research shows that administrative failures occur in ICU; which are mostly of avoidable type (99.3%). The manager role in this scenario is to monitor administrative tasks of risk to patient safety using different tools and even preventing them by adding barriers for management risk seeking for better quality and higher operational efficiency.
