Objectives: To determine in patients with Peripheral Arterial Disease (PAD) whether if the type of treatment and the degree of ischemia infl uences smoking habits. Material and methods: From January 2010 to December 2011 317 smokers were treated, 304 men and 13 women, mean age 66 ± 9,7 years. Of these, 44% were medically managed, 22% underwent endovascular revascularization and 34% surgery. We studied the demographic characteristics of the population, presence of risk factors and cardiovascular events, indications for treatment and smoking cessation after revascularization or during follow-up. Results: Statistical analysis revealed a higher proportion of patients who stopped smoking when they underwent endovascular procedure compared to those undergoing medical treatment (66% vs. 51%, p = 0,036) and also higher when undergoing surgery (66% vs 80% p = 0,021). In the endovascular treatment group, 65% of ex-smokers were claudicants and 69% had critical limb ischemia (p = 0,36). In the surgical group, the percentage of patients who quit smoking was higher among those with critical limb ischemia compared to claudicants (86% vs 72%, p = 0,039). Conclusions: Patients with Peripheral Arterial Disease who are undergoing more invasive revascularization procedures such as surgery, are more likely to quit smoking than those subjected to endovascular or medical treatment. The presence of critical limb ischemia during surgical treatment infl uenced, in a statistically signifi cant way, smoking cessation. Therefore, the moment of surgical proposal is crucial to promote smoking cessation.
