Objective: To systematically review the literature on the pectoralis major rupture and compare several repair methods clinical results. Methods: A systematic literature review was performed for case-reports, case-series and clinical studies, published since 1941 and indexed for PubMed, that describe pectoralis major surgical treatment results. The primary outcome was clinical result according to Bak et al. classification. The secondary outcomes included time to surgery, age and follow-up. The several repair methods outcomes and acute versus chronic treatment were compared statistically. Results: Twenty-seven studies with 110 patients and 9 repair methods were included: suture; transosseous suture; screws and washers, non-specified anchors, absorbable anchors, titanium anchors, suture anchors, titanium anchors and transosseous suture, cortical button. The mean age and follow-up were 29,6 years and 28 months respectively. Excellent or good results were observed in 90,9 % of the patients and no statistical difference between repair methods groups was detected. Anchors subgroup analysis (11 cases) revealed similar results for the different types of anchors. The acute repair (54 cases) had significantly better results than chronic repair (56 cases). Conclusions: According to this systematic review, acute surgical repair should be preferred with no sufficient available evidence to recommend any specific repair method.
