Abstract

Osteonecrosis of the femoral head in the late-juvenile and adolescent age is rare, nevertheless often leads to serious destructive changes, ostheoarthritis and significant disability. There are several risk factors that can be categorized in four types: traumatic, idiopathic, systemic illnesses or iatrogenic (chemotherapy, radiotherapy, corticosteroid therapy). The treatment remains a challenge, leading in the worst scenario to total hip arthroplasty or in certain cases to hip fusion in short/medium term. The authors report two cases of osteonecrosis of the femoral head, one traumatic and other idiopathic, treated with trapdoor procedure and autogenous bone grafting. We described the surgical technique and the evolution of both cases. Although the experience is limited, we consider a demanding technique that in severe cases doesn’t prevent hip osteoarthritis however may delay its progression and postpone the need for total hip arthroplasty.
