The most common type of ankle injury is supination-external rotation (SER-4) which results in a frature of the distal fibula followed by internal injury, that can be internal malleolus frature or deltoid ligament rupture. We conducted a retrospective case-control study which included patients with SER-4 fratures of the ankle operated in our department between June 1998 and June 2008 - 220 patients: 141 with bimalleolar fratures and 79 with the external malleolus frature with internal ligament injury. Through consultation of medical records was done epidemiological characterization of patients. Two random samples were collected for clinical and radiographic evaluation: 30 patients with bimalleolar fratures and 30 patients with external malleolus frature with deltoide ligament injury, with mean follow-up of 5 years. All data were recorded and compared. We found statistically significant differences between the two groups in the mean age and the presence of comorbidities. We have also found a higher prevalence of bimalleolar fratures in women, although this difference was not statistically signifi cant. Regarding the clinical and radiographic evaluation we found significantly better results in patients with external malleolus frature with internal ligament injury. We found also a slightly higher percentage of complications and need for extraction of material due to intolerance in patients with bimalleolar fratures, although this difference was not statistically significant. The functional and radiographic results in the medium and long terms in ankle fratures appear to be dependent on the type of lesion in the inner region and age, gender and the presence of comorbidities.
