The male breast can be affected by a variety of benign and malignant entities. Breast hemangioma is a rare benign vascular tumor occurring in both genders, being even rarer in men, with only a few cases reported in the literature. This is a case report of a 78-year- old male patient with a known history of advanced laryngeal cancer previously treated with radiotherapy who was admitted to our emergency department in the setting of severe dyspnea. A neck and chest computerized tomography were performed, revealing post radiation changes of the neck and no significant pulmonary abnormalities. An incidental left breast nodule was noted and considered indeterminate. The mammogram and the breast ultrasound revealed a well-circumscribed hyperdense and hypoechoic oval nodule, without spiculations nor calcifications. It also demonstrated a slightly heterogeneous echo pattern with no posterior acoustic shadowing. An ultrasound-guided core needle biopsy was performed and the histopathologic examination was compatible with a cavernous hemangioma.
