Abstract

Myelolipomas are benign tumors composed of adipocytes and hematopoietic cells, presenting in older adults. They commonly occur in the adrenal gland and less frequently in extra-adrenal locations. Their etiology is unknown, but eventually related to adrenocorticotropic hormonal excess. Most extra-adrenal myelolipomas are detected incidentally on cross-sectional imaging, they have mature fat and radiologically are difficult to differentiate from other neoplasm with fat, including liposarcoma.

We present a case of an extra-adrenal myelolipoma in a perirenal location and discuss its epidemiology, pathologic and radiologic features, differential diagnosis, and management.
