<self-uri xlink:href="http://scielo.pt/scielo.php?script=sci_arttext&amp;pid=S0871-97212023000200137&amp;lng=en&amp;nrm=iso"></self-uri><self-uri xlink:href="http://scielo.pt/scielo.php?script=sci_abstract&amp;pid=S0871-97212023000200137&amp;lng=en&amp;nrm=iso"></self-uri><self-uri xlink:href="http://scielo.pt/scielo.php?script=sci_pdf&amp;pid=S0871-97212023000200137&amp;lng=en&amp;nrm=iso"></self-uri><abstract abstract-type="short" xml:lang="pt"><p><![CDATA[RESUMO  Fundamentos:  Os alergénios envolvidos na alergia alimentar exacerbada por cofator (CEFA) variam geograficamente; no Sul da Europa, as Lipid transfer proteins (LTP) têm maior relevância e na Ásia a ómega-5-gliadina (&#969;5G). As características clinicas parecem diferir consoante o alergénio envolvido.  Objetivos:  Comparação e caracterização dos doentes com CEFA a LTP (CEFA-LTP) e/ou &#969;5G (CEFA- &#969;5G).  Métodos:  Estudo observacional retrospetivo dos doentes com CEFA-LTP e/ou &#969;5G, entre 01/2012 e 12/2020. Análise demográfica, sintomas, diagnóstico e seguimento.  Resultados:  Incluídos 49 doentes: 34 com CEFA-LTP e 15 a &#969;5G. Grupo CEFA-LTP: média de idades de 30,0±14,7 anos e 50,0% do sexo masculino. O alimento suspeito foi tolerado na ausência de cofatores em 32,4% dos doentes. Os testes cutâneos por picada (TCP) com LTP do pêssego foram positivos em 80,2%. Grupo CEFA-&#969;5G: média de idades de 40,7±15,9 anos e 67,0% do sexo masculino. Todos toleravam trigo na ausência de cofatores. Os TCP com extrato de trigo foram positivos em 5/12 doentes (41,7%) e com gliadina em 10/12 (66,7%) [valor preditivo positivo (VPP): 90,9%]. O exercício foi o cofator mais frequente (74% CEFA-LTP e 87% CEFA-&#969;5G). A anafilaxia foi manifestação em 62,0% dos doentes com CEFA-LTP (2 com choque) e em 80,0% dos com CEFA-&#969;5G (5 com choque). Encontrou-se uma associação entre o alergénio envolvido e choque anafilático (p=0,02): os doentes com CEFA-&#969;5G tiveram oito vezes mais odd de choque anafilático do que os com CEFA-LTP (OR=8,0, IC 95% [1,34-47,7] (p=0,02).  Conclusões:  As LTP foram os alergénios mais frequentes. Contudo, considerando a tolerância ao trigo na ausência de cofatores, o baixo VPP do TCP com extrato de trigo e associação a reações mais graves, a sensibilização a &#969;5G deve ser ativamente pesquisada em todos os doentes com CEFA, para além da sensibilização a LTP.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT  Context:  Co-factor-Enhanced Food Allergy (CEFA) varies geographically; Lipid Transfer Proteins (LTP) are the most relevant allergens in southern Europe and omega-5-gliadine (&#969;5G) in Asia. CEFA characteristics may differ depending on the involved allergen.  Objective:  Comparison and characterization of patients with CEFA to LTP (CEFA-LTP) and /or &#969;5G (CEFA-&#969;5G).  Methods:  Observational retrospective study of patients with CEFA to LTP and/or &#969;5G (1/2012-12/2020). Demographics, symptoms, workup and management were analyzed.  Results:  Forty-nine patients were included: 34 with CEFA-LTP and 15 with CEFA-&#969;5G. CEFA-LTP group: mean age 30.0±14.7 years; 50.0% males. The eliciting food was tolerated in the absence of cofactors in 32.4% of patients. Skin prick tests (SPT) with peach LTP were positive in 80.2%. CEFA-&#969;5G group: mean age of 40.7±15.9 years and 67.0% male. All patients tolerated wheat in the absence of cofactors. SPT with wheat extract were positive in 5/12 (42%) and with gliadin in 10/12 (67%) [Positive Predictive Value (PPV): 90.2%]. Exercise was the most frequente cofactor (74% CEFA-LTP and 87% CEFA-&#969;5G). Anaphylaxis was reported in 62% of patients with CEFA-LTP (2 with shock) and in 80% of patients with CEFA-&#969;5G (5 with shock). There was an association between the allergen involved and anaphylactic shock (p=0.02): patients with CEFA-&#969;5G had 8 times the odd of having an anaphylactic shock of those with CEFA-LTP (OR=8.0, 95% CI [1.34-47.7] (p=0.02).  Conclusion:  LTP were the most frequent allergen. However, considering the tolerance to wheat in the absence of CF, the severity of the reactions and the low PPV of the wheat SPT, &#969;5G allergy should be actively searched in all patients with CEFA, along with LTP allergy.]]></p></abstract>
