Endocrinologia e Diabetes Infantil, Hospital Dona Estefânia </p>      <p>&nbsp;</p>      <p>A Diabetes Mellitus é uma doença de elevada e crescente incidência com um grande impacto socioeconómico <sup>(1)</sup>. As terapêuticas actualmente disponíveis, destinadas à normalização da glicémia, se bem que razoavelmente eficazes e largamente utilizadas, são exclusivamente sintomáticas. Surge pois a necessidade de actuar no <i>primum movens</i> da doença, com uma &#64257;nalidade curativa. </p>      <p>Tudo indica, pelas experiências dos últimos 10 anos que será possível, num futuro não muito longínquo alcançar a cura da diabetes tipo 1 (DT1). Esta irá depender de estratégias ba­seadas em <b>terapias celulares</b> seja de transplante de células ou regeneração celular do pâncreas bem como de <b>tratamentos imunomoduladores, </b>fundamentais para o controlo da autoimunidade. </p>      <p><b>O transplante de ilhéus pancreáticos humanos </b>demonstrou que a insulino-independência prolongada com normalização da glicémia pode ser alcançada em doentes com DT1.No entanto, esta forma de terapêutica celular apresenta grandes limitações devidas à escassez de órgãos de dadores, à necessidade de imunosupressão crónica nestes doentes, para além de que o tempo de sobrevivência do enxerto é apenas de 3 anos <sup>(2)</sup>. </p>      <p>Assim, fontes alternativas de células produtoras de insulina têm sido intensamente investigadas nos últimos anos. Estudos de diferenciação de<b> células estaminais embrionárias e células estaminais adultas </b>de origem pancreática ou extra-pancreática, em tipos celulares semelhantes às células beta do ilhéu de Langerhans mostraram-se bastante encorajadores <sup>(3)</sup>. Técnicas recentes de reprogramação celular in vitro permitiram já gerar, a partir de células somáticas, células pluritipotentes <b>(iPS) </b><sup>(4)</sup>. Estas podem ser induzidas em células diferenciadas de múltiplos tecidos, incluindo células insulínicas <sup>(5)</sup>. </p>      <p>Contudo, as células geradas até à data, carecem ainda de características importantes das células &#946; maduras, nomeadamente a secreção adequada de insulina em resposta ao estímulo da glicose. Além do mais, muitas destas células reprogramadas podem conduzir ao aparecimento de tumores quando transplan­tadas em ratinhos diabéticos <sup>(3)</sup>. </p>      ]]></body>
<body><![CDATA[<p><b>A regeneração de células do pâncreas</b> endócrino na DT1, tornou-se num desa&#64257;o. A descoberta de inúmeros factores e bio­moléculas com efeito tró&#64257;co na massa de células beta constitui um grande avanço científico. São exemplos a nicotinamida, betacelulina, a gastrina, a exendina e outros agonistas dos recep­tores da glucacon-like-peptide 1 (GLP-1) podendo, alguns deles, vir a ser associado ao tratamento da diabetes tipo1<sup>(6)</sup>. </p>      <p>Não devemos no entanto esquecer que a Diabetes tipo 1 é, acima de tudo, uma doença imunológica. As mais recentes correntes de investigação, defendem que a causa primária da destruição da célula beta, está na perda da tolerância imunológica. Esta traduz-se num desequilíbrio entre populações de células T reguladoras e células T citotóxicas, associado à libertação inapropriada de citoquinas pro-in&#64258;amatórias e anti-in&#64258;amatórias cujo papel na manutenção do balanço imunológico é fundamental <sup>(7)</sup>. </p>      <p>Inúmeros estudos em modelos animais de DT1, bem como ensaios clínicos utilizando terapêuticas com Ac anti CD20, anti CD3, anti IL1, em doentes diabéticos estão actualmente em curso <sup>(8)</sup>. </p>      <p>Não restam dúvidas, de que a implementação real e eficaz de novas terapias para o tratamento da Diabetes, bem como de outras doenças degenerativas, dependerá dos esforços conjuntos da investigação básica e de uma nova área médica, translac­cional, a <b>Medicina Regenerativa. </b></p>      <p><b>&nbsp;</b></p>      <p><b>BIBLIOGRAFIA </b></p>        <!-- ref --><p>1.
