Rede Nacional de Cuidados Continuados Integrados was created to provide continuous care to people that, independent of their age, are dependent. However, most of the people admitted to these units are older adults, with multiple comorbidities. Older adults suffer from physiological transformations due to the ageing process, being particularly predisposed to changes in nutritional status, namely the development of undernutrition, which is associated with higher morbidity and mortality. The aim of this protocol is to optimize nutrition and health care given to the elderly and standardize the proceedings related to the nutritional assessment of the patients admitted to these units. This protocol proposes a model for the nutritional assessment of individuals in Unidades de Cuidados Continuados, mostly older adults.
