Este artigo apresenta uma breve revisão de literatura da teoria e investigação da vinculação na infância e pretende contribuir para uma perspectiva desenvolvimental na prática clínica. A investigação na vinculação tem-nos esclarecido, sobre os efeitos nas crianças de experiências precoces associadas a: (a) rotinas de cuidados fora de casa; (b) separações; (c) disfunções significativas de prestação de cuidados; (d) características das crianças com necessidades especiais. Apresentamos ainda orientações para a prática clínica dada a centralidade dos aspectos relacionais no desenvolvimento saudável das crianças.]]></p></abstract>

We also present guidelines to clinical intervention since relational issues are central to  infants&rsquo; healthy development.</p>     <p><b>Key words</b>: Attachment, Attachment disruption, Special needs.</p>     <p>&nbsp;</p>     <p><b>Introdu&ccedil;&atilde;o</b></p>     <p>A vincula&ccedil;&atilde;o &eacute; um tipo espec&iacute;fico de um conjunto mais vasto de liga&ccedil;&otilde;es que Bowlby e Ainsworth  designaram por &ldquo;liga&ccedil;&otilde;es afectivas&rdquo;. Segundo Ainsworth (1985) o conceito de vincula&ccedil;&atilde;o pode ser definido  de acordo com um conjunto de crit&eacute;rios, segundo os quais esta liga&ccedil;&atilde;o afectiva: (a) &eacute; persistente e n&atilde;o  transit&oacute;ria; (b) envolve uma figura espec&iacute;fica e reflete uma atra&ccedil;&atilde;o que um indiv&iacute;duo tem por outro  indiv&iacute;duo; (c) &eacute; uma rela&ccedil;&atilde;o emocionalmente significativa; (d) o indiv&iacute;duo deseja manter a proximidade ou  contacto com essa figura, ainda que tal possa variar em fun&ccedil;&atilde;o de v&aacute;rios factores (e.g., idade, o estado do indiv&iacute;duo  ou as condi&ccedil;&otilde;es do meio); (e) o indiv&iacute;duo experiencia uma certa perturba&ccedil;&atilde;o face a uma situa&ccedil;&atilde;o de  separa&ccedil;&atilde;o involunt&aacute;ria e, sobretudo, quando deseja a proximidade e tal n&atilde;o lhe &eacute; poss&iacute;vel; (f) existe uma  procura de seguran&ccedil;a e de conforto na rela&ccedil;&atilde;o com essa pessoa. Este &uacute;ltimo crit&eacute;rio &eacute; determinante para a  refer&ecirc;ncia e distin&ccedil;&atilde;o entre &ldquo;Liga&ccedil;&otilde;es parentais&rdquo; e &ldquo;Vincula&ccedil;&atilde;o das  crian&ccedil;as aos pais&rdquo; (Cassidy, 1999).</p>     <p>Bowlby (1982) teorizou a vincula&ccedil;&atilde;o como sendo um sistema motivacional, que tem como objectivo a prote&ccedil;&atilde;o e o  reconforto da crian&ccedil;a. Enquanto sistema motivacional ele &eacute; complementar do sistema de explorat&oacute;rio da crian&ccedil;a e do  sistema de <i>caregiving</i> dos progenitores (i.e., presta&ccedil;&atilde;o de cuidados). Ainsworth (1973) introduziu um conceito de &ldquo;base  segura&rdquo;, que nos ajuda a compreender o comportamento explorat&oacute;rio da crian&ccedil;a. A figura de vincula&ccedil;&atilde;o assume o  papel de &ldquo;base segura&rdquo; quando se apresenta na rela&ccedil;&atilde;o com a crian&ccedil;a como estando dispon&iacute;vel e em  proximidade, de forma a poder assegurar a sua protec&ccedil;&atilde;o, sempre que necess&aacute;rio, e quando solicitado.</p>     <p>Ao longo do desenvolvimento, as crian&ccedil;as interiorizam aspectos importantes da experi&ecirc;ncia relacional vivida com as figuras de  vincula&ccedil;&atilde;o, organizando-a em representa&ccedil;&otilde;es internas (Ver&iacute;ssimo, Silva, Santos, &amp; Gu&eacute;deney, 2016).  Estes esquemas relacionais, ou &ldquo;modelos internos operantes&rdquo;, incluem representa&ccedil;&otilde;es sobre o eu, o outro, e a  rela&ccedil;&atilde;o (Bowlby, 1982).</p>     <p>A avalia&ccedil;&atilde;o da qualidade da rela&ccedil;&atilde;o de vincula&ccedil;&atilde;o na crian&ccedil;a, pode realizar-se em  laborat&oacute;rio atrav&eacute;s do Paradigma da Situa&ccedil;&atilde;o Estranha (12-18 meses). Este sistema de avalia&ccedil;&atilde;o &eacute;  baseado na observa&ccedil;&atilde;o e cota&ccedil;&atilde;o de padr&otilde;es (ao inv&eacute;s de frequ&ecirc;ncia ou dura&ccedil;&atilde;o) de  comportamentos interativos da crian&ccedil;a com a figura de vincula&ccedil;&atilde;o (i.e., procura de proximidade, manuten&ccedil;&atilde;o de  contacto, resist&ecirc;ncia ao contacto e evitamento de aproxima&ccedil;&atilde;o) durante os epis&oacute;dios de reuni&atilde;o, ap&oacute;s duas  breves separa&ccedil;&otilde;es. As d&iacute;ades crian&ccedil;a-figura de vincula&ccedil;&atilde;o observadas neste contexto laboratorial  s&atilde;o classificadas numa de quatro categorias: segura, insegura-evitante, insegura-ambivalente/resistente (Ainsworth, Blehar, Waters, &amp;  Wall, 1978) ou desorganizada/desorientada (Main &amp; Solomon, 1990). A avalia&ccedil;&atilde;o da qualidade da vincula&ccedil;&atilde;o da  crian&ccedil;a (12-48 meses) pode tamb&eacute;m ser realizada, tendo em conta um continuo de Inseguran&ccedil;a-Seguran&ccedil;a, desta feita  atrav&eacute;s de observa&ccedil;&otilde;es em contexto natural&iacute;stico ao domic&iacute;lio, segundo o m&eacute;todo do &ldquo;Attachment  Q-Sort&rdquo; (Waters &amp; Deane, 1985).</p>     <p>Mais tarde (3-9 anos), com a complexifica&ccedil;&atilde;o dos processos cognitivos e o desenvolvimento das capacidades lingu&iacute;sticas,  torna-se poss&iacute;vel o acesso ao mundo interno da crian&ccedil;a e, consequentemente, &agrave;s suas representa&ccedil;&otilde;es da  vincula&ccedil;&atilde;o que podem agora ser avaliadas atrav&eacute;s das suas narrativas. Um dos m&eacute;todos de avalia&ccedil;&atilde;o das  representa&ccedil;&otilde;es de vincula&ccedil;&atilde;o para crian&ccedil;as em idade pr&eacute;-escolar mais utilizado &eacute; o  &ldquo;Attachment Story Completion Task&rdquo; (Bretherton, Ridgeway, &amp; Cassidy, 1990) que disp&otilde;e de diferentes metodologias de  cota&ccedil;&atilde;o (Maia, Ver&iacute;ssimo, Ferreira, Silva, &amp; Fernandes, 2009; Miljkovitch, Pierrehumbert, Karmaniola, &amp; Halfon, 2003).  O &ldquo;MacArthur Story Stem Battery&rdquo; (Bretherton &amp; Oppenheim, 2003) e o &ldquo;Manchester Child Attachment Story&rdquo; (Green,  Stanley, Smith, &amp; Goldwyn, 2000) s&atilde;o exemplos de outros. Como referido, estes m&eacute;todos privilegiam uma cota&ccedil;&atilde;o  quantitativa da vincula&ccedil;&atilde;o atrav&eacute;s de um &iacute;ndice cont&iacute;nuo de Inseguran&ccedil;a-Seguran&ccedil;a.</p>     ]]></body>
<body><![CDATA[<p>De acordo com Bowlby (1982), se um desenvolvimento favor&aacute;vel da vincula&ccedil;&atilde;o &eacute; tido como importante para a  sa&uacute;de mental, torna-se imperativo distinguir um desenvolvimento favor&aacute;vel de outro desfavor&aacute;vel, bem como conhecer que  condi&ccedil;&otilde;es promovem um e outro. A hip&oacute;tese de Bowlby de que diferen&ccedil;as na presta&ccedil;&atilde;o de cuidados d&atilde;o  origem a diferen&ccedil;as na qualidade da vincula&ccedil;&atilde;o, foi posteriormente aprofundada por Ainsworth atrav&eacute;s do conceito de  &ldquo;Sensibilidade Materna&rdquo; (Ainsworth, 1967; Ainsworth et al., 1978; Atkinson et al., 2000; De Wolff &amp; Van IJzendoorn, 1997). Assim, a  sensibilidade da resposta do prestador de cuidados aos sinais emitidos pela crian&ccedil;a, durante o primeiro ano de vida, encontra-se associada  &agrave; qualidade da rela&ccedil;&atilde;o de vincula&ccedil;&atilde;o avaliada tanto em contexto familiar como laboratorial: uma resposta pronta  e eficaz aos sinais do beb&eacute; est&aacute; associada a uma vincula&ccedil;&atilde;o segura na Situa&ccedil;&atilde;o Estranha (aos 12 meses) e  a um menor n&uacute;mero de comportamentos mal-adaptativos (e.g., choro excessivo e explora&ccedil;&atilde;o empobrecida) (Ainsworth et al., 1978).</p>     <p>Pelo contr&aacute;rio, um cuidado menos sens&iacute;vel no contexto familiar (i.e., atraso nas respostas &agrave;s solicita&ccedil;&otilde;es do  beb&eacute; e comportamento menos afectuoso) tem sido relacionado com uma vincula&ccedil;&atilde;o insegura (Ainsworth et al., 1978; Blehar,  Lieberman, &amp; Ainsworth, 1977; Posada et al., 1999). Enquanto a presta&ccedil;&atilde;o de cuidados caracterizada pela indisponibilidade  emocional e rejei&ccedil;&atilde;o cr&oacute;nica tem sido relacionada com uma vincula&ccedil;&atilde;o insegura-evitante (Main, 1981), cuidados  simultaneamente intrusivos e n&atilde;o responsivos, n&atilde;o sendo congruentes e fi&aacute;veis, t&ecirc;m sido associados a uma  vincula&ccedil;&atilde;o insegura-ambivalente/resistente.</p>     <p>Outros estudos t&ecirc;m confirmado a liga&ccedil;&atilde;o entre o padr&atilde;o de vincula&ccedil;&atilde;o desorganizado e um conjunto de  comportamentos parentais assustadores, assustados ou confusionais (Jacobitz, Hazen, &amp; Riggs, 1997; Lyons-Ruth, Bronfman, &amp; Parsons, 1999;  Schuengel, Bakermans-Kranenburg, Van Ijzendoorn, &amp; Blom, 1999). Quando comparamos crian&ccedil;as que vivem em popula&ccedil;&otilde;es  consideradas de alto e baixo risco, as primeiras t&ecirc;m maior tend&ecirc;ncia a formar vincula&ccedil;&otilde;es desorganizadas e menos seguras  (Cyr, Euser, Bakermans-Kranenburg, &amp; Van Ijzendoorn, 2010). Segundo Cyr e colegas (2010) os maus tratos est&atilde;o fortemente associados  &agrave; desorganiza&ccedil;&atilde;o mas as crian&ccedil;as que vivem em situa&ccedil;&otilde;es de alto risco psicossocial t&ecirc;m igualmente  mais probabilidade de desenvolver uma vincula&ccedil;&atilde;o insegura e desorganizada, do que as crian&ccedil;as que vivem em contexto de baixo  risco, e isto independentemente de a este risco se associar ou n&atilde;o ocorr&ecirc;ncia de maus tratos. Uma ideia importante a reter &eacute;  que a acumula&ccedil;&atilde;o de factores de risco afecta a disponibilidade e a sensibilidade parental de tal forma que parece ter um impacto  t&atilde;o negativo na seguran&ccedil;a da vincula&ccedil;&atilde;o na crian&ccedil;a como os maus-tratos.</p>     <p>De sublinhar que alguns beb&eacute;s, por terem um temperamento muito exigente, podem colocar em risco o sistema de presta&ccedil;&atilde;o de  cuidados ao tornarem muito dif&iacute;cil a tarefa de <i>caregiving</i> dos pais (Egeland &amp; Farber, 1984). No entanto, a  investiga&ccedil;&atilde;o tem demonstrado que, uma vez proporcionado um suporte adequado aos progenitores, os cuidado sens&iacute;veis permitem  enfrentar melhor as dificuldades destas crian&ccedil;as (Crokenberg, 1981; Susman-Stillman, Kalkowske, Egeland, &amp; Waldman, 1996). Al&eacute;m  disso, temperamento e vincula&ccedil;&atilde;o t&ecirc;m sido cada vez menos associados de maneira linear para serem considerados como factores  independentes que influenciam, de maneira diferente e especifica, as interac&ccedil;&otilde;es interpessoais (Vaughn, Bost, &amp; Van IJzendoorn,  2008). O risco de uma vincula&ccedil;&atilde;o insegura parece advir de uma combina&ccedil;&atilde;o espec&iacute;fica das elevadas  exig&ecirc;ncias do beb&eacute; associadas a dificuldades inerentes ao prestador de cuidados (e.g., propens&atilde;o do beb&eacute; para a  perturba&ccedil;&atilde;o e uma presta&ccedil;&atilde;o de cuidados exageradamente r&iacute;gida) (Gunnar, 2001; Mangelsdorf, Gunnar,  Kestenbaum, Lang, &amp; Andreas, 1990).</p>     <p>&nbsp;</p>     <p><i>Disfun&ccedil;&otilde;es na forma&ccedil;&atilde;o das rela&ccedil;&otilde;es de vincula&ccedil;&atilde;o</i></p>     <p>As disfun&ccedil;&otilde;es na presta&ccedil;&atilde;o de cuidados variam desde as de rotina (e.g., cuidados em infant&aacute;rios) a muito  graves (e.g., coloca&ccedil;&otilde;es m&uacute;ltiplas em fam&iacute;lias de acolhimento tempor&aacute;rio e institucionaliza&ccedil;&atilde;o),  com consequ&ecirc;ncias bem distintas. Em geral, os seus efeitos dependem do momento, dura&ccedil;&atilde;o da disfun&ccedil;&atilde;o e da  extens&atilde;o em que a disponibilidade emocional do prestador de cuidados est&aacute; comprometida.</p>     <p>&nbsp;</p>     <p><i>Sistemas de guarda em colectividade</i>. A rotina da presta&ccedil;&atilde;o de cuidados em infant&aacute;rios parece ter, regra geral,  consequ&ecirc;ncias moderadas. Quando o cuidado substituto durante o primeiro ano de vida se restringe a ser em <i>part-time</i>, est&aacute;vel e  de elevada qualidade (i.e., n&atilde;o mais de tr&ecirc;s crian&ccedil;as por adulto), n&atilde;o foram demonstradas consequ&ecirc;ncias negativas  (Sroufe &amp; Ward, 1984). No entanto, cuidados precoces prestados em infant&aacute;rios a <i>full-time</i>, podem colocar alguns riscos, sendo que  os resultados negativos mais evidentes ocorrem na presen&ccedil;a simult&acirc;nea de cuidados de reduzida qualidade em casa e fora de casa  (Belsky, 1990, 2001, 2004, 2009, 2010).</p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p><i>Hospitaliza&ccedil;&otilde;es</i>. As disfun&ccedil;&otilde;es de presta&ccedil;&atilde;o de cuidados na inf&acirc;ncia mais graves incluem  as experi&ecirc;ncias de hospitaliza&ccedil;&atilde;o, separa&ccedil;&otilde;es, experi&ecirc;ncias de adop&ccedil;&atilde;o e de fam&iacute;lias  de acolhimento tempor&aacute;rio. Robertson e Robertson, em colabora&ccedil;&atilde;o com Bowlby foram os primeiros a documentar os efeitos no  bem-estar da crian&ccedil;a da separa&ccedil;&atilde;o crian&ccedil;a-progenitor nas hospitaliza&ccedil;&otilde;es em idades precoces (Robertson,  1953; Robertson &amp; Robertson, 1971). A evid&ecirc;ncia de sinais de luto nos beb&eacute;s (i.e., protesto, desespero e  desvincula&ccedil;&atilde;o) ap&oacute;s separa&ccedil;&otilde;es prolongadas alertou os profissionais de sa&uacute;de para esta grave  problem&aacute;tica e gerou mudan&ccedil;as significativas nas pr&aacute;ticas e pol&iacute;ticas pedi&aacute;tricas, em v&aacute;rios  pa&iacute;ses. Apesar destas descobertas e das liga&ccedil;&otilde;es encontradas entre experi&ecirc;ncias de hospitaliza&ccedil;&atilde;o precoces  e posteriores perturba&ccedil;&otilde;es de comportamento (Douglas, 1975; Quinton &amp; Rutter, 1976), poucos estudos t&ecirc;m examinado o impacto  da hospitaliza&ccedil;&atilde;o no desenvolvimento das rela&ccedil;&otilde;es de vincula&ccedil;&atilde;o (Van Der Horst &amp; Van Der Veer, 2009).</p>     <p>&nbsp;</p>     <p><i>Adop&ccedil;&atilde;o</i>. De forma consistente com uma abordagem desenvolvimentista da vincula&ccedil;&atilde;o, a seguran&ccedil;a na  rela&ccedil;&atilde;o de vincula&ccedil;&atilde;o em casos de adop&ccedil;&atilde;o tem sido relacionada com o momento da transi&ccedil;&atilde;o e  com a qualidade dos cuidados prestados. Os estudos realizados neste &acirc;mbito sugerem que os beb&eacute;s adoptados nos primeiros meses de vida  t&ecirc;m uma elevada probabilidade de desenvolverem rela&ccedil;&otilde;es de vincula&ccedil;&atilde;o seguras quase equivalente aos beb&eacute;s  n&atilde;o adoptados (Norhaus &amp; Solnit, 1990; Singer, Brodzinsky, Ramsay, Steir, &amp; Waters, 1985; Van den Dries, Juffer, Van IJzendoorn,  &amp; Bakermans-Kranenburg, 2009). Especialmente, quando o ambiente &eacute; caloroso, consistente e contingente &agrave;s necessidades do  beb&eacute;, a vincula&ccedil;&atilde;o em situa&ccedil;&atilde;o de adop&ccedil;&atilde;o constru&iacute;da desde cedo no processo de  desenvolvimento, difere pouco da que &eacute; estabelecida entre progenitores e os seus filhos biol&oacute;gicos (Singer et al., 1985). As  crian&ccedil;as adoptadas em idades tardias, no entanto, t&ecirc;m maior probabilidade de apresentar dificuldades de ajustamento socioemocional e  comportamental (Feigelman &amp; Silverman, 1983; Tizard, 1977; Tizard &amp; Hodges, 1978). Estas dificuldades nas adop&ccedil;&otilde;es tardias  s&atilde;o multideterminadas e incluem as experi&ecirc;ncias de pr&eacute;-adop&ccedil;&atilde;o e outros factores relacionados com o momento da  adop&ccedil;&atilde;o (i.e., experi&ecirc;ncia da crian&ccedil;a e compreens&atilde;o da separa&ccedil;&atilde;o e da perda). No entanto, alguns  estudos apontam para a influ&ecirc;ncia das representa&ccedil;&otilde;es maternas na capacidade da crian&ccedil;a em estabelecer uma  rela&ccedil;&atilde;o segura. A idade n&atilde;o seria um factor determinante para a seguran&ccedil;a da vincula&ccedil;&atilde;o, mas sim a  qualidade das experi&ecirc;ncias que a crian&ccedil;a ter&aacute; com os pais adoptivos (Ver&iacute;ssimo &amp; Salvaterra, 2006). Uma  traject&oacute;ria de rupturas relacionais experienciada at&eacute; ao momento da adop&ccedil;&atilde;o, caracterizada por m&uacute;ltiplos  cuidadores e v&aacute;rias perdas de figuras de vincula&ccedil;&atilde;o (e.g., hist&oacute;rias relacionais anteriores inconsistentes e  hist&oacute;rias de institucionaliza&ccedil;&otilde;es), podem afectar negativamente a capacidade da crian&ccedil;a se vincular de forma segura aos  pais adoptivos.</p>     <p>&nbsp;</p>     <p><i>Fam&iacute;lias de acolhimento tempor&aacute;rio</i>. A ruptura relacional seguida de coloca&ccedil;&atilde;o em fam&iacute;lias de  acolhimento tempor&aacute;rio foi associada a problemas emocionais e comportamentais (e.g., comportamentos hetero e auto-agressivos, procura de  aten&ccedil;&atilde;o, hiperactividade e agita&ccedil;&atilde;o para comer). Atualmente, est&atilde;o a decorrer investiga&ccedil;&otilde;es sobre  os efeitos da presta&ccedil;&atilde;o de cuidados no desenvolvimento da organiza&ccedil;&atilde;o de vincula&ccedil;&atilde;o, em fam&iacute;lias  de acolhimento tempor&aacute;rio. As descobertas iniciais confirmam os efeitos comprometedores de experi&ecirc;ncias precoces de maus-tratos (e.g.,  abuso e neglig&ecirc;ncia emocional e f&iacute;sica), separa&ccedil;&atilde;o, perda e arranjos m&uacute;ltiplos de presta&ccedil;&atilde;o de  cuidados (Fisher, Gunnar, Chamberlain, &amp; Reid 2000; Stovall &amp; Dozier, 2000). Perturba&ccedil;&otilde;es traum&aacute;ticas na  presta&ccedil;&atilde;o de cuidados parecem levar a distor&ccedil;&otilde;es nas rela&ccedil;&otilde;es de vincula&ccedil;&atilde;o e ao  desenvolvimento de comportamentos mal-adaptativos (e.g., sociabilidade indiscriminada e terror a estranhos) e a perturba&ccedil;&otilde;es na  percep&ccedil;&atilde;o e interpreta&ccedil;&atilde;o de pistas sociais (Stovall &amp; Dozier, 2000). Nowacki e Schoelmerich (2010) falam dos  efeitos ben&eacute;ficos, a n&iacute;vel do desenvolvimento psicol&oacute;gico da crian&ccedil;a, da coloca&ccedil;&atilde;o em fam&iacute;lias de  acolhimento est&aacute;veis quando comparada &agrave; institucionaliza&ccedil;&atilde;o. No entanto, no estudo de Laurence, Carlson e Egeland  (2006), foi verificado que o desenvolvimento psicossocial de um grupo de crian&ccedil;as maltratadas e mantidas na fam&iacute;lia biol&oacute;gica  se mostrou equivalente ao do grupo das crian&ccedil;as maltratadas e colocadas em fam&iacute;lia de acolhimento. Os autores explicam que esta  alternativa s&oacute; &eacute; ben&eacute;fica &agrave;s crian&ccedil;as, se a elevada qualidade relacional for assegurada nas fam&iacute;lias de  acolhimento.</p>     <p>&nbsp;</p>     <p><i>Institucionaliza&ccedil;&atilde;o em colectividade</i>. A profunda import&acirc;ncia da conex&atilde;o afectiva entre os  beb&eacute;s e os seus cuidadores adultos &eacute; ilustrada quer por pesquisas cl&aacute;ssicas, quer pela investiga&ccedil;&atilde;o recente  relativa ao estudo dos efeitos da institucionaliza&ccedil;&atilde;o no desenvolvimento das crian&ccedil;as. Os estudos cl&aacute;ssicos de Spitz  (1946), evidenciaram que o desenvolvimento perceptivo, motor, cognitivo e emocional do beb&eacute; ocorre atrav&eacute;s da intera&ccedil;&atilde;o  privilegiada com um adulto, prestador de cuidados prim&aacute;rios, sendo que a falta de um relacionamento com um parceiro humano pode ser um  mecanismo de desintegra&ccedil;&atilde;o desenvolvimental. De forma consistente com os estudos iniciais, as investiga&ccedil;&otilde;es recentes,  com crian&ccedil;as que estiveram institucionalizadas (Chisholm, 1998; Kaler, &amp; Freeman, 1994; Marcovitch, Goldberg, Gold, Washington et al.,  1997; O&rsquo;Connor, Bredenkamp, Rutter, &amp; The English and Romanian Adoptees (ERA) Study Team, 1999), revelam que estas crian&ccedil;as tendem  a apresentar dificuldades relacionais muito acentuadas nas suas fam&iacute;lias adoptivas (e.g., agress&atilde;o, sociabilidade indiscriminada,  hiperatividade e problemas com os pares). Tais efeitos s&atilde;o mais fortes quando a institucionaliza&ccedil;&atilde;o perdura para al&eacute;m  do primeiro ano de vida, do que se limitada a alguns meses iniciais (Gunnar, 2001; O&rsquo;Connor et al., 1999).</p>     <p>Um estudo mais recente comparando crian&ccedil;as institucionalizadas e crian&ccedil;as que vivem com os pais, mostra que as primeiras revelam  significativamente menos seguran&ccedil;a da vincula&ccedil;&atilde;o assim como menos compet&ecirc;ncias verbais e comportamentos mais agressivos  (Torres, Maia, Ver&iacute;ssimo, Fernandes, &amp; Silva, 2011).</p>     <p>&nbsp;</p>     <p><i>Vincula&ccedil;&atilde;o em popula&ccedil;&otilde;es at&iacute;picas</i></p>     ]]></body>
<body><![CDATA[<p>Os estudos de vincula&ccedil;&atilde;o em beb&eacute;s e crian&ccedil;as com necessidades especiais incluem crian&ccedil;as com  diagn&oacute;stico de s&iacute;ndroma Down, autismo, nascimento prematuro, dificuldades neuronais e perturba&ccedil;&otilde;es regulat&oacute;rias  (e.g., dificuldades alimentares e associadas ao sono). No seu conjunto, estas investiga&ccedil;&otilde;es apoiam a perspectiva que concebe a  vincula&ccedil;&atilde;o como uma adapta&ccedil;&atilde;o de origem biol&oacute;gica e como um padr&atilde;o de regula&ccedil;&atilde;o  di&aacute;dico, constru&iacute;do ao longo do tempo atrav&eacute;s de processos transacionais crian&ccedil;a-progenitor.</p>     <p>&nbsp;</p>     <p><i>S&iacute;ndroma Down</i>. Os beb&eacute;s e crian&ccedil;as pequenas portadores de s&iacute;ndroma Down alcan&ccedil;am os mesmos  est&aacute;dios desenvolvimentais que as crian&ccedil;as saud&aacute;veis, mas num ritmo mais lento. O que pressup&otilde;e uma  adapta&ccedil;&atilde;o do estilo parental para facilitar o desenvolvimento da crian&ccedil;a, particularmente a n&iacute;vel da linguagem  (Sterling &amp; Warren, 2014). No que concerne ao desenvolvimento da rela&ccedil;&atilde;o de vincula&ccedil;&atilde;o, nestes casos, a sua  consolida&ccedil;&atilde;o ocorre entre os 12 e os 24 meses de idade (Cicchetti &amp; Beeghly, 1990; Thompson, Cicchetti, Lamb, &amp; Malkin, 1985).  Estes beb&eacute;s usam a figura de vincula&ccedil;&atilde;o como base segura para explora&ccedil;&atilde;o do ambiente e como ref&uacute;gio  seguro quando est&atilde;o assustados ou angustiados. Distinguem-se das crian&ccedil;as de desenvolvimento normal quanto &agrave; qualidade das  suas respostas afectivas e comportamentais &agrave; separa&ccedil;&atilde;o (e.g., manifesta&ccedil;&atilde;o emocional menos intensa aquando da  separa&ccedil;&atilde;o, recupera&ccedil;&otilde;es mais r&aacute;pidas e express&atilde;o de um menor leque de emo&ccedil;&otilde;es), mas  n&atilde;o relativamente &agrave; organiza&ccedil;&atilde;o comportamental que estabelecem com a figura de vincula&ccedil;&atilde;o (Thompson et  al., 1985).</p>     <p>&nbsp;</p>     <p><i>Autismo</i>. Apesar de algumas posi&ccedil;&otilde;es defenderem que as dificuldades sociais significativas associadas ao autismo podem  impedir o desenvolvimento da vincula&ccedil;&atilde;o, os estudos nesta &aacute;rea demonstraram que as crian&ccedil;as autistas s&atilde;o capazes  de formar relacionamentos de vincula&ccedil;&atilde;o com o seu prestador de cuidados (Capps, Sigman, &amp; Mundy, 1994; Patone &amp; Rogers, 1984;  Rogers, Ozonoff, &amp; Maslin-Cole, 1993; Shapiro, Sherman, Calamari, &amp; Koch, 1987; Sigman &amp; Ungerer, 1984). Como o demonstram Oppenheim,  Koren-Karie, Dolev e Yirmiya (2009), os mecanismos em jogo na forma&ccedil;&atilde;o de uma vincula&ccedil;&atilde;o segura nestas  crian&ccedil;as s&atilde;o o insight materno para compreender as necessidades da crian&ccedil;a e a aceita&ccedil;&atilde;o do diagn&oacute;stico.  Este estudo sugere ainda que a rela&ccedil;&atilde;o encontrada entre esta capacidade de mentaliza&ccedil;&atilde;o materna dos estados  psicol&oacute;gicos da crian&ccedil;a e o desenvolvimento de uma vincula&ccedil;&atilde;o segura, em crian&ccedil;as sem esta patologia, p&ocirc;de  ser confirmada no caso das crian&ccedil;as autistas.</p>     <p>Assim, v&aacute;rios estudos indicam que aproximadamente 50% dos beb&eacute;s autistas apresentam vincula&ccedil;&otilde;es seguras (Capps et  al., 1994; Shapiro et al., 1987; Rutgers, Bakermans-Kranenburg, Van IJzendoorn, &amp; Van Berckelaer-Onnes, 2004). No entanto, uma vez que as  pesquisas sobre vincula&ccedil;&atilde;o e autismo refletem diferen&ccedil;as metodol&oacute;gicas significativas (e.g., procedimentos  laboratoriais modificados, crian&ccedil;as com idades entre os 3 e os 4 anos), torna-se dif&iacute;cil discernir a extens&atilde;o em que estes  <i>padr&otilde;es </i>seguros da vincula&ccedil;&atilde;o, observados nas crian&ccedil;as autistas, correspondem aos observados em crian&ccedil;as  saud&aacute;veis. A quest&atilde;o permanece uma problem&aacute;tica para investiga&ccedil;&atilde;o emp&iacute;rica.</p>     <p>&nbsp;</p>     <p><i>Prematuridade</i>. Os estudos que comparam beb&eacute;s moderadamente prematuros e beb&eacute;s de termo revelaram n&atilde;o existirem  diferen&ccedil;as na seguran&ccedil;a das rela&ccedil;&otilde;es de vincula&ccedil;&atilde;o (Rode, Chang, Fisch, &amp; Sroufe, 1981). As  exig&ecirc;ncias particulares, caracter&iacute;sticas deste tipo de beb&eacute;s, podem ser mediadas pela <i>responsividade</i> do prestador de  cuidados, resultando no desenvolvimento de padr&otilde;es regulat&oacute;rios adaptativos da d&iacute;ade e em subsequentes rela&ccedil;&otilde;es  de qualidade (Cox, Hopkins, &amp; Hans, 2000). A falta de oportunidade de contacto imediato com o rec&eacute;m-nascido, em si, n&atilde;o  compromete a qualidade futura da vincula&ccedil;&atilde;o. Em contraste, quando os beb&eacute;s prematuros apresentam riscos de sa&uacute;de  suficientemente severos que exigem uma hospitaliza&ccedil;&atilde;o prolongada (e.g., s&iacute;ndroma de perturba&ccedil;&atilde;o  respirat&oacute;ria), tem sido observado um aumento de vincula&ccedil;&otilde;es inseguras-ambivalente/resistente (Mangelsdorf et al., 1996;  Plunkett, Meisels, Stiefel, Pasick, &amp; Roloff, 1986). Isto pode dever-se, em parte, &agrave; situa&ccedil;&atilde;o de stress que os  progenitores e os beb&eacute;s experimentam.</p>     <p>&nbsp;</p>     <p><i>Perturba&ccedil;&otilde;es neurol&oacute;gicas</i>. N&iacute;veis ligeiramente mais elevados de inseguran&ccedil;a da  vincula&ccedil;&atilde;o t&ecirc;m sido observados em popula&ccedil;&otilde;es de crian&ccedil;as com perturba&ccedil;&otilde;es  neurol&oacute;gicas, quando comparadas com crian&ccedil;as saud&aacute;veis (Barnett et al., 1999). Por exemplo, crian&ccedil;as com paralisia  cerebral exibem menores n&iacute;veis de seguran&ccedil;a na vincula&ccedil;&atilde;o (49%) do que o esperado em amostras de baixo-risco de  crian&ccedil;as saud&aacute;veis (Marvin &amp; Pianta, 1996). Barnett e colegas (1999) sugerem que a inseguran&ccedil;a na  vincula&ccedil;&atilde;o em crian&ccedil;as com perturba&ccedil;&otilde;es neurol&oacute;gicas, bem como com outras complica&ccedil;&otilde;es  m&eacute;dicas, pode ser atribu&iacute;vel aos desafios que as suas necessidades especiais colocam aos progenitores, de outro modo adequadamente  sens&iacute;veis. Adicionalmente, valores elevados de casos de vincula&ccedil;&atilde;o desorganizada em crian&ccedil;as com este tipo de  perturba&ccedil;&atilde;o podem estar relacionados com quest&otilde;es de medida (i.e., semelhan&ccedil;as entre sintomas neurol&oacute;gicos e os  marcadores de desorganiza&ccedil;&atilde;o) (Pipp-Siegal, Siegal, &amp; Dean, 1999) e n&atilde;o tanto com a qualidade da rela&ccedil;&atilde;o de  vincula&ccedil;&atilde;o com o progenitor (Barnett et al., 1999).</p>     ]]></body>
<body><![CDATA[<p>&nbsp;</p>     <p><i>Dificuldades de crescimento</i>. Apesar da falta de adequa&ccedil;&atilde;o das defini&ccedil;&otilde;es de perturba&ccedil;&otilde;es  alimentares ou das orienta&ccedil;&otilde;es universais aceites para determinar quando um problema alimentar se torna uma  perturba&ccedil;&atilde;o, as dificuldades alimentares na inf&acirc;ncia t&ecirc;m sido associadas com a rela&ccedil;&atilde;o de  presta&ccedil;&atilde;o de cuidados (Benoit, 2000). Os diagn&oacute;sticos de Dificuldades de Crescimento (DC) t&ecirc;m sido relacionados com  elevados n&iacute;veis de vincula&ccedil;&atilde;o insegura (especialmente desorganizada) em comunidades de risco (Crittenden, 1987; Valenzuela,  1990) e em amostras de pacientes em tratamento ambulat&oacute;rio (Ward, Kessler, &amp; Altman, 1993; Ward, Lee, &amp; Lipper, 2000). Al&eacute;m  disso, a desorganiza&ccedil;&atilde;o da vincula&ccedil;&atilde;o tem sido associada com graves d&eacute;fices ponderais em grupos de baixo peso  (Valenzuela, 1990). Os estudos transculturais tamb&eacute;m sugerem uma liga&ccedil;&atilde;o entre seguran&ccedil;a na vincula&ccedil;&atilde;o e  estatuto nutricional, apesar da dire&ccedil;&atilde;o e condi&ccedil;&otilde;es desta rela&ccedil;&atilde;o permanecerem pouco claras (Dixon,  DeVine, &amp; Brazelton, 1982). Estudos de seguimento (follow-up) de beb&eacute;s com DC e com vincula&ccedil;&otilde;es inseguras demonstraram um  aumento de rigidez f&iacute;sica relacionada com o stress, um decr&eacute;scimo de compet&ecirc;ncia e um aumento do n&uacute;mero de  hospitaliza&ccedil;&otilde;es nos tr&ecirc;s anos ap&oacute;s o diagn&oacute;stico, comparados com os beb&eacute;s com DC seguramente vinculados  (Brinich, Drotar, &amp; Brinich, 1989). A investiga&ccedil;&atilde;o desta &aacute;rea, recorrendo a avalia&ccedil;&otilde;es da  vincula&ccedil;&atilde;o para adultos, sugere que os progenitores das crian&ccedil;as com diagn&oacute;stico de DC podem ser portadores de  distor&ccedil;&otilde;es no que concerne &agrave;s suas pr&oacute;prias experi&ecirc;ncias de presta&ccedil;&atilde;o de cuidados (Coolbear &amp;  Benoit, 1999; Benoit, Zeanah, &amp; Barton, 1989), incluindo perdas n&atilde;o resolvidas e traumas (Ward et al., 2000), que podem contribuir para  perturba&ccedil;&otilde;es no relacionamento progenitor-crian&ccedil;a.</p>     <p>&nbsp;</p>     <p><i>Perturba&ccedil;&otilde;es de sono</i>. Para os beb&eacute;s, as transi&ccedil;&otilde;es entre o sono e a vig&iacute;lia ocorrem muitas  vezes por dia, conferindo oportunidades de regula&ccedil;&atilde;o fisiol&oacute;gica (e.g., fome e temperatura) e socioemocional (e.g.,  separa&ccedil;&atilde;o, reuni&atilde;o e conforto). O desenvolvimento de uma organiza&ccedil;&atilde;o dos ciclos de sono-vig&iacute;lia, nos  primeiros 3 anos de vida, pode estar relacionado com fun&ccedil;&otilde;es regulat&oacute;rias da rela&ccedil;&atilde;o de presta&ccedil;&atilde;o  de cuidados (Anders, Goodlin-Jones, &amp; Sadeh, 2000), uma vez que a investiga&ccedil;&atilde;o tem apoiado a no&ccedil;&atilde;o de que as  intera&ccedil;&otilde;es recorrentes e previs&iacute;veis do prestador de cuidados facilitam a regula&ccedil;&atilde;o biol&oacute;gica e social  (Anders, 1994). Factores parentais como conflito conjugal, psicopatologia materna (e.g., depress&atilde;o) e stress familiar foram identificados  como contribuindo para os problemas de sono (Sadeh, 1996), sendo que as m&atilde;es dos beb&eacute;s com perturba&ccedil;&otilde;es de sono  t&ecirc;m uma maior propens&atilde;o para exibirem representa&ccedil;&otilde;es distorcidas e desligadas dos seus beb&eacute;s (Benoit, Zeanah,  Parker, Nicholson, &amp; Coolbear, 1997) e das suas pr&oacute;prias experi&ecirc;ncias de cuidados (Benoit, Zeanah, Boucher, &amp; Minde, 1992).</p>     <p>&nbsp;</p>     <p><i>Avalia&ccedil;&atilde;o da qualidade da vincula&ccedil;&atilde;o em contexto cl&iacute;nico</i></p>     <p>Uma consulta cl&iacute;nica &eacute; uma situa&ccedil;&atilde;o ativadora do sistema da vincula&ccedil;&atilde;o da crian&ccedil;a pequena  (Crowell, 2003). Mesmo tendo em conta que ela n&atilde;o ativa o sistema de vincula&ccedil;&atilde;o da mesma maneira que uma  situa&ccedil;&atilde;o estandardizada como a Situa&ccedil;&atilde;o Estranha, n&atilde;o sendo portanto pertinente do ponto de vista da  investiga&ccedil;&atilde;o, ela &eacute; clinicamente interessante, e pode ser um auxiliador na avalia&ccedil;&atilde;o informal da qualidade da  vincula&ccedil;&atilde;o da crian&ccedil;a (<a href="#q1">Quadro 1</a>). Neste contexto, o cl&iacute;nico deve prestar particular  aten&ccedil;&atilde;o aos seguintes marcadores de uma crian&ccedil;a insegura: (a) aus&ecirc;ncia ou distor&ccedil;&atilde;o do comportamento de  base segura (i.e., aus&ecirc;ncia de monitoriza&ccedil;&atilde;o visual ou contacto f&iacute;sico pouco frequente); (b) incapacidade em envolver-se  em atividades de explora&ccedil;&atilde;o (i.e., extrema inibi&ccedil;&atilde;o, medo ou depend&ecirc;ncia); (c) perturba&ccedil;&atilde;o extrema  ou aus&ecirc;ncia de perturba&ccedil;&atilde;o aquando de uma separa&ccedil;&atilde;o da parte da Figura de Vincula&ccedil;&atilde;o (F.V.); (d)  agressividade extrema; (e) dificuldade no estabelecimento das intera&ccedil;&otilde;es com a F.V; (f) contrastando com o comportamento de  familiaridade excessiva com os desconhecidos.</p>     <p>&nbsp;</p>     <p><a name="q1"></a></p>     <p><img src="/img/revistas/aps/v35n4/35n4a01q1.jpg" width="853" height="575"></p>

<body><![CDATA[<p>&nbsp;</p>     <p>Ao n&iacute;vel da intera&ccedil;&atilde;o da d&iacute;ade, a observa&ccedil;&atilde;o deve permitir responder &agrave;s seguintes  quest&otilde;es: (a) em que situa&ccedil;&otilde;es solicitam eles o contacto uns com os outros?; (b) como reage a crian&ccedil;a face &agrave;s  emo&ccedil;&otilde;es negativas?; (c) como varia o comportamento de cada um em fun&ccedil;&atilde;o do n&iacute;vel e tipo de stress?; (d)  manifestam eles falta de calor e afecto nas intera&ccedil;&otilde;es?; (e) os pais apresentam comportamentos inadaptados ou at&iacute;picos, em  especial comportamentos assustadores, assustados ou confusionais?</p>     <p>As observa&ccedil;&otilde;es da qualidade da intera&ccedil;&atilde;o da crian&ccedil;a com a sua F.V. devem ainda ser acompanhadas por um  conjunto de quest&otilde;es facilmente colocadas no contexto cl&iacute;nico, a saber: (1) como utiliza a crian&ccedil;a a F.V. como base segura,  isto &eacute;, o que faz a crian&ccedil;a quando est&aacute; triste, inquieta, preocupada ou quando se magoa? (2) que experi&ecirc;ncias de  separa&ccedil;&atilde;o e de perda viveu a crian&ccedil;a?</p>     <p>Os cl&iacute;nicos de sa&uacute;de mental infantil ocupam um lugar privilegiado na promo&ccedil;&atilde;o da seguran&ccedil;a na  vincula&ccedil;&atilde;o, aquando da detec&ccedil;&atilde;o de rela&ccedil;&otilde;es inseguras. De acordo com os estudos de Ainsworth e colegas  (1978), a vincula&ccedil;&atilde;o segura encontra-se relacionada com o seguinte comportamento materno: (a) contacto f&iacute;sico frequente e  sustentado entre o beb&eacute; e a m&atilde;e, especialmente durante os primeiros seis meses, conjuntamente com a capacidade materna em acalmar o  seu beb&eacute; pegando-lhe ao colo; (b) sensibilidade materna aos sinais do seu beb&eacute; e, em particular, capacidade para gerir as suas  interven&ccedil;&otilde;es em harmonia com os ritmos daquele (Guedeney et al., 2011); (c) um ambiente regulado e previs&iacute;vel, que permita ao  beb&eacute; retirar dele um sentido de consequ&ecirc;ncias para as suas pr&oacute;prias a&ccedil;&otilde;es; (d) deleite m&uacute;tuo sentido pela  m&atilde;e e pelo beb&eacute; na companhia um do outro.</p>     <p>A sensibiliza&ccedil;&atilde;o dos progenitores a estas caracter&iacute;sticas parentais &eacute; importante, uma vez que elas conferem ao  beb&eacute; a possibilidade de exprimir livremente as suas necessidades de vincula&ccedil;&atilde;o. Elas permitem ainda a constru&ccedil;&atilde;o  de uma rela&ccedil;&atilde;o de progressiva autonomia, onde a confian&ccedil;a nas suas pr&oacute;prias compet&ecirc;ncias, mas tamb&eacute;m na  disponibilidade da F.V. em caso de necessidade de ajuda, &eacute; constru&iacute;da. Estes s&atilde;o os guias de orienta&ccedil;&atilde;o gerais  que o cl&iacute;nico deve transmitir aos progenitores para nortear o seu comportamento, promovendo, consequentemente, uma vincula&ccedil;&atilde;o  mais segura nos seus filhos. No caso das vincula&ccedil;&otilde;es desorganizadas, a identifica&ccedil;&atilde;o e corre&ccedil;&atilde;o de  comportamentos parentais assustadores ou assustados &eacute; essencial. Uma hist&oacute;ria de luto precoce ou de trauma parental n&atilde;o  elaborados s&atilde;o frequentes, podendo ser necess&aacute;rio orientar o adulto para uma terapia individual. Na sua globalidade, a pesquisa sobre  a interven&ccedil;&atilde;o cl&iacute;nica indica que a esta deve: (a) come&ccedil;ar cedo; (b) ser compreensiva; (c) envolver diretamente os  progenitores; (d) remeter-se aos aspectos emocionais da rela&ccedil;&atilde;o de cuidados e, (e) garantir um ambiente terap&ecirc;utico seguro e  sustentador (Egeland, Weinfield, Bosquet, &amp; Cheng, 2000; Robinson, 2002). N&atilde;o existe nenhum ponto a partir do qual uma  interven&ccedil;&atilde;o ou apoio prestados se apresentam como in&uacute;teis ou demasiado tardios.</p>     <p>&nbsp;</p>     <p><b>S&iacute;ntese conclusiva</b></p>     <p>As varia&ccedil;&otilde;es da qualidade de vincula&ccedil;&atilde;o representam condi&ccedil;&otilde;es precoces que, em contexto de  intera&ccedil;&atilde;o entre factores biol&oacute;gicos e ambientais, desempenham um papel din&acirc;mico no desenvolvimento do funcionamento  s&oacute;cio-emocional do indiv&iacute;duo.</p>     <p>A investiga&ccedil;&atilde;o na vincula&ccedil;&atilde;o tem-nos esclarecido sobre os efeitos nas crian&ccedil;as de experi&ecirc;ncias  precoces associadas a rotinas de cuidados fora de casa, separa&ccedil;&otilde;es e perturba&ccedil;&otilde;es significativas de  presta&ccedil;&atilde;o de cuidados. S&atilde;o ainda colocadas em causa as pr&aacute;ticas de separa&ccedil;&atilde;o progenitor-crian&ccedil;a  quando a crian&ccedil;a est&aacute; doente ou durante procedimentos m&eacute;dicos dolorosos. A presen&ccedil;a apoiante de figuras relacionais  significativas permite ajudar as crian&ccedil;as pequenas a lidarem e a recuperarem do trauma e do <i>stress</i>. As crian&ccedil;as com  necessidades espec&iacute;ficas podem desenvolver uma qualidade de vincula&ccedil;&atilde;o de boa qualidade, especialmente se um apoio &eacute;  dado aos progenitores, a fim de tornar poss&iacute;vel um aumento da sua sensibilidade e capacidade de resposta para fazer face &agrave;s  dif&iacute;ceis exig&ecirc;ncias da crian&ccedil;a.</p>     <p>As crian&ccedil;as n&atilde;o ficam para sempre &ldquo;marcadas por causa&rdquo; de padr&otilde;es relacionais precoces mas as  rela&ccedil;&otilde;es de vincula&ccedil;&atilde;o t&ecirc;m significado desenvolvimental para o seu funcionamento (mal)adaptativo subsequente. A  interven&ccedil;&atilde;o cl&iacute;nica para promover uma melhor qualidade relacional com as figuras significativas ou a corre&ccedil;&atilde;o de  certos padr&otilde;es interacionais disfuncionais apresenta-se, nos nossos dias, como extremamente pertinente.</p>     ]]></body>
