Hyperreactio luteinalis (HL) is a rare pregnancy-related condition characterized by a massive enlargement of the ovaries due to the development of multiple benign theca lutein cysts. Most pregnant women remain asymptomatic and adnexal masses are incidentally diagnosed on routine obstetrical ultrasound. Expectant management should be considered as HL spontaneously regresses in the postpartum period. However, HL may mimic an ovarian malignancy and so it is essential to remind this condition, to avoid unnecessary surgical interventions. We describe a case of a HL in a spontaneous singleton pregnancy, discussing the conducted conservative approach.
