Periapical cemental dysplasia is a disorder of bone tissue formation and remodeling where, during its development, bone tissue suffers a change in cell differentiation, resulting in malformed tissue. It presents predominantly in the female sex, black ethnicity, and the age group of 40 years. It can be confused with inflammatory periapical lesions of endodontic origin due to its similar radiographic characteristics. Periapical cemental dysplasia is usually identified through routine radiographic examinations, as it tends to show no clinical signs and symptoms.

Early diagnosis is important for the treatment plan of this pathology, which, in most

cases, is conservative and translates into clinical and radiographic monitoring. The aim of this study was to present a case of periapical cemental dysplasia with early diagnosis in the first stage and a seven-year follow-up, focusing on the diagnostic and treatment methods available.
