Green nail syndrome, also known as Goldman-Fox syndrome, is a condition characterized by green discoloration of the nail plate, proximal paronychia and distal onycholysis.

The diagnosis is usually clinical, based on the physical examination of the nail.

Treatment begins with measures to keep the nails dry, avoiding wet environments, and pharmacological treatment using topical and/or oral antibiotic therapy. In refractory cases, surgical removal of the nail may be necessary.

The authors describe the case of a 36-year-old woman presenting with green nail dyschromia following a traumatic injury. Based on the clinical findings, the patient was diagnosed with green nail syndrome and different treatment options were tried. This pathology remains a challenge to clinical practice.
