Abstract

DINIZ, Pedro et al. Poderá Realizar-se o Tratamento de Joanete de Alfaiate por Via Percutânea sem Recurso a Osteotomia do Quinto Metatársico?. Rev. Port. Ortop. Traum. [online]. 2016, vol.24, n.2, pp.103-111. ISSN 1646-2122.

Introduction: Tailor’s bunion is a forefoot disease characterized by a painful bony prominence in the fith metatarsal head (M5). Several surgical techniques have been described, frequently accompanied by non-neglectable complications. In recent years we witnessed a growing interest in minimally invasive techniques. The objective of this study was to evaluate the results of percutaneous correction of tailor’s bunion. Materials and Methods: We retrospectively evaluated 26 cases, regarding 16 patients (1 man, 15 women). Average age: 62 years (45-78). Average follow-up: 3 years (1-8). The surgical technique consisted of diaphyseal varization osteotomy of the M5 and/or exostosectomy of the M5 head. Results were measured regarding clinical and radiographic evaluation. Results: Procedures were performed by 5 surgeons. In 15 cases a M5 osteotomy was perfomed, in 9 cases an isolated M5 exostectomy and in 2 cases concurrent exostectomy and M5 osteotomy. In 23 cases (88.5%), patients were satisfied or very satisfied. The 3 cases concerning unsatisfied patients correspond to cases were an isolated exostectomy was performed. We registered 4 cases of transfer metatarsalgia due to other procedures and 2 cases of diminished range of motion in the fifth metatarsophalangeal joint. Conclusion: The studied technique granted good results, with high patient satisfaction. According to this study an M5 osteotomy should almost always be performed, being rarely indicated an isolated M5 osteotomy.
