Resumen

VARANDAS, Maria de Lourdes  y  LOPES, Albino. Continuous professional training and nursing care quality: the need for a change of educational paradigm. Rev. Lusófona de Educação [online]. 2012, n.22, pp.141-158. ISSN 1645-7250.

The evolution and improvement of nursing services' quality has been a constant concern both to health care professionals and the Nursing Teaching system. The training doesn't limit itself to the simple acquisition of knowledge and techniques. It requires the training process to be based on critical-reflexive pedagogical referential, contributing to foster professional skills that enable holistic care centered action. Adding to this, training should be based on a reflection about habitus and professional socialization, the transference of which takes place on real life situations, clinical teaching, according to the consistency of the models, norms and values present or acquired during the internship. Thus, we aim to understand "what is the contribution of initial training to the patients' perceived quality? Our purpose is to understand how nurses construe their knowledge and offer it to the community. We focus on the role of nursing education and on the skills acquired in the course of continuous nurse training. Our goals are: 1) to assess whether nurses perceive the initial training to be directed towards the emergence of skills required to provide quality healthcare; 2) to assess the perception of patients' satisfaction with nursing care; 3) and to identify the differences between nurses' and patients' perceptions about the quality of healthcare. We conducted a multimethod research with two lines of studies. The first line includes two studies: on study 1, based on a qualitative methodology, critical literature was reviewed and exploratory interviews to 17 field experts were conducted; on study 2, semi-structured interviews to 26 nurses were conducted in order to understand nurses' perception about the role of training on the quality of care. The second line of studies is carried out in "deductive form" and intends to evaluate patient's satisfaction with healthcare. Two studies were conducted, both descriptive cross-sectional combining quantitative and qualitative methodologies. On study 1, we applied the instruments SUCEH21, to 116 customers for the CSP context, and SUCECS26. On study 2 we ran a document analysis of 94 complaints presented to the studied organizations in digital format. The results allowed the identification of a set of variables that influence directly and indirectly both the quality of healthcare and patient's satisfaction. They also allowed the construction of the final framework to analyze the problem posed. On that framework we highlight the status of emerging competencies on organizational context as a mediating factor.
