Introduction: Chronic obstructive pulmonary disease (COPD) is an important cause of morbidity and mortality and it is not usually diagnosed until advanced stages. Some authors advocate the use of screening tools. The aim of this study is to evaluate the performance of the International Primary Care Airways Guidelines (IPAG) and the COPD-6® mini-spirometer in a population of patients. Methods: A diagnostic test study was carried out at a Family Health Unit of Porto, in patients aged 40 years or over. Demographic variables were collected, the IPAG questionnaire was completed, the COPD-6® device was used, and the participants performed a spirometry. The values of sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and area under the curve for the IPAG and COPD-6® were calculated. Results: A total of 97 users participated in the study. Sensitivity of COPD-6® device was 66.67%, specificity 97.80%, PPV 66.67%, and NPV 97.80%. IPAG and COPD-6® combined showed a sensitivity of 66.67%, specificity of 98.90%, PPV of 80%, and VPN of 97.83%. The accuracy of COPD-6® was 82%, and of IPAG combined with COPD-6® was 83%. Conclusion: The COPD-6® device and the IPAG questionnaire are useful tools for the early detection of COPD, improving accessibility to spirometry, and potentially reducing the underdiagnosis of COPD.
