Este trabalho de investigação teve como objetivos, descrever e caraterizar Agrimonia eupatoria L. em termos das suas propriedades farmacológicas e interações medicamentosas. Tendo em conta os constituintes, são várias as ações referidas para esta espécie: antidiabética, anti-tumoral, antibacteriana, antiviral, dermatológica, gastrointestinal, diurética, metabólica, antidiarreica, colagoga, hepatoprotectora, analgésica, hipotensora, imunomoduladora, urolítica, cardioprotectora, antirreumática, antioxidante, anti-inflamatória e anticoagulante. Quando ingeridos em doses excessivas, os constituintes da planta podem interagir com as terapias que recorrem ao uso de anticoagulantes e anti- hipertensores e, na generalidade, com a ação de fármacos metabolizados no fígado. Foram encontradas interações farmacocinéticas do tipo sinergismo de potenciação com anticoagulantes e medicamentos anti-hipertensivos. Apesar da intensificação de pesquisas acerca das plantas medicinais ainda se desconhecem muitas das suas interações com outros medicamentos. Ainda assim, a maioria dos utilizadores não relata o seu uso ao profissional de saúde. Tal como é defendido por vários investigadores, na prática clínica, questionar os pacientes sobre o uso de plantas medicinais e fitoterápicos é determinante para prevenir de interações planta-medicamento.]]></p></abstract>

This research aimed to describe and characterize the Agrimonia eupatoria L. about pharmacological properties and medicinal interactions. Taking into account the constituents, the plant exhibits pharmacologic and therapeutic action, anti-diabetic, antitumor, antibacterial, antiviral, dermatological, gastrointestinal, diuretic, metabolic, antidiarrheal, bile duct, hepatoprotective, analgesic, antihypertensive, immunomodulatory, urolithic, cardioprotective, antirheumatic, antioxidant, anti-inflammatory and anticoagulant. When taken in excessive doses, the constituents of the plant may interact with therapies that use anticoagulation and antihypertensive therapy and, in general, with the action of other medicaments metabolized in the liver. Pharmacokinetic interactions of the potentiation synergism type were found with anticoagulants and antihypertensive medicines. Despite the intensification of research on medicinal plants, many of their interactions with other medicaments are still unknown.     <p><font face = Verdana size = 3><b>INTRODUÇÃO</b></font></p>


     <p><font face = Verdana size = 2>A utilização popular de plantas medicinais baseia-se
num saber milenário, transmitido ao longo das gerações, constituindo na maioria
das vezes o único recurso em termos de cuidados médicos, curativos ou preventivos
(Neves <i>et al</i>., 2008). Nos últimos anos observa-se, a nível mundial, um aumento
considerável do uso de plantas medicinais naturais nos cuidados de saúde primários
(Ahmed <i>et al</i>., 2016). Incentivado pelo aparecimento de novas doenças que
ainda não apresentam tratamento adequado; pela propagação da ideia do que é “natural”
é sempre saudável; pela perceção de que os medicamentos naturais são superiores
aos sintéticos; pelas dificuldades de acesso aos serviços de saúde e pelos elevados
preços dos remédios alopáticos. A cultura popular na utilização de plantas medicinais,
trazida através dos tempos, contribui para o uso indiscriminado de plantas medicinais
dentro do contexto da automedicação. Para além disso, o fácil acesso às plantas
medicinais incentiva a procura do “medicamento” mais acessível economicamente (Nicoletti
<i>et al.</i>, 2007).</font></p>

    <p><font face = Verdana size = 2>Este trabalho
teve como objetivos descrever e caraterizar <i>Agrimonia eupatoria </i>L. em termos
das suas propriedades farmacológicas e interações medicamentosas. Para a elaboração
deste trabalho realizou-se um levantamento bibliográfico em bases de dados, nomeadamente,
Medscape, Infomed, PubMed, MEDLINE, Scopus, RCAAP e SCIELO. Foram utilizados com
maior frequência, os descritores, <i>Agrimonia, eupatoria</i>, interações, constituintes
e propriedades farmacológicas. </font></p>



     <p><font face = Verdana size = 2><i>Distribuição </i></font></p>

    <p><font face
= Verdana size = 2><i>Agrimonia eupatoria</i> é uma planta herbácea, autóctone da
Europa do Norte e Central, Ásia temperada e América do Norte (Ahmed <i>et al</i>.,
2016). É, frequentemente, encontrada em todos os terrenos, em especial nos argilosos,
desde que bem expostos ao sol. Cresce em solos húmidos e/ou moderadamente secos
(Ivanova <i>et al</i>., 2011). Geralmente é encontrada em áreas montanhosas do Paquistão,
colinas da Pérsia, Sibéria, Java, América do Norte, Roma e Índia (Khan <i>et al.</i>,
2013). Na europa encontra-se por toda a parte (Forey e Lindsay, 1997) e, em Portugal,
existe em quase todo o país (Proença da Cunha <i>et al</i>., 2002; Proença da Cunha,
2005; Araújo <i>et al.</i>, 2014).<b></b></font></p>


    <p><font face = Verdana size = 2><i>Caraterização
botânica</i></font></p>

    <p><font face = Verdana size = 2>A espécie <i>Agrimonia
eupatoria</i>, vulgarmente conhecida como agrimónia, pertence à família <i>Rosaceae</i>.
A planta inteira é delgada, ereta e perene. A haste floral pode medir até cerca
de 60 a 90&nbsp;cm de altura e é pouco ramificada (<a href = "/img/revistas/rca/v40nspe/v40nspea33f1.jpg" target = "_blank">Figura 1A</a>) (Khan <i>et al</i>.,
2013; Al-Snafi, 2015a). Possui folhas pinadas e dentadas (<a href = "/img/revistas/rca/v40nspe/v40nspea33f1.jpg" target = "_blank">Figura 1B</a>) e flores amarelas
no terminal (Araújo <i>et al.</i>, 2014) A planta, de cor verde-escura, está coberta
com pelos sedosos e, quando cortada, exala um odor aromático peculiar, mas agradável
(Proença da Cunha <i>et al.</i>, 2002; INFARMED, 2009; Khan <i>et al</i>., 2013;
Araújo <i>et al.</i>, 2014; Al-Snafi, 2015a). As folhas medem entre 6 a 25&nbsp;cm
e podem possuir entre 3 a 6 pares de folíolos principais e, entre eles, folíolos
mais pequenos, diferentes em tamanho. Os folículos principais medem cerca de 8 a
75&nbsp;mm por 8 a&nbsp;35 mm, são elípticos a largamente obovados, grosseiramente
crenados a serrados, de cor verde-escura na parte superior e esbranquiçados ou acinzentado-tomentoso
na parte inferior (Proença da Cunha <i>et al</i>., 2002; Proença da Cunha, 2005;
INFARMED, 2009; Khan <i>et al</i>., 2013; Araújo <i>et al.</i>, 2014; Al-Snafi,
2015a).</font></p>

<body><![CDATA[<p><font face = Verdana size = 2>As flores, pentâmeras, são
em grande número, medem cerca de 6,0&nbsp;mm de diâmetro e são de cor amarelada.
Aparecem entre julho e agosto, as pétalas são livres e caducas (<a href = "/img/revistas/rca/v40nspe/v40nspea33f1.jpg" target = "_blank">Figura 1C</a>) (Proença
da Cunha <i>et al</i>., 2002; INFARMED, 2009; Khan <i>et al</i>., 2013; Araújo <i>et
al.</i>, 2014; Al-Snafi, 2015a). </font></p>

    
<p><font face = Verdana size = 2>Os
frutos são aquénios fechados num cálice duro e espinhoso (hipanto) e quando maduros
medem cerca de 6 a 7,5&nbsp;mm por 5 a 6&nbsp;mm, são em forma obcónica a turbinada,
profundamente côncavos, com sulcos e com pelos longos (<a href = "/img/revistas/rca/v40nspe/v40nspea33f1.jpg" target = "_blank">Figura 1D</a>) (Proença da Cunha
<i>et al</i>., 2002; Proença da Cunha, 2005; INFARMED, 2009; Khan <i>et al</i>.,
2013; Araújo <i>et al.</i>, 2014; Al-Snafi, 2015a). </font></p>

    
<p><font face = Verdana size = 2><i>Constituintes da planta</i></font></p>

    <p><font face =
Verdana size = 2>A ação farmacológica e terapêutica de um fármaco vegetal depende
dos constituintes químicos presentes na planta. A análise fitoquímica de <i>A. eupatoria
</i>revelou a presença de vários constituintes entre os quais se destacam: hidrocarbonetos
aromáticos, polissacáridos e compostos fenólicos onde se incluem ácidos fenólicos,
como o ácido silícico e o ácido salicílico, cumarinas, lactonas, flavonóides e taninos.
O material fresco contém <i>ácidos</i> gordos polinsaturados como os ácidos oleico,
linoleico e linolénico e ácidos gordos saturados como os ácidos palmítico, esteárico
e cetílico, além de fitoesteróis. Encontram-se ainda aminoácidos como o ácido aspártico,
glicina, alanina, valina e lisina e vitaminas C, B e K (Feldman <i>et al</i>., 2000;
Feldman e Lawlor, 2000; Copland <i>et al</i>., 2003; Al-Snafi, 2015a).</font></p>



    <p><font face = Verdana size
= 2><i>Atividade farmacológica</i></font></p>

    <p><font face = Verdana size =
2>As partes da planta mais frequentemente utilizadas são as partes aéreas, folhas
e flores, usadas em infusões, decocções ou cataplasmas. A planta apresenta atividade
farmacológica, anti-diabética, anti-tumoral, antibacteriana, antiviral, dermatológica,
gastrointestinal, diurética, metabólica (anti-obesidade), antidiarreica, colagoga,
hepatoprotectora, analgésica, hipotensora, imunomoduladora, uricolítica, cardioprotectora,
antirreumática, antioxidante, anti-inflamatória e anticoagulante (<a href = "/img/revistas/rca/v40nspe/v40nspea33q1.jpg" target = "_blank">Quadro 1</a>).</font></p>

    
<p><font face = Verdana size = 2><i>Interações medicamentosas</i></font></p>

    <p><font face = Verdana size = 2>No <a href = "/img/revistas/rca/v40nspe/v40nspea33q2.jpg" target = "_blank">Quadro 2</a> constam as interações farmacocinéticas
de potenciação com fármacos comercializados em Portugal, em que é aconselhável o
uso com precaução. Neste tipo de interação é recomendado a vigilância contínua do
doente e no caso de ser necessário alterar a terapêutica uma vez que existe o risco
de ocorrerem hemorragias ou complicações trombolíticas (Chevallier, 1996; Dharmananda,
2000; Cardoso <i>et al.,</i> 2009; Ge <i>et al</i>., 2014). Por outro lado, uma
vez que <i>A. eupatoria</i> tem ação hipotensora, existe o risco de haver dificuldade
no controlo da diurese e de hipotensão.</font></p>



    
<p><font
face = Verdana size = 2>No <a href = "/img/revistas/rca/v40nspe/v40nspea33q3.jpg" target = "_blank">Quadro 3</a> referem-se os fármacos comercializados em Portugal
que, quando usados em simultâneo com <i>A. eupatoria</i>, podem originar interações
farmacocinéticas de potenciação, de risco menor. Embora se tenha que ter precaução
no seu uso, uma constante vigilância do doente não é necessária, contrariamente,
ao que acontece com os fármacos que constam do <a href = "/img/revistas/rca/v40nspe/v40nspea33q2.jpg" target = "_blank">Quadro 2</a>.</font></p>

    
<p><font face
= Verdana size = 2>Foi, ainda, identificada uma interação farmacocinética da planta
com o Meloxicam, com a classificação farmacoterapêutica: oxicans. Nesta interação,
regista-se diminuição da atividade anticoagulante, muito embora esteja, ainda, pouco
claro o seu mecanismo.</font></p>

    ]]></body>
<body><![CDATA[<p><font face = Verdana size = 2>Em relação
às interações farmacodinâmicas, e tendo em conta os fármacos comercializados em
Portugal, apenas se conhecem os efeitos de <i>A. eupatoria</i> com o do Captopril,
inibidor da enzima de conversão da angiotensina. Ambos diminuem a glicemia e a pressão
sanguínea. É, por isso, aconselhável monitorizar/controlar a glicemia e a pressão
arterial dos doentes.<b></b></font></p>

    <p><font face = Verdana size = 2>Tal
como acontece com outras plantas ricas em taninos, estes constituintes de <i>A.
eupatoria </i>podem, ainda, interferir com a ação de fármacos metabolizados no fígado
potenciando o seu efeito (Schabana <i>et al</i>, 2003).</font></p>

    <p><font face = Verdana size = 2><i>Efeitos
colaterais</i></font></p>

    <p><font face = Verdana size = 2>Os efeitos colaterais
da planta incluem hipotensão, arritmia, náuseas, vómitos e, até, paragem cardíaca.
Por ter como constituintes taninos, estes podem causar distúrbios digestivos (Al-Snafi,
2015a; Medscape, 2017). Alegadamente, também, pode afetar o ciclo menstrual (Schabana
<i>et al</i>., 2003; Al-Snafi, 2015a). Por outro lado, tendo em conta a falta de
informação sobre a sua toxicidade, o seu uso deve ser evitado durante a gravidez
e a lactação (Schabana <i>et al</i>., 2003; Medscape, 2017).Quando ingerida em doses
elevadas pode causar obstipação e insuficiência hepática/renal, bem como problemas
gastrointestinais (Al-Snafi, 2015a; Medscape, 2017). Existe, também, a possibilidade
do seu consumo em doses elevadas aumentar o risco de cancro da língua e lábios.
Apesar destes efeitos nocivos, não foram reportadas quaisquer reações adversas graves
(Al-Snafi, 2015a; Medscape, 2017).</font></p>

    <p><font face = Verdana size = 3><b>CONCLUSÃO</b></font></p>

     <p><font face = Verdana size = 2>Por dificuldades no acesso aos cuidados de saúde,
pela persistência da ideia de que tudo o que é natural é saudável, as plantas são
amplamente utilizadas no tratamento e na prevenção de doenças. Apesar da intensificação
de pesquisas acerca das plantas medicinais ainda se desconhecem muitas das suas
interações com outros medicamentos. Ainda assim, a maioria dos utilizadores não
relata o seu uso ao profissional de saúde. Tal como é defendido por vários investigadores,
na prática clínica, questionar os pacientes sobre o uso de plantas medicinais e
fitoterápicos é determinante para a prevenção de interações planta-medicamento.
