Introduction: In family medicine, the patient must be seen as a part of a whole - the family. The family assessment instruments are essential tools for the family doctor (FD). Another auxiliary is the children's drawing. With this, it is possible to overcome verbal inhibition and communicate in childhood. In this age group, many of the behavioral changes are due to problems in the socio-family environment. In the case of children from separated parents, the effects of separation and conflict on parental dyad are extensive and can affect many aspects of child development. This case aims to demonstrate that a good relationship between the FD, the child, and his family may be ‘the key' for the identification and orientation of risk situations. Case description: Male child, 8 years old, in the third year of schooling, separated parents, living with mother and sister. The mother schedules an appointment to the son because he has been verbalizing negative thoughts and dislikes the school. We used children's drawing, discovering that this child was a victim of bullying and that the parents' separation was the cause for suffering. We constructed with the mother the genogram and Mitchell's psycho figure, which exposed data so far unknown that were in the origin of this child's behavior. He was referenced for psychology and we carried out intervention with the family and the school. Comments: Working with families is as interesting, as challenging, and it is important that the FD knows when to move from the individual register to an observation of the family system. Used instruments have proved essential to clarify and guide this clinical case. Many of the problems in childhood have their origin in the socio-family context, to which the clinician must be aware. The FD should be seen as an element of reception, support, and health promotion in the family system.
