Background: Fibular hemimelia has been reported as the most common congenital longitudinal deficiency of the long bones. In the pass ablative surgery was the first choice of treatment. With the improvements of the surgical techniques the reconstructive surgery has gained more importance and gradually has substitute the ablative procedure. Objectives: case report of clinical case of limb lenght discrepancy in a paraxial intercalary fibular hemimelia with short femur. Methods: Presentation a clinical case: child with 6 years old, gender masculine, with fibular hemimelia of the right leg, presenting a 6.8cm lenght discrepançy. Results: The patient suffer a limb lengthening of both femur and tibia of the affected leg in a single intervention getting a complete resolution of the inicial discrepancy of the limb. Conclusions: A simultaneous lengthening of both the femur and the tibia in a patient with fibular hemimelia can be a reasonable surgical option in the presence of a severe discrepancy.
