Elderly patients usually present with multiple comorbidities and polymedication, thus any health disturbance is al-ways a source of serious concern. As our population grows older it is very important to be familiar with how to deal with multiple diagnosis in our clinical practice. Herpes simplex virus type 1 is the main pathogen causing encephalitis worldwide. In elderly patients, changes in consciousness status may correspond to a variety of underlying clinical conditions, therefore, differential diagnosis based on complementary diagnostic test results becomes essential for the correct approach to the patient in clinical practice.
