Materiais e métodos A elaboração do plano editorial e a produção dos materiais tiveram por base um estudo exploratório, que contou com uma revisão da literatura e com um diagnóstico ecológico e educacional numa área-piloto. Ao longo de todo o processo de produção dos materiais foram consultados peritos nas diversas áreas temáticas. A legibilidade dos materiais produzidos foi testada. Resultados Através do estudo exploratório identificámos os fatores predisponentes, facilitadores e de reforço inerentes a comportamentos relacionados com a obesidade infantil, que nortearam a produção dos materiais. Os materiais produzidos, baseados na mais recente evidência científica, demonstraram ser acessíveis para pessoas com baixos níveis de escolaridade. Conclusão A metodologia participativa utilizada no projeto permitiu-nos compilar informação relevante quanto aos principais pontos críticos para a compreensão e prevenção da obesidade infantil. A elaboração das mensagens, com linguagem simples e organização lógica, adequadas à população, poderá ser facilitadora da mudança comportamental, particularmente se tiver o apoio de mediadores que as adaptem a cada contexto, reforçando a abordagem socioecológica na prevenção da obesidade infantil.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Introduction «Papa Bem» project («Eat Well») aims to promote health literacy through the production and dissemination of written and audiovisual materials concerning childhood obesity prevention. The focus is the period that goes from pregnancy to 5 years of age. Methodology The editorial plan and materials production was based on an exploratory study, which included a literature review and an ecological and educational diagnosis conducted in a pilot area. During the process of production of the materials several experts were consulted. The readability of the materials was tested. Results In the exploratory study, predisposing, enabling and reinforcement factors related to the behaviors associated with child obesity were identified to guide the materials production. Based in the most recent scientific evidence, these materials proved to be accessible to people with lower education levels. Conclusion Through the participatory methodology, it was possible to get relevant information about the main critical issues in understanding and managing the problem of child obesity. The development of simple and logically organized messages may facilitate behavior change, especially with the support of mediators who adapt them to each context, ensuring the continuity of the socio-ecological approach required for controlling child obesity.]]></p></abstract>

Parenting. </P>     <p>&nbsp;</p>     <P> <B>Introdu&ccedil;&atilde;o</B> </P>     <P>A obesidade &eacute; considerada como um dos mais s&eacute;rios desafios de sa&uacute;de p&uacute;blica no in&iacute;cio do s&eacute;culo XXI. &Eacute; um problema mundial, que afeta principalmente os pa&iacute;ses de m&eacute;dio e alto rendimento<SUP>1</SUP>. A sua preval&ecirc;ncia entre crian&ccedil;as e adultos tem vindo a aumentar substancialmente<SUP>2</SUP>, sendo as popula&ccedil;&otilde;es com baixos n&iacute;veis socioecon&oacute;micos e de escolaridade as que est&atilde;o em maior risco<SUP>3</SUP>. Apesar dos esfor&ccedil;os realizados nos &uacute;ltimos anos, as medidas de preven&ccedil;&atilde;o e tratamento n&atilde;o t&ecirc;m demonstrado sucesso no controlo deste problema de sa&uacute;de p&uacute;blica<SUP>2</SUP>.</P>     <P>O excesso de peso e a obesidade est&atilde;o associados ao desenvolvimento de condi&ccedil;&otilde;es adversas &agrave; sa&uacute;de, como a hipertens&atilde;o arterial, hipercolesterolemia, hipertrigliceridemia e resist&ecirc;ncia &agrave; insulina. Os riscos de doen&ccedil;a coron&aacute;ria, acidente vascular cerebral isqu&eacute;mico e diabetes tipo II aumentam com o incremento do &iacute;ndice de massa corporal (IMC). Um IMC elevado parece influenciar tamb&eacute;m o risco de alguns tipos de cancro, como o cancro da mama, c&oacute;lon-retal, do endom&eacute;trio, dos rins, do es&oacute;fago e do p&acirc;ncreas<SUP>4</SUP>.</P>     <P>Em crian&ccedil;as, o excesso de peso e a obesidade est&atilde;o associados a uma redu&ccedil;&atilde;o na qualidade de vida e maior risco de <I>bullying</I> e isolamento social, com impacto negativo na sa&uacute;de mental<SUP>1</SUP>.</P>     <P>Em 2011, mais de 40 milh&otilde;es de crian&ccedil;as em todo o mundo j&aacute; tinham excesso de peso antes dos 5 anos de idade<SUP>5</SUP>. Relativamente a Portugal, dados de um estudo realizado em 2001 revelavam uma preval&ecirc;ncia de 23,1% de excesso de peso (incluindo obesidade) e 6,2% de obesidade em crian&ccedil;as de 4 anos de idade<SUP>6</SUP>.</P>     <P>Dados mais recentes demonstram um panorama ainda mais preocupante. Em 2010, o estudo <I>Childhood Obesity Surveillance Initiative</I> encontrou uma preval&ecirc;ncia de 30,2% de excesso de peso (incluindo obesidade) e 14,3% de obesidade em crian&ccedil;as de 6-8 anos<SUP>7</SUP>. J&aacute; o Estudo do Padr&atilde;o Alimentar e de Crescimento Infantil (EPACI) revela uma preval&ecirc;ncia de cerca de 38% de excesso de peso (incluindo obesidade) e de 6,5% de obesidade em crian&ccedil;as dos 12-36 meses de vida<SUP>8</SUP>.</P>     <P>Sendo a obesidade uma doen&ccedil;a cr&oacute;nica, com um impacto t&atilde;o negativo na sa&uacute;de de adultos e crian&ccedil;as e cada vez mais prevalente por todo o mundo, a sua preven&ccedil;&atilde;o precoce &eacute; estrat&eacute;gica e os per&iacute;odos da gravidez e dos primeiros anos de vida t&ecirc;m sido apontados como oportunidades importantes neste sentido<SUP>9,10</SUP>.</P>     <P>De facto, alguns fatores de risco para a obesidade infantil surgem desde muito cedo. A vida intrauterina e os primeiros anos de vida s&atilde;o considerados per&iacute;odos cr&iacute;ticos na programa&ccedil;&atilde;o da regula&ccedil;&atilde;o do balan&ccedil;o energ&eacute;tico a longo prazo<SUP>10</SUP>. Al&eacute;m disso, os h&aacute;bitos e prefer&ecirc;ncias alimentares come&ccedil;am a ser estabelecidos durante este per&iacute;odo<SUP>11</SUP>.</P>     ]]></body>
<body><![CDATA[<P>A obesidade no in&iacute;cio da gravidez, o aumento excessivo de peso da m&atilde;e<SUP>2</SUP> e o consumo de tabaco<SUP>10</SUP> s&atilde;o exemplos de fatores de risco para a obesidade infantil no per&iacute;odo da gravidez. Logo ap&oacute;s o nascimento, a alimenta&ccedil;&atilde;o com leite de f&oacute;rmula, a diversifica&ccedil;&atilde;o alimentar antes dos 4 meses de vida e um crescimento acelerado no primeiro ano de vida s&atilde;o outros exemplos<SUP>2</SUP>. Mais tarde, o baixo consumo de frutas e hort&iacute;colas, o h&aacute;bito de comer fora de casa, as grandes por&ccedil;&otilde;es de alimentos, o consumo excessivo de alimentos e bebidas de alta densidade energ&eacute;tica, o h&aacute;bito de petiscar, os baixos n&iacute;veis de atividade f&iacute;sica, o excesso de atividades sedent&aacute;rias, como ver televis&atilde;o e dormir menos horas do que o recomendado, s&atilde;o tamb&eacute;m fatores que contribuem para o excesso de peso<SUP>2</SUP>.</P>     <P>As recomenda&ccedil;&otilde;es mais recentes apontam claramente para a necessidade de abordagens precoces e mais ecol&oacute;gicas e holistas no combate &agrave; obesidade infantil, focando-se na fam&iacute;lia e na comunidade em que esta se integra<SUP>9,10,12</SUP>.</P>     <P>Entre as estrat&eacute;gias de promo&ccedil;&atilde;o da sa&uacute;de, a import&acirc;ncia da literacia e da literacia em sa&uacute;de tem sido largamente documentada<SUP>13</SUP>. No contexto da obesidade, a baixa literacia e numeracia est&atilde;o associadas a piores conhecimentos sobre aleitamento materno, a maiores dificuldades para compreender r&oacute;tulos e por&ccedil;&otilde;es e a um IMC mais elevado<SUP>14</SUP>. V&aacute;rios estudos t&ecirc;m demonstrado que uma propor&ccedil;&atilde;o elevada dos pais tem um n&iacute;vel de literacia em sa&uacute;de que compromete a sua capacidade para realizar tarefas preventivas b&aacute;sicas, como seguir as recomenda&ccedil;&otilde;es de uma brochura ou usar uma tabela de percentis<SUP>15</SUP>. Adicionalmente, alguns estudos internacionais t&ecirc;m apontado que muita da informa&ccedil;&atilde;o sobre sa&uacute;de infantil que &eacute; disponibilizada aos pais n&atilde;o se adequa aos seus n&iacute;veis de literacia<SUP>16,17</SUP>. Desta forma, melhorar os resultados das interven&ccedil;&otilde;es preventivas implica tamb&eacute;m que estas sejam sens&iacute;veis e adequadas ao n&iacute;vel de literacia da popula&ccedil;&atilde;o-alvo. As recomenda&ccedil;&otilde;es mais recentes para a preven&ccedil;&atilde;o da obesidade infantil incluem a produ&ccedil;&atilde;o e dissemina&ccedil;&atilde;o alargada de mensagens simples<SUP>14</SUP>, que respeitem os princ&iacute;pios de produ&ccedil;&atilde;o e fornecimento de informa&ccedil;&atilde;o em sa&uacute;de a popula&ccedil;&otilde;es com baixos n&iacute;veis de literacia<SUP>18&ndash;20</SUP>.</P>     <P>O n&iacute;vel de literacia em sa&uacute;de da popula&ccedil;&atilde;o portuguesa &eacute; desconhecido<SUP>21</SUP>. No entanto, estando a literacia em sa&uacute;de intimamente relacionada com os n&iacute;veis de literacia em geral, cabe aqui referir que os estudos em Portugal sobre o tema, apesar de escassos, manifestam de forma semelhante os baixos n&iacute;veis de literacia da popula&ccedil;&atilde;o<SUP>22&ndash;24</SUP>.</P>     <P>O presente artigo descreve os procedimentos no projeto &laquo;Papa Bem&raquo;, um projeto de produ&ccedil;&atilde;o e dissemina&ccedil;&atilde;o de informa&ccedil;&atilde;o sobre a obesidade infantil, veiculada pela Internet e destinada ao p&uacute;blico em geral, desenvolvido no &acirc;mbito do Programa Harvard Medical School &ndash; Portugal.</P>     <P>O projeto &laquo;Papa Bem&raquo; pretende constituir um contributo para a preven&ccedil;&atilde;o da obesidade infantil, tendo como finalidade melhorar a literacia em sa&uacute;de de futuros pais, pais e outros cuidadores de crian&ccedil;as at&eacute; aos 5 anos de idade, atrav&eacute;s da dissemina&ccedil;&atilde;o de mensagens baseadas na evid&ecirc;ncia cient&iacute;fica e veiculadas por materiais escritos e audiovisuais acerca da obesidade infantil e crescimento saud&aacute;vel.</P>     <p>&nbsp;</p>     <P> <B>Materiais e m&eacute;todos</B> </P>     <P>O quadro de refer&ecirc;ncia te&oacute;rica que orientou o projeto &laquo;Papa Bem&raquo; &eacute; composto por 3 refer&ecirc;ncias complementares: o modelo contextual de Davison e Birch<SUP>12</SUP>, a abordagem <I>Health, Exercise, Nutrition for the Really Young</I> (HENRY) de combate &agrave; obesidade infantil de Hunt e Rudolf<SUP>9</SUP>, adotada pelo Servi&ccedil;o Nacional de Sa&uacute;de do Reino Unido, e o relat&oacute;rio <I>Early Childhood Obesity Prevention Policies</I>, do Institute of Medicine<SUP>10</SUP>.</P>     <P>Estas recomenda&ccedil;&otilde;es v&atilde;o al&eacute;m das orienta&ccedil;&otilde;es mais habituais em torno da alimenta&ccedil;&atilde;o e da atividade f&iacute;sica, valorizando o papel dos pais e do ambiente familiar para a forma&ccedil;&atilde;o de bons h&aacute;bitos de alimenta&ccedil;&atilde;o, de atividade f&iacute;sica e de sono e para um crescimento saud&aacute;vel, incluindo dimens&otilde;es mais alargadas, como o estilo de vida, a organiza&ccedil;&atilde;o familiar e a parentalidade. Por outro lado, &eacute; enfatizada a import&acirc;ncia de abordagens de interven&ccedil;&atilde;o centradas na fam&iacute;lia, apoiando as mudan&ccedil;as de estilo de vida de acordo com as suas necessidades e investindo no reconhecimento do problema e motiva&ccedil;&atilde;o para a mudan&ccedil;a<SUP>25,26</SUP>. Diversos estudos evidenciam a associa&ccedil;&atilde;o entre fatores parentais e alguns comportamentos dos filhos<SUP>12,27,28</SUP>. As pr&aacute;ticas parentais de oferta de alimentos, como a restri&ccedil;&atilde;o e a press&atilde;o para comer, tamb&eacute;m t&ecirc;m merecido destaque nesta &aacute;rea de estudo<SUP>12,29,30</SUP>. Alguns trabalhos discutem ainda a associa&ccedil;&atilde;o entre o estilo parental e os comportamentos alimentares das crian&ccedil;as e a obesidade infantil<SUP>31</SUP>, havendo autores que recomendam a promo&ccedil;&atilde;o de um estilo parental &laquo;autoritativo&raquo;, ou seja, firme, que estabelece limites (por exemplo, s&atilde;o os adultos que definem os alimentos que s&atilde;o oferecidos &agrave; crian&ccedil;a), por&eacute;m, caloroso e responsivo aos sinais da crian&ccedil;a (por exemplo, respeito pelos sinais de fome e saciedade)<SUP>31&ndash;33</SUP>. As refei&ccedil;&otilde;es em fam&iacute;lia tamb&eacute;m aparecem associadas a comportamentos alimentares e peso mais adequados em estudos com crian&ccedil;as e adolescentes<SUP>34&ndash;37</SUP>. Estas associa&ccedil;&otilde;es podem evidenciar a import&acirc;ncia de aspetos relacionados com a din&acirc;mica familiar<SUP>31</SUP>.</P>     ]]></body>
<body><![CDATA[<P>No sentido de conjugar a melhor evid&ecirc;ncia cient&iacute;fica dispon&iacute;vel com as necessidades espec&iacute;ficas do p&uacute;blico-alvo seguimos os passos do modelo de planeamento <I>PRECEDE-PROCEED</I><SUP>38</SUP> e a estrat&eacute;gia de investiga&ccedil;&atilde;o-a&ccedil;&atilde;o a ele subjacente.</P>     <P>Segundo este modelo de planeamento, come&ccedil;a-se por identificar a quest&atilde;o ou problema de sa&uacute;de e os fatores gen&eacute;ticos, comportamentais e ambientais eventualmente implicados na situa&ccedil;&atilde;o. Quando &eacute; necess&aacute;rio alterar comportamentos h&aacute; que identificar os fatores predisponentes dos comportamentos em causa, bem como os fatores de facilita&ccedil;&atilde;o e de refor&ccedil;o necess&aacute;rios para apoiar a mudan&ccedil;a destes comportamentos de forma sustentada<SUP>39</SUP>.</P>     <P>Desenvolvemos um projeto-piloto numa &aacute;rea do distrito de Lisboa, escolhida por conveni&ecirc;ncia devido &agrave; sua diversidade socioecon&oacute;mica e cultural e ao interesse manifestado pelos profissionais de sa&uacute;de dos cuidados de sa&uacute;de prim&aacute;rios e do hospital de refer&ecirc;ncia em colaborar no projeto.</P>     <P>Este projeto-piloto foi organizado em 4 fases interdependentes, que passaremos a descrever: estudo explorat&oacute;rio, produ&ccedil;&atilde;o dos materiais, avalia&ccedil;&atilde;o dos materiais, implementa&ccedil;&atilde;o (<a href="#f1">fig. 1</a>).</P>     <p>&nbsp;</p> <a name="f1"></a> <img src="/img/revistas/rpsp/v33n1/33n1a03f1.jpg">     
<p>&nbsp;</p> <B>Estudo explorat&oacute;rio</B>     <p></P>     <P>Seguindo o modelo de planeamento PRECEDE-PROCEED, ap&oacute;s a identifica&ccedil;&atilde;o da obesidade como um problema de sa&uacute;de p&uacute;blica e a sua caracteriza&ccedil;&atilde;o em Portugal, partimos para a an&aacute;lise das vari&aacute;veis modific&aacute;veis para a sua preven&ccedil;&atilde;o atuando sobre os comportamentos determinantes, abordando os seus fatores predisponentes, bem como os fatores facilitadores e de refor&ccedil;o para uma mudan&ccedil;a sustentada dos mesmos.</P>     <P>Ap&oacute;s a revis&atilde;o da literatura avan&ccedil;&aacute;mos para um diagn&oacute;stico ecol&oacute;gico e educacional da &aacute;rea-piloto, que envolveu a recolha de dados junto da popula&ccedil;&atilde;o-alvo e de alguns dos seus principais mediadores, nomeadamente, os profissionais de sa&uacute;de.</P>     <P>A etapa de recolha de dados assentou em diferentes metodologias e fontes de informa&ccedil;&atilde;o. <UL>       ]]></body>

<body><![CDATA[<P>A elabora&ccedil;&atilde;o do plano editorial e a produ&ccedil;&atilde;o dos materiais seguiram um processo sistem&aacute;tico de sele&ccedil;&atilde;o de mensagens, desenvolvimento de conte&uacute;dos, revis&atilde;o e avalia&ccedil;&atilde;o. Num primeiro momento foram considerados os fatores predisponentes, facilitadores e de refor&ccedil;o identificados na revis&atilde;o da literatura e no diagn&oacute;stico da &aacute;rea-piloto realizados no &acirc;mbito do estudo explorat&oacute;rio, dando origem a um plano editorial.</P>     <P>Os materiais foram produzidos e revistos pela equipa multidisciplinar do projeto, incluindo m&eacute;dicos de sa&uacute;de p&uacute;blica, pediatras, nutricionistas, especialistas em atividade f&iacute;sica, psic&oacute;logos, entre outros. Esta produ&ccedil;&atilde;o e revis&atilde;o considerou o conte&uacute;do, mas tamb&eacute;m teve em conta os baixos n&iacute;veis de literacia da popula&ccedil;&atilde;o portuguesa e as diretrizes do programa, que indicavam a produ&ccedil;&atilde;o de materiais escritos para um p&uacute;blico com o 10.&deg; ano de escolaridade e vers&otilde;es simplificadas dos mesmos para um p&uacute;blico de mais baixa literacia (5.&deg; ano de escolaridade), com recurso aos meios audiovisuais. Assim, foram consideradas as recomenda&ccedil;&otilde;es para produ&ccedil;&atilde;o de materiais que t&ecirc;m por base a metodologia <I>Plain Language</I> e se focam em 4 componentes principais: a adequa&ccedil;&atilde;o &agrave; popula&ccedil;&atilde;o-alvo, a linguagem e o estilo, a organiza&ccedil;&atilde;o, o <I>layout</I> e o <I>design</I><SUP>40,41</SUP>.</P>     <P>A equipa do projeto contou com a consultoria de especialistas internacionais em obesidade infantil e literacia em sa&uacute;de. Adicionalmente, ao longo de todo o processo de produ&ccedil;&atilde;o dos materiais foram consultados os profissionais de sa&uacute;de da &aacute;rea do estudo-piloto, quer ao n&iacute;vel dos cuidados de sa&uacute;de prim&aacute;rios quer ao n&iacute;vel da consulta hospitalar da especialidade de obesidade infantil. Estes profissionais contribu&iacute;ram para a triagem das mensagens, sua revis&atilde;o e avalia&ccedil;&atilde;o, at&eacute; chegar ao produto final.</P>     <P>Por forma a motivar e capacitar a equipa do projeto e outros profissionais envolvidos, organizaram-se 2 <I>workshops</I>: um sobre a produ&ccedil;&atilde;o de materiais de educa&ccedil;&atilde;o para a sa&uacute;de para a popula&ccedil;&atilde;o, orientado por uma perita em literacia em sa&uacute;de da Universidade de Harvard, e outro sobre o modelo de interven&ccedil;&atilde;o <I>HENRY</I> para o controlo da obesidade infantil, orientado pela pediatra correspons&aacute;vel pela cria&ccedil;&atilde;o e dissemina&ccedil;&atilde;o deste modelo no Reino Unido<SUP>9,31</SUP>.</P>     <p>&nbsp;</p>     <P> <B>Avalia&ccedil;&atilde;o dos materiais</B> </P>     <P>Com o objetivo de validar a adequa&ccedil;&atilde;o cultural, a clareza, a acessibilidade e a compreens&atilde;o das mensagens presentes nos materiais produzidos passou-se &agrave; etapa de avalia&ccedil;&atilde;o da qual fizeram parte as seguintes componentes. <UL>       <LI>         <P>Revis&atilde;o dos materiais de acordo com uma lista de princ&iacute;pios de boa comunica&ccedil;&atilde;o (por exemplo, utiliza&ccedil;&atilde;o da voz ativa, frases curtas, recurso m&iacute;nimo a termos t&eacute;cnicos&hellip;).</P>   </LI>       <LI>         ]]></body>

<P>Podemos destacar de entre estes resultados que os pais se preocupam, sobretudo, que a crian&ccedil;a coma pouco (47,1% dos 87 pais que preencheram question&aacute;rio), sendo muito menos os que atribuem import&acirc;ncia ao facto da crian&ccedil;a comer demais (21,8%). De acordo com um <I>focus group</I>, alguns profissionais de sa&uacute;de atribuem o excesso de peso nas crian&ccedil;as aos maus h&aacute;bitos dos pais. No que respeita ao sono verifica-se que a maioria dos pais n&atilde;o lhe atribui a devida import&acirc;ncia (70,9% dos pais n&atilde;o se preocupa ou tem preocupa&ccedil;&otilde;es inadequadas acerca do n&uacute;mero de horas de sono que os seus filhos devem dormir). Os profissionais de sa&uacute;de apontam h&aacute;bitos familiares sedent&aacute;rios (em <I>focus group</I> afirmou-se que &laquo;Muitas vezes encontramos os pais com as crian&ccedil;as no shopping [&hellip;] e nas zonas urbanas os pais n&atilde;o saem dos apartamentos&raquo;) e confirma-se a forte presen&ccedil;a da televis&atilde;o no quotidiano das crian&ccedil;as (em entrevista aos pais: &laquo;A televis&atilde;o do quarto dele est&aacute; sempre ligada, mas ele s&oacute; olha de vez em quando&raquo;), mesmo durante as refei&ccedil;&otilde;es (39,1% dos pais &laquo;concordam&raquo; ou &laquo;concordam um pouco&raquo; que a televis&atilde;o ajuda a crian&ccedil;a a comer melhor).</P>     <P>Atrav&eacute;s dos question&aacute;rios e das entrevistas levadas a cabo com os pais verificou-se haver interesse por aceder &agrave; informa&ccedil;&atilde;o esclarecedora sobre a melhor forma de apoiar o crescimento dos seus filhos.</P>     ]]></body>
<body><![CDATA[<P>Os profissionais de sa&uacute;de sugeriram que a abordagem das tem&aacute;ticas em causa junto dos pais deveria ocorrer nas consultas de sa&uacute;de materna, considerando que &laquo;a sala de espera tamb&eacute;m ajuda como ambiente de informa&ccedil;&atilde;o&raquo;.</P>     <P>&Eacute; reconhecida pelos profissionais de sa&uacute;de a necessidade de envolver todos os protagonistas e locais onde os pais vivem e trabalham &ndash; &laquo;escola, fam&iacute;lia, <I>media</I>, c&acirc;mara, locais de trabalho&raquo; &ndash; para apoiar os pais no processo educativo dos seus filhos.</P>     <p>&nbsp;</p>     <P> <B>Produ&ccedil;&atilde;o de materiais</B> </P>     <P>Com a an&aacute;lise dos resultados do estudo explorat&oacute;rio foram tomadas op&ccedil;&otilde;es estrat&eacute;gicas para a produ&ccedil;&atilde;o dos materiais tendo em conta o contexto e as caracter&iacute;sticas da popula&ccedil;&atilde;o-alvo.</P>     <P>Assim, elabor&aacute;mos um plano editorial orientado para a perce&ccedil;&atilde;o do risco, dete&ccedil;&atilde;o precoce e preven&ccedil;&atilde;o da obesidade infantil e crescimento saud&aacute;vel desde a gravidez at&eacute; aos 5 anos de idade da crian&ccedil;a, com foco nas seguintes &aacute;reas tem&aacute;ticas: gravidez saud&aacute;vel, alimenta&ccedil;&atilde;o, atividade f&iacute;sica, sedentarismo, sono, intera&ccedil;&atilde;o educador/crian&ccedil;a e din&acirc;micas intrafamiliares e da comunidade (nomeadamente, creches e jardins de inf&acirc;ncia).</P>     <P>Considerando as orienta&ccedil;&otilde;es para a produ&ccedil;&atilde;o de materiais para a popula&ccedil;&atilde;o em geral, no desenvolvimento das mensagens utiliz&aacute;mos uma linguagem coloquial, com frases pequenas e simples, alguns exemplos ilustrativos das mensagens, voz ativa, amig&aacute;vel e envolvente e o modelo de perguntas e respostas.</P>     <P>Para lidarmos com n&iacute;veis de literacia, necessidades e contextos de intera&ccedil;&atilde;o diferentes, decorrentes da realidade de cada indiv&iacute;duo e fam&iacute;lia, bem como da sua sensibilidade &agrave;s propostas para mudan&ccedil;as comportamentais, desenvolvemos diversos tipos de materiais para cada &aacute;rea tem&aacute;tica: textos, folhetos, v&iacute;deos, &aacute;udios e testes do conhecimento <I>(quizzes)</I>. Assim, os v&iacute;deos pretendem, principalmente, atender &agrave;s necessidades da popula&ccedil;&atilde;o de mais baixa escolaridade e permitem uma intera&ccedil;&atilde;o em salas de espera, por exemplo. Os testes do conhecimento <I>(quizzes)</I> aparecem como instrumentos de refor&ccedil;o &agrave; aprendizagem, podendo considerar-se, tamb&eacute;m, adequados a qualquer n&iacute;vel de escolaridade. J&aacute; as sugest&otilde;es pr&aacute;ticas e os instrumentos de apoio &agrave; introdu&ccedil;&atilde;o de novos comportamentos, como folhetos de autorregisto e instrumentos de planeamento, constituem-se como uma aposta para a tomada de decis&atilde;o, perce&ccedil;&atilde;o de autoefic&aacute;cia e manuten&ccedil;&atilde;o do comportamento. Os folhetos s&atilde;o particularmente adequados para a utiliza&ccedil;&atilde;o por parte de mediadores, como os profissionais de sa&uacute;de.</P>     <P>Organiz&aacute;mos os materiais segundo uma sequ&ecirc;ncia l&oacute;gica, agrupados por sec&ccedil;&otilde;es e por &aacute;reas tem&aacute;ticas e identificados com pictogramas.</P>     <P>Acerca do <I>layout e d</I>o <I>design</I>, procurando ultrapassar eventuais resist&ecirc;ncias ou dificuldades no reconhecimento do problema, apost&aacute;mos numa abordagem positiva, de um crescimento saud&aacute;vel e bem-estar global da crian&ccedil;a. Dedic&aacute;mos especial aten&ccedil;&atilde;o &agrave; consist&ecirc;ncia entre a imagem e o conte&uacute;do e procur&aacute;mos encontrar uma identidade para o projeto, com um nome, um logotipo e um <I>slogan</I> positivos e cativantes (<a href="#f2">fig. 2</a>). Assim, a designa&ccedil;&atilde;o &laquo;Papa Bem&raquo; decorre da constata&ccedil;&atilde;o da dificuldade dos pais em reconhecer o excesso de peso nos seus filhos e do seu grande interesse em ver os filhos &laquo;bem alimentados&raquo;. O <I>slogan</I> &laquo;Alimentar &eacute; educar&raquo; remete para a ideia central do projeto de que alimentar uma crian&ccedil;a &eacute; muito mais do que disponibilizar-lhe alimentos. Implica ter em conta o tipo de rela&ccedil;&atilde;o afetiva e pedag&oacute;gica entre educador e crian&ccedil;a no que diz respeito aos comportamentos alimentares, de sono, de sedentarismo e de atividade f&iacute;sica, em resposta &agrave;s necessidades globais da crian&ccedil;a em termos de desenvolvimento, quer f&iacute;sico quer psicol&oacute;gico quer social.</P>     ]]></body>

<P>Foram produzidos cerca de 300 textos de base para a estrutura do <I>Website</I>, 17 v&iacute;deos de curta dura&ccedil;&atilde;o (cerca de 3 minutos), 15 &aacute;udios de cerca de um minuto, 5 <I>quizzes</I> e 40 folhetos.</P>     <p>&nbsp;</p>     <P> <B>Avalia&ccedil;&atilde;o dos materiais</B> </P>     <P>Os materiais escritos produzidos foram revistos e reavaliados sucessivamente at&eacute; que todos os crit&eacute;rios da vers&atilde;o adaptada do teste <I>Suitability Assessment of Materials</I> recebessem pontua&ccedil;&atilde;o 2 (excelente) ou, em alguns casos, um (adequado), quando n&atilde;o se conseguiu encontrar melhor formula&ccedil;&atilde;o.</P>     <p>&nbsp;</p>     <P> <B>Implementa&ccedil;&atilde;o</B> </P>     <P>No que se refere &agrave; sensibiliza&ccedil;&atilde;o e capacita&ccedil;&atilde;o de profissionais de sa&uacute;de, participaram 33 profissionais de sa&uacute;de no <I>workshop</I> sobre produ&ccedil;&atilde;o de materiais de educa&ccedil;&atilde;o para a sa&uacute;de para a popula&ccedil;&atilde;o e 28 profissionais de sa&uacute;de no <I>workshop</I> sobre o modelo HENRY de interven&ccedil;&atilde;o na preven&ccedil;&atilde;o da obesidade infantil, incluindo m&eacute;dicos de fam&iacute;lia, enfermeiros, pediatras, psic&oacute;logos e m&eacute;dicos de sa&uacute;de p&uacute;blica. Para al&eacute;m da cria&ccedil;&atilde;o de uma linguagem e abordagem comum ao problema, estes <I>workshops</I> contribu&iacute;ram tamb&eacute;m para o fortalecimento da parceria com os profissionais.</P>     ]]></body>
<body><![CDATA[<P>A divulga&ccedil;&atilde;o do projeto e disponibiliza&ccedil;&atilde;o de materiais tem envolvido estrat&eacute;gias de dissemina&ccedil;&atilde;o orientadas pela Dire&ccedil;&atilde;o do Programa Harvard Medical School &ndash; Portugal em parceria com a Funda&ccedil;&atilde;o para a Computa&ccedil;&atilde;o Cient&iacute;fica Nacional e estrat&eacute;gias de dissemina&ccedil;&atilde;o elaboradas pela equipa de investiga&ccedil;&atilde;o no decorrer do pr&oacute;prio projeto.</P>     <P>Assim, de acordo com as orienta&ccedil;&otilde;es da dire&ccedil;&atilde;o do programa, os materiais desenvolvidos foram divulgados em diversos canais de informa&ccedil;&atilde;o, nomeadamente, ecr&atilde;s em grandes superf&iacute;cies comerciais, programas de r&aacute;dio e televis&atilde;o e p&aacute;ginas da Internet (<I>Facebook, Youtube</I> e <I>blog</I> do programa).</P>     <P>Relativamente &agrave;s estrat&eacute;gias de dissemina&ccedil;&atilde;o desenvolvidas pela equipa do projeto, a participa&ccedil;&atilde;o dos profissionais de sa&uacute;de da &aacute;rea-piloto nos <I>workshops</I> foi o primeiro passo deste processo.</P>     <P>Ap&oacute;s a etapa de produ&ccedil;&atilde;o e teste dos materiais, organiz&aacute;mos 3 reuni&otilde;es que contaram com a participa&ccedil;&atilde;o de 97 profissionais de sa&uacute;de, entre enfermeiros, m&eacute;dicos de fam&iacute;lia, m&eacute;dicos de sa&uacute;de p&uacute;blica, nutricionistas e psic&oacute;logos da &aacute;rea-piloto. Nestas reuni&otilde;es foram apresentados alguns materiais e disponibilizados exemplares aos participantes.</P>     <P>A Dire&ccedil;&atilde;o Geral de Sa&uacute;de, reconhecendo a validade das informa&ccedil;&otilde;es veiculadas pelos materiais produzidos, iniciou a disponibiliza&ccedil;&atilde;o dos materiais no seu <I>website</I> atrav&eacute;s de uma sec&ccedil;&atilde;o denominada &laquo;Papa Bem&raquo; que se encontra na &aacute;rea do Programa Nacional de Promo&ccedil;&atilde;o da Alimenta&ccedil;&atilde;o Saud&aacute;vel. Cabe referir que todos os v&iacute;deos desenvolvidos no projeto est&atilde;o dispon&iacute;veis na Internet atrav&eacute;s do <I>Youtube</I>, num canal denominado &laquo;Papa Bem&raquo;.</P>     <P>Foram ainda produzidos 200 DVD com todos os v&iacute;deos e folhetos desenvolvidos no projeto. Estes DVD t&ecirc;m sido distribu&iacute;dos em reuni&otilde;es de apresenta&ccedil;&atilde;o do projeto &laquo;Papa Bem&raquo; com profissionais do setor da sa&uacute;de e da educa&ccedil;&atilde;o e por requisi&ccedil;&atilde;o de algumas institui&ccedil;&otilde;es com interesse na divulga&ccedil;&atilde;o dos materiais.</P>     <P>Finalmente, no que se refere ao estabelecimento de contactos para promover a utiliza&ccedil;&atilde;o sistem&aacute;tica dos materiais, por proposta de uma das equipas dos cuidados de sa&uacute;de prim&aacute;rios que participou numa das reuni&otilde;es de divulga&ccedil;&atilde;o inici&aacute;mos a elabora&ccedil;&atilde;o de um projeto-piloto, designado por &laquo;Comunidades Papa Bem&raquo;, para o desenvolvimento de uma metodologia de dissemina&ccedil;&atilde;o das mensagens do projeto &laquo;Papa Bem&raquo; a contar com diversos mediadores a partir das comunidades.</P>     <p>&nbsp;</p>     <P> <B>Discuss&atilde;o</B> </P>     <P>O projeto &laquo;Papa Bem&raquo; foi iniciado com o objetivo de produzir e disponibilizar um conjunto alargado de informa&ccedil;&atilde;o, tendo em conta os fatores determinantes da obesidade infantil, incluindo uma abordagem integrada das v&aacute;rias vertentes de um estilo de vida saud&aacute;vel. O facto do projeto &laquo;Papa Bem&raquo; ter sido desenvolvido atrav&eacute;s de um projeto-piloto nos cuidados de sa&uacute;de prim&aacute;rios e respetivo hospital de refer&ecirc;ncia permitiu compilar informa&ccedil;&atilde;o relevante quanto aos principais pontos cr&iacute;ticos da compreens&atilde;o e gest&atilde;o do problema da obesidade infantil pelos diferentes atores: mediadores e pais.</P>     ]]></body>
<body><![CDATA[<P>Apesar do recurso a uma amostra de conveni&ecirc;ncia de mediadores e pais, com dimens&otilde;es reduzidas, os dados encontrados no diagn&oacute;stico da &aacute;rea-piloto est&atilde;o de acordo com os resultados de outras investiga&ccedil;&otilde;es<SUP>42&ndash;55</SUP>. A pouca disponibilidade dos pais, que foram abordados no momento das consultas dos filhos, e as limita&ccedil;&otilde;es de tempo para a concretiza&ccedil;&atilde;o dos objetivos do projeto, foram os principais obst&aacute;culos que levaram &agrave; limita&ccedil;&atilde;o do tamanho da amostra de pais e mediadores envolvidos no estudo. &Eacute; de referir ainda o maior envolvimento de m&atilde;es (93,1%), em compara&ccedil;&atilde;o com pais (6,9%), como ali&aacute;s &eacute; comum em estudos desta natureza<SUP>42,44,45,50</SUP>.</P>     <P>A metodologia usada na produ&ccedil;&atilde;o dos materiais investiu na transla&ccedil;&atilde;o do conhecimento produzido pela evid&ecirc;ncia cient&iacute;fica para a utiliza&ccedil;&atilde;o nas pr&aacute;ticas do dia-a-dia da popula&ccedil;&atilde;o, de acordo com as caracter&iacute;sticas e necessidades da mesma.</P>     <P>O resultado foi um conjunto diverso de materiais baseados na melhor evid&ecirc;ncia dispon&iacute;vel, adaptados &agrave;s necessidades espec&iacute;ficas expressas por diferentes atores e adequados a n&iacute;veis de literacia diversos, orientando a sua acessibilidade a pessoas com mais baixos n&iacute;veis de literacia. Os materiais t&ecirc;m sido divulgados e disponibilizados em contextos diversos e recebido o apoio entusiasta de profissionais com um papel central na preven&ccedil;&atilde;o da obesidade infantil e promo&ccedil;&atilde;o de um crescimento saud&aacute;vel.</P>     <P>A forma como s&atilde;o apresentadas as propostas de mudan&ccedil;as para reduzir a obesidade, bem como o papel dos mediadores na utiliza&ccedil;&atilde;o dos materiais dispon&iacute;veis basearam-se em modelos sobejamente comprovados como &uacute;teis para apoio na mudan&ccedil;a de comportamentos, tendo presente que a focaliza&ccedil;&atilde;o apenas no balan&ccedil;o energ&eacute;tico a n&iacute;vel individual &eacute; reducionista face a uma problem&aacute;tica complexa e multifacetada, que exige uma abordagem sist&eacute;mica.</P>     <P>Contudo, sabe-se que a obesidade n&atilde;o &eacute; um problema que se resolva, apenas, atrav&eacute;s da informa&ccedil;&atilde;o sobre como promover comportamentos saud&aacute;veis. O conhecimento e a capacidade cr&iacute;tica s&atilde;o componentes fundamentais para a mudan&ccedil;a de comportamentos pelo que &eacute; t&atilde;o importante investir numa literacia cr&iacute;tica. No entanto, este problema extravasa a c&eacute;lula familiar e est&aacute;, tamb&eacute;m, relacionado com a atividade das grandes ind&uacute;strias alimentares com as suas estrat&eacute;gias de marketing agressivas que colocam os seus produtos a pre&ccedil;os convidativos, induzindo, assim, o seu consumo<SUP>56</SUP>.</P>     <P>Conforme refere o relat&oacute;rio do <I>Institute of Medicine</I> sobre a preven&ccedil;&atilde;o da obesidade, a abordagem deste problema requer a combina&ccedil;&atilde;o de v&aacute;rias interven&ccedil;&otilde;es que ao serem integradas num programa compreensivo tornam-se componentes fundamentais para o seu sucesso<SUP>57</SUP>. Os materiais desenvolvidos poder&atilde;o ser parte integrante de programas desta natureza.</P>     <P>A oportunidade de fazer forma&ccedil;&atilde;o aos mediadores (profissionais de sa&uacute;de e educadores de creches, jardins-de-inf&acirc;ncia, autarquias, entre outros), conforme est&aacute; previsto no projeto-piloto de implementa&ccedil;&atilde;o, &laquo;Comunidades Papa Bem&raquo;, permitir&aacute; desenvolver compet&ecirc;ncias para identificar e compreender a influ&ecirc;ncia dos fatores contextuais, dos afetos &agrave;s capacidades cognitivas que determinam as caracter&iacute;sticas das escolhas e prefer&ecirc;ncias das comunidades e indiv&iacute;duos, dos recursos &agrave;s pol&iacute;ticas, numa abordagem socioecol&oacute;gica que tem em conta a forma como estes fatores interagem<SUP>58,59</SUP>.</P>     <P>&Eacute; no tri&acirc;ngulo &laquo;investiga&ccedil;&atilde;o, pr&aacute;ticas e pol&iacute;ticas&raquo;<SUP>60</SUP> que a combina&ccedil;&atilde;o dos resultados da evid&ecirc;ncia cient&iacute;fica nos v&aacute;rios dom&iacute;nios pode dar suporte e sustentabilidade a interven&ccedil;&otilde;es efetivas em que a conjuga&ccedil;&atilde;o de literacia em sa&uacute;de e pol&iacute;ticas relevantes, facilitadoras de escolhas saud&aacute;veis, poder&aacute; conduzir a pr&aacute;ticas de sucesso na resolu&ccedil;&atilde;o de problemas como o da obesidade infantil.</P>     <p>&nbsp;</p>     <P> <B>Conclus&otilde;es</B> </P>     ]]></body>
<body><![CDATA[<P>Face &agrave; magnitude e severidade que o problema da obesidade assume hoje em todo o mundo e considerando que os fatores de risco para a obesidade come&ccedil;am a surgir ainda durante a gravidez, &eacute; necess&aacute;rio encontrar estrat&eacute;gias mais eficazes para a sua preven&ccedil;&atilde;o em est&aacute;dios precoces.
