Atualmente, através de equipamentos sofisticados como a hipertermia de corpo inteiro por infravermelhos, têm sido utilizados na área da dor, em patologias como a fibromialgia, na área oncológica (tumores), verificando-se paralelamente uma melhoria dos sintomas depressivos. Pretende-se apresentar uma revisão da literatura sobre a utilização da hipertermia na depressão e estados emocionais. Método: Pesquisa nas bases Ebsco/Medline/Pubmed, em Setembro/Outubro 2019, com estudos em língua inglesa, analisados em função da amostra, intervenção e principais resultados. Resultados: Encontraram-se 12 estudos elegíveis, publicados entre 2011 e 2019 (embora um em 1992), sendo um com animais, um de descrição histórica do tratamento, um de revisão teórica na aplicação à depressão e três de comentários. Assim, seis estudos analisaram pacientes com sintomas depressivos, encontrando melhorias no estado emocional após a intervenção, nomeadamente com o aumento da temperatura corporal de um grau. Discussão: Os resultados sugerem que a hipertermia parece constituir uma intervenção promissora na melhoria dos estados emocionais e da saúde psicológica, sobretudo nos sintomas depressivos, podendo ser combinada com intervenções farmacológicas ou de outro tipo.]]></p></abstract>

Currently, sophisticated equipment as infrared full body hyperthermia have been used in pain, pathologies such as fibromyalgia and in oncology (tumors), and results show a parallel improvement in depressive symptoms. This study aims to present a literature review about the use of hyperthermia in depression and emotional states. Method: Ebsco/Medline/Pubmed databases were explored in September/October 2019 and studies in English were analyzed according to sample, intervention and main results. Posteriormente,   na área da oncologia, no tratamento de tumores, a hipertermia tem sido   utilizada em concomitância com a radioterapia e quimioterapia, enquanto   modalidade médica que através dos efeitos biológicos de febre artificialmente   induzida potencia as respostas imunológicas (Skitzki, Repasky, &amp; Evans, 2009; Yagawa, Tanigawa, Kobayashi, &amp; Yamamoto, 2017). </p>      <p>Atualmente, através de equipamentos sofisticados, nomeadamente a hipertermia de corpo inteiro por infravermelhos (Hanusch &amp; Janssen, 2019), nas intervenções mais simples, a aplicação consiste numa indução artificial de calor, estando o utilizador monitorizado em termos de sinais vitais e com a medição da temperatura através de uma sonda sublingual. O equipamento faz subir a temperatura corporal até cerca dos 38.5 graus, o que variando em função das características do utilizador (ex.: pele, gordura abdominal) pode demorar cerca de uma hora. Atingida esta temperatura, é interrompido o aquecimento artificial, procedendo-se a uma etapa de manutenção desta temperatura durante cerca de uma hora. A intervenção termina quando após esta hora de <i>plateau</i> a temperatura desce para os 37 graus. As intervenções na área da oncologia apresentam outras características em função do tumor, sendo utilizadas temperaturas mais elevadas (entre 38,5 e 40,5 graus, ou com sedação acima dos 40,5 graus segundo SOMS, 2019).</p>      <p>Em Portugal, na área da oncologia já em 1987 Patrício defendeu, na Universidade Nova de Lisboa, uma tese de doutoramento sobre &#8220;<i>Efeitos da hipertermia na radioterapia (estudos vasculares)</i>&#8221;. Mais recentemente, na área da oncologia vários trabalhos têm sido apresentados descrevendo os efeitos positivos do tratamento com recurso à hipertermia de corpo inteiro ou hipertermia localizada (Costa et al., 2019; Moreira-Pinto et al., 2017, 2018a, 2018b).</p>      <p>Atualmente, para além da área da oncologia, estudos têm sido realizados na fibromialgia (Borckow, Wagner, Franke, Offenbacher, &amp; Resch, 2007; Hauser et al., 2012; Romeyke &amp; Stummer, 2014), reforçando a ideia de alterações biológicas que estimulam a resposta imunitária (Manjili et al., 2002; Puri, Ijej, &amp; Monro, 2019). Paralelamente, nas investigações em oncologia e fibromialgia começou a ser referida a melhoria de sintomas depressivos, surgindo estudos sobre a aplicação da hipertermia de corpo inteiro no âmbito da depressão (Hanusch &amp; Janssen, 2019). </p>      <p>Pretende-se apresentar uma revisão da literatura sobre a utilização da hipertermia na depressão e sua influência nos estados emocionais.</p>     <p><b>Método</b></p>     ]]></body>
<body><![CDATA[<p>Entre Setembro e Outubro 2019 foi realizada uma pesquisa nas bases Ebsco/Medline/Pubmed, utilizando como equação de pesquisa &#8220;<i>depression or depressive (or) mood (or) emotional sates (and) hyperthermia</i>&#8221; com estudos em língua inglesa, posteriormente analisados em função do tipo de estudo e amostra, características da intervenção e principais resultados obtidos.</p>     <p><b>Resultados</b></p>     <p>Encontraram-se 12 estudos elegíveis, publicados entre 1992 e 2019 (<a href="#f1">Figura 1</a>), com 4 estudos em 2016 e 2 em 2017 e 2019, sugerindo um interesse crescente sobre o tema. </p>     <p>&nbsp;</p>    <p align="center"><a name="f1"></a><img src="/img/revistas/psd/v21n1/21n1a31f1.jpg"/></p>    
<p>&nbsp;</p>     <p>Verificou-se (<a href="/img/revistas/psd/v21n1/21n1a31q1.jpg" target="_blank">Quadro 1</a>) que um estudo foi   realizado com animais (Hale et al., 2017) e implicou a administração de um   inibidor da captação de serotonina, um de descrição histórica da hipertermia   (Woesner, 2019), um de revisão teórica na aplicação à depressão (Hanusch &amp;   Janssen, 2019) e 3 de comentários críticos ao estudo empírico de Janssen e   colaboradores realizado em 2016 (Berk et al., 2016; Fink &amp; Shorter, 2016;   Raison et al., 2016). Verificou-se que 6 estudos analisaram pacientes com   sintomas depressivos, encontrando melhorias no estado emocional após a   intervenção, nomeadamente com o aumento da temperatura corporal de um grau. Contudo,   apenas 4 foram centrados em pacientes com depressão, enquanto dois incluíram   pacientes com fibromialgia (Romeyke &amp; Stummer, 2014) ou cancro (Koltyn et   al., 1992). Nos   estudos de tipo empírico, existiu um número variável de sessões ou apenas uma   sessão, bem como patamares diferentes de temperatura induzida, embora   predominem os 38.5 graus, valor de referência para a hipertermia de baixa   intensidade (<i>Mild Whole Body Hyperthermia</i>), tolerada mais facilmente   pelo participante (Puchinger et al., 2009; SOMS, 2019). A duração da sessão   parece aproximar-se das três horas no total, com cerca de uma hora na fase de   retenção, enquanto a fase inicial do aumento da temperatura corporal pode variar entre 50 e 127 minutos.</p>

<body><![CDATA[<p><b>Discussão</b></p>     <p>Os estudos sobre o efeito benéfico na depressão foram desenvolvidos na Europa por Hanusch e Janssen, pois foi em 2012 que Kay Hanusch apresentou a sua tese de Mestrado sobre &#8220;<i>Antidepressive effect through heat therapy - Influence of the psychical condition of depressive patients through passive whole-body hyperthermia: Literature survey and clinical study</i>&#8221;. Contudo, a Universidade do Arizona (EUA) registou já em 2012 (atualizado em 2015) um ensaio clínico sobre hipertermia de corpo inteiro com aumento de temperatura até 38,5 graus e depressão <i>major</i>, com 34 participantes e um <i>follow up</i> 3 meses (<a href="https://clinicaltrials.gov/ct2/show/NCT01625546" target="_blank">https://clinicaltrials.gov/ct2/show/NCT01625546</a> ), bem como outro em 2014 aplicado à perturbação de stress pós-traumático (<a href="https://clinicaltrials.gov/ct2/show/NCT02077972" target="_blank">https://clinicaltrials.gov/ct2/show/NCT02077972</a> ). Na Alemanha, em outubro/2018, o Centro Médico e de Investigação <i>Charité</i>, da Universidade de Berlim, registou o protocolo DRKS00015754 também para a utilização da hipertermia de corpo inteiro por infravermelhos com aumento de temperatura até 38,5 graus aplicada à depressão. Em abril 2019 e até 2021, a Universidade Duisburg-Essen registou um ensaio (<a href="https://clinicaltrials.gov/ct2/show/NCT03906175" target="_blank">https://clinicaltrials.gov/ct2/show/NCT03906175</a> ) para utilização do mesmo tipo de hipertermia em 46 participantes com depressão. Em Portugal, para além da hipertermia já estar a ser utilizada na área da oncologia desde 2017, a Escola Superior de Saúde (Politécnico do Porto) e a Faculdade de Psicologia e de Ciências da Educação (Universidade do Porto) estão desde novembro 2019 com um estudo piloto sobre o impacto da hipertermia nos estados emocionais, em participantes sem diagnóstico de patologia psiquiátrica/psicológica, aferindo procedimentos, para em 2020 efetuarem a sua aplicação à depressão e ao stress/<i>burnout.</i></p>      <p>Apesar de serem ainda escassos os estudos, os resultados sugerem que a hipertermia parece constituir uma intervenção promissora na melhoria dos estados emocionais e da saúde psicológica, sobretudo nos sintomas depressivos, podendo ser combinada com intervenções farmacológicas ou de outro tipo. Contudo, são necessários mais estudos que analisem com detalhe este efeito benéfico e isolem o contributo da hipertermia na melhoria do humor, bem como possíveis efeitos adversos. A aplicação que já decorre na área da oncologia (Costa et al., 2019) revela alterações fisiológicas benéficas no tratamento de tumores, sendo necessário explorar o impacto da hipertermia a nível fisiológico e psicológico nas alterações do humor. </p>      <p>Em Portugal, o consumo de antidepressivos tem aumentado exponencialmente (OECD, 2019) e outras situações do foro patológico têm também aumentado e surgem associadas a sintomas depressivos, com destaque para a ansiedade, stress e <i>burnout</i> (recentemente definido pela Organização Mundial de Saúde como um fenómeno ocupacional a incluir na próxima versão da CID-11 segundo a WHO, 2019). Assim, novas formas de intervenção poderão ser desenvolvidas e utilizadas, como por exemplo a hipertermia de baixa intensidade e de corpo inteiro por infravermelhos, complementarmente a intervenções de tipo farmacológico ou psicoterapêutico (Hanusch &amp; Janssen, 2019). </p>     <p>&nbsp;</p>     <p><b>REFERÊNCIAS</b></p>     <!-- ref --><p>Berk, M., Tye, S., Walder, K., &amp; McGee, S.
