Abstract

Introduction:

Endometrial carcinoma is the most common gynecological malignancy in women. Local or distant recurrence is one of the main problems that occur after treatment. This case is intended to alert to the eventual late recurrence after endometrial carcinoma treatment, highlighting the role of the family doctor (FD) in monitoring patients over time.

Case description:

A 74-year-old woman with a history of endometrial cancer who underwent surgery and radiotherapy in 2010, consulted the FD because she had had a sudden onset of dysarthria, changes in motor coordination, and subsequent installation of right hemiparesis one month ago. A non-acute ischemic stroke was admitted, and complementary diagnosis exams were requested. When she returned to the appointment to show the exams, the diagnostic hypotheses raised were cerebral metastasis vs glioblastoma, having been referred to the emergency department due to the concomitant worsening of the neurological state. She was evaluated by neurosurgery and underwent surgical intervention, being completely asymptomatic and the histological result revealed metastasis of a well-differentiated adenocarcinoma of probable gynecological origin.

Commentary:

The development of metastases in the central nervous system from primary gynecological cancers is a rare event, being more frequent in the first three years after treatment. Contrary to the data in the current literature, this case demonstrates the diagnosis of metastatic disease nine years after the treatment of endometrial carcinoma, reinforcing the central role of FD in the monitoring of patients, since, knowing them, he is alert to identify risk factors, as well as to detect clinical manifestations and warning signs of various diseases.
