Objectives: The use of C2 translaminar fixation, proposed by Wright in 2004 with crossed screws allows rigid fixation to the posterior elements. Several studies have shown a biomechanical equivalence to other methods with the advantage of being more technically simple. This study aimed to report the clinical results of the initial experience of using C2 translaminar screws technique in atlantoaxial arthrodesis, in our hospital. Methods: The authors conducted a retrospective review of patients who underwent atlantoaxial arthrodesis with translaminar screws at our institution. Results: We treated 10 patients and in 6 cases the indication was a secondary atlantoaxial instability caused by rheumatoid arthritis. 20 translaminar screws with postoperative imaging evaluation demonstrated the correct positioning of all of them. Intraoperatively a case of vertebral artery lesion was recorded, not directly related to the placing of C2 screws. No other intraoperative complications were recorded. At 6 months postoperatively there was no recording of cases of nonunion or collapse of material. Conclusion: The atlantoaxial arthrodesis with C2 translaminar screws is a technique that ensures biomechanical stability and excellent fusion rates with potentially lower complications compared to other techniques that are more demanding.
