Resumo

In this case study the age, nationality and the suspected respiratory tuberculosis of two workers were the warnings that led public health professionals to visit a metalworking company, to track their workers and investigate all possible contacts of suspected cases. The visit to the blasting plant with fire projection metallization enabled the survey of the conditions of the workplace, the private rented rural housing and the number of workers at risk from chemical agents dispersed in the environment. The managing partner collaborated fully with the local public health structure, periodically providing all safety, hygiene and health documentation, from 2005 to 2017.

In 2007 Social Security validated a permanent disability of 45%, which increased to 95% in 2013 due to silicotuberculosis, the date on which the multipurpose medical certificate was issued by the health authority. There were two confirmed cases of respiratory tuberculosis in the group of eight workers of the company.

In the approach of this case it is discussed what is the functionality of the public health doctor, the occupational doctor and the family medicine doctor, and the difficulty in exchanging information by the data protection law. We emphasize that the doctor of general and family medicine and the doctor of public health are the ones who accompany the patient throughout his life, so they are some of the health professionals who are qualified to diagnose and treat the diseases of the active population and promote their health in their general and work context. However, the completion of the aptitude form is the competence attributed to the Occupational Physicians, in accordance with current regulations, which will reflect the diagnosis of any occupational disease or any other pathologie that may interfere with the work ability and/ or evolve negatively with the current work tasks, being able to propose adjustments in them, if necessary and clarifying some legal issues for all involved in the work process.

The security conditions of the production unit improved after the intervention of all competent entities, driven by the visit by the public health structure in 2008 and the change of management in 2018. We emphasize that the information system does not register with SIARS (ACSS, 2019) occupational diseases as a health problem code and it is included in the preventive activities of the functional units of the health center cluster.
