Na RAL a produção de IgE específica ocorre localmente, havendo evidência da presença de um padrão inflamatório Th2 a nível da mucosa nasal. Uma vez que os testes cutâneos por picada e as IgE específicas séricas são negativas, o diagnóstico de RAL implica a realização de provas de provocação nasal específicas com os aeroalergénios suspeitos. O seu tratamento farmacológico baseia-se na utilização das mesmas classes de medicamentos que habitualmente são usadas na rinite alérgica (corticoides intranasais e anti-histamínicos), apesar de não haver estudos de eficácia dirigidos. É de destacar, no entanto, que existem vários estudos a suportar o benefício e eficácia da imunoterapia com alergénios na RAL.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT Local allergic rhinitis (LAR) is a phenotype of rhinitischaracterizedd by the occurrence of clinical symptoms of suggestive allergic rhinitis, without evidence of IgE-mediated sensitization on skin prick tests or serum specific IgE quantification. In LAR the production of specific IgE is local and an inflammatory Th2 pattern in the nasal mucosa is present. Since skin prick tests and specific IgE are negative, a nasal allergen challenge with the suspected aeroallergens is required to make the diagnosis. Similarly, to allergic rhinitis, pharmacological treatment with oral antihistamines and intranasal corticosteroids are the treatment of choice, although their efficacy currently remains to be proven. Allergen specific immunotherapy may be a promising therapeutic approach in LAR.]]></p></abstract>
