Introduction: Assessment, in higher education in health, in view of the changes in educational strategies that have been implemented in undergraduate health courses, encompass a variety of forms and instruments both from a qualitative and quantitative point of view, with a view to the development maximum of human learning. Thus, the research question was What are the social representations of teachers and students about the evaluation for the development of professional competence in the training of nurses and doctors carried out through Format 5 and what suggestions to improve its use? Objective: to analyze the social representation of professors and students of Nursing and Medicine courses on the evaluation for the development of professional competence, through Format 5 and to describe the suggestions for improvements on its use. Method: This is a Qualitative Research based on the Theory of Social Representations developed in a college in the interior of São Paulo. 14 professors and 60 students participated, 40 from the Medicine Course and 20 from the Nursing Course, all from the second grade, through a semi-structured interview with two open questions: What is your opinion about the evaluation format of the Educational Unit - F5 adopted in this school? Would you have suggestions for adequacy and improvements in the Formats. The data obtained were organized using the Collective Subject Discourse technique. Results: The organization of the results revealed four central ideas, three of which are equivalent to question 1 and one to question 2, respectively: Importance of Format 5; Formative assessment in the curricular proposal; Feedback via Format 5; Suggestions on the content, use and structure of the Format, presented followed by their speeches. Conclusions: The results obtained emphasized the importance of evaluation for the development of teaching and learning, the relevance of the evaluation format and the indicators of improvement of the evaluation process.
