Hip arthroscopy is a procedure increasingly used in the treatment of hip pathology. Its development, in the last decade, is due to the greater knowledge of the bases of "hip pain" in young adults, particularly the syndrome of femoroacetabular impingement. Like in other joints, hip arthroscopy could be a diagnostic or therapeutic procedure. It is indicated in multiple clinical situations, being, currently, the most common, treatment of femoroacetabular impingement (FAI). We present the arthroscopic "Out-In" technique that by performing an anterior capsulotomy, allows the reproduction of the steps of mini-open arthrotomy by facilitating arthroscopic procedure, minimizing the time and intensity of traction and allowing the treatment of articular hip pathology. Were performed 30 hip arthroscopies in the period between July 2012 and December 2012. All the procedures were performed by the same surgeon and by the arthroscopic out-in technique. The diagnoses that led to the intervention were: 1 snapping hip, 1 monoarthritis, 1 epiphysiolysis sequel, 1 "os acetabuli", 1 enchondroma and 25 femoroacetabular impingement. We present our experience and detailed description of the out-in arthroscopic technique. The realization of hip arthroscopy is a complex procedure with a long learning curve. The out-in arthroscopic technique by enabling the arthroscopic reproduction of steps of mini-open anterior technique leads, in our opinion, to a reduced learning curve for surgeons with previous experience in anterior arthrotomy and arthroscopic procedures.
