Abstract

We present the clinical case of a male patient, 50 years old, construction worker in Angola where he suffered a fall from a height with bilateral intra-articular fracture of the calcaneus, subjected to conservative treatment. Two years later he returned to Portugal, we observed the patient in our service, he had severe pain in the foot, with retro and medium valgus deformity, total loss of internal plantar arch. Was able to walk only with the help of crutches with perimeter limited to five hundred yards by pain, featured a preoperative AOFAS Score 26. The radiograph of the feet in charge showed loss of height of the calcaneus, reducing of the Böhler angle and hind foot with valgus deviation and residual deformity type IV. Because of his age, the remarkable functional and labour limitation, the intensity of pain complaints, underwent a calcaneal varus osteotomy combined with a subtalar arthrodesis with compression screws. We obtained internal plantar arch replacement with a foot able to do charge, the patient regained the ability to walk without crutches with postoperative A.O.F.A.S. Score 69. He returned to work, now with minor physical intensity tasks. The surgical treatment could possibly have been the first option in this particular patient, currently the criteria for conservative treatment in the calcaneus fracture are the Sanders type I fractures non displaced fracture or fracture with displacement less than 2 mm, without damage of joint surface, without deviations in valgus or varus position.
