Objective: To analyse the effect of Highly Active Antiretroviral Therapy (HAART) on human papillomavirus (HPV) infections in patients with positive human immunodeficiency virus (HIV), namely men who have sex with men (MSM). The secondary objective is to assess the importance of the family doctor in the screening of these infections in view of their impact on patients' quality of life. Data sources: Guidelines Finder, National Guidelines Clearinghouse, UpToDate, The Cochrane Database, Medscape, MEDLINE/Pubmed. Methods: A literature search was performed in the databases of evidence-based medicine sites. The MeSH terms used were ‘HPV’, ‘HIV’, ‘HAART’ and articles published after 2005, in Portuguese and English languages were searched. Articles that fulfilled the objective of the study and the inclusion criteria were selected. The photo archive of the venereology department of Hospital de Santa Maria (HSM) was also consulted. Results: Six articles were selected (five systematic reviews and one clinical guideline). HAART therapy reduced the incidence of opportunistic diseases, but had no effect on the decrease of HPV infections. The most common anogenital lesions are condylomata acuminata, but premalignant and malignant lesions such as anal intraepithelial neoplasia and anal cancer may occur. The incidence and prevalence of dermatological changes by HPV have been increasing in patients with HIV infection, namely in MSM. Conclusion: HPV lesions in patients infected with HIV have been increasing. In this aspect, there was no benefit resulting from HAART therapy. The high morbidity and mortality associated with these lesions highlights the importance of creating screening programs in order to diagnose these changes early and to implement the most appropriate treatment for a better prognosis.
