Introduction: By understanding nursing as a science, technology, and art in promoting individualized, collective care in its singularities, articulated with other professionals, it assumes that only the expanded concept of care is capable of meeting the complexity of the subject. Considering this context and its challenges, the question is: what are the guidelines that can guide the systematization of nursing care in aging in Primary Health Care (PHC). Objective: To analyze how nurses perform the nursing process in caring for individual health needs in aging in Primary Health Care. Method: Qualitative approach. The research participants were nurses from the Family Health Strategy. Data collection occurred in two phases. In first phase, a focus group was carried out, analyzed by the Discourse of the Collective Subject. In 2nd phase, a Knowledge Production and Problem-Solving Workshop was developed. Results: The care in aging process is still focused on the elderly and on disease; however, depending on the nurse's training, it is possible to promote care in a perspective focused on promotion and protection, but it is necessary to use protocols to guide this practice. The question is how the municipality is organizing the health care network to promote this care’s integrality. It is possible to do using the Citizen's Electronic Health Record, but there are professionals who do not perform the record, which hinders the continuity of care. The workshop delimited proposals for the construction of care in aging from the perspective of integrality, focusing on shared management as a form of collective construction of changes in professional practice and for activities with the community. Final Considerations: The methodology adopted allowed us to promote reflection on the professional practice of nurses in PHC and its developments in face of population aging process.
