The infection or inflammation of the maxillary sinus mucosa is denominated maxillary sinusitis. Odontogenic sinusitis corresponds to about 10-12% of the cases of maxillary sinusitis, and its causes are caries, periodontal disease, odontogenic cysts, and iatrogenesis.

The dentigerous cyst is a bone-destructive lesion, classified as a developmental odontogenic cyst. Such lesions can reach large proportions and, when located in the maxillary sinus, can present symptoms of sinusitis. The present report aims to describe the case of a young female patient who presented a large dentigerous cyst associated with a third molar in the left maxillary sinus, with sinusitis symptoms, as well as discuss the clinical and radiographic characteristics. Surgical treatment was performed under general anesthesia, with complete enucleation of the lesion using the Caldwell-Luc technique, and the 12-month postoperative control allowed bone neoformation without recurrence of the lesion and free of surgical complications.
