Abstract

Scheuermann's disease is an entity that, in the growth phase, gives a good response to conservative treatment. However when there is pain, deformity progression in the presence of an orthopedic treatment or existence of a kyphosis exceeding 70 ° in a skeletally mature adolescent, surgical intervention is necessary to obtain a correct sagital balance. The authors have evaluated clinically and radiographically 14 patients that underwent surgical treatment with exclusive use of transpedicular posterior instrumentation and periapical osteotomies. The mean preoperative Cobb angle of 84° kyphosis (T2 - T12), corrected postoperatively at 4 weeks to 55.7º and 59.4º at 12 months. There were three complications: proximal junctional kyphosis, distal junctional kyphosis, superior mesenteric artery syndrome. The transpedicular instrumentation with periapical osteotomies gives a good result without the complications inherent in the anterior.
