A imunoterapia com veneno de vespídeos (ITVV) é o tratamento mais eficaz e seguro para prevenção de reações alérgicas graves. O objetivo deste estudo foi caraterizar doentes sob ITVV e avaliar a sua eficácia e segurança.  Métodos:  Realizou-se um estudo retrospetivo de doentes com história de reação sistémica a veneno de vespídeos submetidos a ITVV entre 2010 e 2024.  Resultados:  Foram incluídos 67 doentes, 54 homens, com mediana [mín-máx] de idade de início da ITVV de 44 [27-70] anos. As reações que motivaram a ITVV foram maioritariamente de grau IV (52,2%). Foi iniciada ITVV com veneno de Vespula spp em 43 doentes, de Polistes dominula em 12 e de Vespa velutina nigrithorax em 12. Durante a fase de iniciação nenhum doente apresentou reação sistémica. Na fase de manutenção, 2 doentes apresentaram reações sistémicas. Vinte e nove doentes foram repicados com uma mediana [mín-máx] de 24 [2-96] meses após o início da ITVV. Verificou-se uma diminuição estatisticamente significativa na gravidade das reações após ITVV (gravidade mediana [mín-máx]: 6 [3-6] antes vs. 1 [0-6] após ITVV, p&lt;0,001). Cinco doentes foram repicados por vespídeos distintos do da reação pré-ITVV: 4 apresentaram reação local e 1 apresentou reação do tipo III. Este último estava sob ITVV Polistes e foi picado por 8 Vespas velutinas. Trinta e dois doentes completaram ou suspenderam ITVV, com redução estatisticamente significativa do valor da IgE total e específica para os vespídeos (p&lt;0,05).  Conclusão:  Este estudo corrobora a eficácia e segurança da ITVV descritas na literatura. A realização de mais estudos e a partilha de experiências são cruciais para aumentar conhecimento e melhorar a orientação dos doentes.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT  Introduction:  Allergy to vespid venom is a common cause of anaphylaxis. Vespid venom immunotherapy (VVI) is the most effective treatment for preventing severe allergic reactions, both safe and effective. This study aimed to characterize patients undergoing VVI and to evaluate its efficacy and safety.  Methods:  A retrospective study was conducted on patients with a history of systemic reaction to vespid venom treated with VVI between 2010 and 2024.  Results:  Sixty-seven patients were included, of whom 54 were men, with a median [min-max] age at VVI initiation of 44 [27-70] years. The reactions prompting VVI were predominantly grade IV (52.2%). VVI was initiated with Vespula spp venom in 43 patients, Polistes dominula venom in 12, and Vespa velutina nigrithorax in 12. During the initiation phase, no patients experienced systemic reactions. In the maintenance phase, two patients experienced systemic reactions. Twenty-nine patients were re-stung at a median [min-max] of 24 [2-96] months after starting VVI. There was a statistically significant decrease in reaction severity after VVI initiation (median [min-max] severity: 6 [3-6] before vs. 1 [0-6] after VVI, p&lt;0.001. Five patients were re-stung by different vespids than those causing the initial reaction: 4 had local reactions and 1 had a type III reaction. The latter was on Polistes VVI and was stung by 8 Vespa velutina. Thirty-two patients completed or suspended VVI, with a statistically significant reduction in total and vespid-specific IgE levels (p&lt;0,05).  Conclusion:  This study supports the efficacy and safety of VVI as described in the literature. Further studies and the sharing of experiences from different centers are essential to increase knowledge in the field and improve patient management.]]></p></abstract>
