Abstract

Displaced radial head fratures in children are rare and when they occur can be difficult to treat and be the cause of complications such as stiffness of the elbow, avascular necrosis or radial head overgrowth. The closed intramedullary pinning - Métaizeau technique allows indirect reduction and fixation of the frature with soft tissue preservation. We evaluated patients with displaced radial head fratures treated by means of closed intramedullary pinning at our institution between 1997 and 2009. A total of 11 patients, 7 boys and 4 girls, with a mean age of 11 years old (minimum of 6 years old and maximum 16 years old). According to Judet classification, 6 fratures were type III and 5 type IV. In functional assessment, with a mean follow-up of 6.5 years, and using the score of Metaizeau we obtained 91% excellent and good results. Using the score of Steinberg and Rodriguez-Merchán: 91% good and moderate results. Radiographically all fratures consolidated, without loss of reduction compared to that achieved intraoperatively. As for complications, we found only a bursitis at the insertion site of the Kirschner wire. No avascular necrosis or infections. We consider this to be the method of choice in radial head fratures with angulation greater than 30 degrees, because it is not technically very demanding, preserving the soft tissues and leading to very good functional results with few complications.
