A quest&atilde;o abordada &eacute; pertinente, nomeadamente porque chama a aten&ccedil;&atilde;o para um aspecto negativo do j&aacute; reconhecidamente oneroso perfil de seguran&ccedil;a da clozapina<sup>1</sup>. Os casos relatados pelos autores s&atilde;o interessantes e did&aacute;ticos. Observei pessoalmente as 3 doentes descritas e em todas existiam mioclonias de caracter&iacute;sticas cl&iacute;nicas que apontam para uma origem cortical<sup>2</sup>; em dois casos surgiram crises epil&eacute;pticas e houve melhoria das mioclonias subsequentemente &agrave; redu&ccedil;&atilde;o da dose de clozapina. Como se pode depreender pela descri&ccedil;&atilde;o, o terceiro caso &eacute; clinicamente diferente, bem como o desfecho, uma vez que n&atilde;o surgiram crises epil&eacute;pticas, mesmo mantendo o regime terap&ecirc;utico. Logo, de um ponto de vista cient&iacute;fico rigoroso, n&atilde;o &eacute; linear que se deva considerar que a etiologia e a fisiopatologia subjacentes sejam semelhantes nos 3 casos. Por outro lado, no v&iacute;deo-EEG referido (feito apenas numa doente), o facto de ter sido registada uma mioclonia palpebral n&atilde;o constitui qualquer evid&ecirc;ncia de rela&ccedil;&atilde;o entre as mioclonias e os achados electroencefalogr&aacute;ficos de tipo epileptiforme, at&eacute; porque n&atilde;o foram feitas t&eacute;cnicas de <i>back-averaging</i>. O facto de as mioclonias serem corticais n&atilde;o implica uma origem epil&eacute;ptica, tal como demonstr&aacute;mos recentemente em casos de LOAM (<i>late-onset asymmetric myoclonus</i>)<sup>3</sup> e como tem sido apontado na literatura<sup>2</sup>. Penso que teria sido desej&aacute;vel aprofundar o estudo neurofisiol&oacute;gico, sobretudo no segundo e terceiro casos, no sentido de caracterizar melhor as mioclonias e explorar uma rela&ccedil;&atilde;o com uma eventual actividade epileptiforme, que n&atilde;o foi demonstrada no texto do ponto de vista cl&iacute;nico nem electroencefalogr&aacute;fico. Em circunst&acirc;ncias similares, a caracteriza&ccedil;&atilde;o cl&iacute;nica e neurofisiol&oacute;gica aprofundada &eacute; &uacute;til para guiar eficazmente as decis&otilde;es terap&ecirc;uticas, particularmente complexas nos casos de esquizofrenia refrat&aacute;ria, sobretudo quando se pondera a prescri&ccedil;&atilde;o de um f&aacute;rmaco com um perfil de seguran&ccedil;a como a clozapina. A revis&atilde;o da literatura dispon&iacute;vel, a que se juntam estes casos, sugere uma rela&ccedil;&atilde;o entre clozapina e mioclonias, mas nem sempre surgem crises epil&eacute;pticas nestes casos<sup>4</sup>. Por outro lado, as perturba&ccedil;&otilde;es psic&oacute;ticas e a administra&ccedil;&atilde;o de antipsic&oacute;ticos est&atilde;o independentemente associadas a um aumento significativo do risco de crises epil&eacute;pticas, e o efeito n&atilde;o &eacute; espec&iacute;fico da clozapina<sup>5</sup>. Por tudo isto, teria sido especialmente interessante explorar em detalhe a quest&atilde;o das mioclonias relacionadas com a clozapina, de uma forma independente das crises epil&eacute;pticas, n&atilde;o obstante a import&acirc;ncia cl&iacute;nica do assunto abordado e o didatismo da mensagem veiculada.</p>     <p>&nbsp;</p>     <p><b>REFER&Ecirc;NCIAS</b></p>     ]]></body>
