Arthroscopic reconstruction for anterior cruciate ligament rupture is a common orthopaedic procedure and the technique has been evolving since its introduction in 1900. Standard ACL reconstruction with a single-bundle graft has been consistently associated with good to excellent clinical and functional outcomes and with high rates of return to previous levels of activity. However, after reports of persistent instability and the development of osteoarthritis after this procedure, some authors have advocated a double-bundle reconstruction as a way of better controlling knee rotational torque, a potential cause of failure after single-bundle reconstruction. The authors investigated outcomes of single-bundle versus double-bundle ACL reconstruction and tried to summarize the anatomical, biomechanical and technical aspects of the anatomical approach of ACL repair.
