Os resultados demonstraram um efeito significativo do tempo de institucionalização (p= 0.003) na depressão; e da escolaridade na função cognitiva (p= 0.030). Os resultados da interação escolaridade x idade foram significativos na função cognitiva (p= 0.010) e na capacidade intelectual (p= 0.032). Os indivíduos mais velhos e com nível de escolaridade superior apresentaram resultados mais elevados na função cognitiva e na capacidade intelectual. Podemos concluir que em idosos mais velhos, a escolaridade é a variável mais determinante da função cognitiva, enquanto a depressão depende do tempo de institucionalização.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[The purpose of this study was verifying the effect and interaction between age, schooling, institutionalisation and physical exercise in the cognitive function and depression in the elderly. The study comprised 39 institutionalized senior citizens (33 females and 6 males) aged between 64 and 96 (82.13±6.66). We used as evaluation tools: Mini Mental State Examination, Geriatric Depression Scale, Raven’s Coloured Progressive Matrices and a socio-demographic questionnaire. In order to understand the effect and interaction between variables we used the General Linear Model, introducing the time of institutionalization as a co-variable, and age (<85 years; &#8805;85 years), schooling (illiterate; 1st cycle of basic education) and physical exercise as variables. The results revealed a statistically significant difference regarding the time of institutionalization when dealing with depression (p= 0.003); and regarding schooling in the cognitive function (p= 0.030). Also, results concerning schooling versus age were significantly different in the cognitive function (p= 0.010) and intellectual ability (p= 0.032). Older individuals with higher levels of education presented superior results regarding the cognitive function and intellectual ability. We can thus infer that when it comes to older seniors schooling is the most decisive variable of the cognitive function, while depression depends on the time of institutionalization.]]></p></abstract>

<body><![CDATA[<p><font size="3" face="verdana"><b>INTRODUÇÃO</b></font></p>     <p><font size="2" face="verdana">Na área da saúde mental, as doenças neuropsiquiátricas, como as   demências e a depressão assumem um papel de grande destaque, com o avançar da   idade (Pláteník et al., 2014; Pocinho, 2007; Williams &amp; Kemper, 2010),   ameaçando a independência e a qualidade de vida dos idosos (Colasanti, Marianetti, Micacchi, Amabile, &amp; Mina, 2010; Williams &amp; Kemper, 2010).</font></p>     <p><font size="2" face="verdana">As mudanças ocorridas na função cognitiva (processamento de informação,   perceção, aprendizagem, memória, atenção, vigilância, raciocínio e solução de   problemas) (Chodzko-Zajko &amp; Moore, 1994), na terceira idade, conduzem a um   comprometimento cognitivo (Paúl, Ribeiro, &amp; Santos, 2010), como   consequência das alterações quer a nível estrutural quer funcional do cérebro,   em particular da memória (Vidal, Martinez, Pereira, &amp; Martínez-Patiño,   2011). Estudos realizados neste âmbito comprovaram que o hipocampo, o cerebelo   e o córtex pré-frontal são as zonas cerebrais que apresentam um maior decréscimo (Raz et al., 2005).</font></p>     <p><font size="2" face="verdana">Com o decorrer da idade, não se registam apenas alterações a nível   cognitivo, mas também, transformações no funcionamento psicomotor, com   implicações no tempo de reação, tempo de movimento e velocidade de desempenho (Chodzko-Zajko &amp; Moore, 1994).</font></p>     <p><font size="2" face="verdana">Apesar da grande heterogeneidade deste fenómeno entre os idosos, a reserva   cognitiva é um dos fatores que contribui para acentuar as diferenças   individuais, apresentando uma proteção parcial contra os efeitos do   envelhecimento (Bastin et al., 2012). Neste contexto, é consensual na   literatura, o papel fulcral da educação (Kramer, Bherer, Colcombe, Dong, &amp;   Greenough, 2004; Sattler, Toro, Schönknecht, &amp; Schröder, 2012), e sua   influência no funcionamento cognitivo (O’Bryant et al., 2007). Os idosos com   nível de escolaridade superior apresentam maior volume de massa branca e cinzenta,   principalmente nos lobos temporoparietal e orbitofrontal, compensando os sinais   de declínio cognitivo que se verificam 15 anos antes. Contudo, os de nível de   escolaridade inferior apenas são evidentes 7 anos antes (Amieva et al., 2014).   Segundo os mesmos autores, a reserva cognitiva funciona como um mecanismo   compensatório dos idosos com nível de escolaridade superior, protegendo-os de um declínio cognitivo mais severo.</font></p>     <p><font size="2" face="verdana">O quadro de depressão é de igual modo um problema grave de saúde mental   e pública (Dillon et al., 2011; Luppa et al., 2012; Pocinho, Farate, Dias, Lee,   &amp; Yesavage, 2009), tendo sido associado com o comprometimento cognitivo e   funcional (Dillon et al., 2011; Reppermund et al., 2011), destacando-se   principalmente os domínios da velocidade de processamento de informação,   memória episódica e funções executivas (Story, Potter, Attix, Welsh-Bohmer,   &amp; Steffens, 2008). Os baixos níveis de alfabetização (Teixeira,   Vasconcelos-Raposo, Fernandes, &amp; Brustad, 2013), a progressiva perda do   papel do idoso na sociedade, o luto, o sentimento de isolamento social, assim   como a institucionalização são considerados como fatores adjuvantes ao   desenvolvimento de sintomatologia depressiva (Carmen, 2013; Runcan, 2012),   apresentando um maior declínio cognitivo (Akdag, Telci, &amp; Cavlak, 2013;   Harmand et al., 2014), com implicações negativas ao nível da saúde (Ford, 2014), especialmente em idades mais avançadas (Gustafson et al., 2013). </font></p>     <p><font size="2" face="verdana">O exercício físico é hoje considerado uma necessidade absoluta para o   homem (Antunes et al., 2006), e uma estratégia eficaz para a promoção da saúde   em geral (Hötting &amp; Röder, 2013; Lautenschlager, Cox, &amp; Cyarto, 2012;   Vidoni et al., 2012). A prática de atividade física regular tem sido associada   a uma série de efeitos benéficos na saúde física e mental, e a inatividade   física considerada como o quarto fator de risco para a mortalidade global (WHO,   2010). Atua como um fator neuroprotetor, proporcionando melhorias   significativas na função cognitiva global em indivíduos que apresentam declínios   cognitivos leves ou demência (Heyn et al., 2004). Várias investigações   salientam a sua ação benéfica sobre a cognição (Colcombe &amp; Kramer, 2003;   Hötting &amp; Röder, 2013; WHO, 2010), sendo de destacar, em particular, o   contributo do exercício aeróbio, o qual produz benefícios ao nível do   funcionamento cognitivo, especialmente na função executiva em pessoas idosas   (Colcombe &amp; Kramer, 2003), assim como, na modulação da depressão (Deslandes, 2013; Underwood et al., 2013). </font></p>     <p><font size="2" face="verdana">Neste contexto, e de acordo com os resultados dos estudos realizados,   temos como objetivo verificar o efeito e interação entre a idade, escolaridade,   institucionalização e exercício físico na função cognitiva e depressão em idosos.</font></p>     <p>&nbsp;</p>     <p><font size="3" face="verdana"><b>MÉTODO</b></font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="verdana"><b>Amostra</b></font></p>     <p><font size="2" face="verdana">Foi utilizada uma amostra de conveniência constituída por 39 idosos   institucionalizados (33 mulheres e 6 homens), com idades compreendidas entre os 64-96 anos (82.13 ± 6.66), do distrito de Vila Real. </font></p>     <p><font size="2" face="verdana">Definiu-se como critério de inclusão ter idade igual ou superior a 60   anos, e como de exclusão apresentarem qualquer tipo de défice que   impossibilitasse de responder aos questionários. O presente estudo foi aprovado   pelo Comitê de Ética e seguiu os padrões éticos da declaração de Helsínquia, recentemente publicado por Harris e Atkinson (2009).</font></p>     <p><font size="2" face="verdana"><b>Instrumentos </b></font></p>     <p><font size="2" face="verdana"><i>Função cognitiva</i></font></p>     <p><font size="2" face="verdana">A avaliação da função cognitiva foi realizada através do teste <i>Mini   Mental State Examination</i> (MMSE), desenvolvido por Folstein, Folstein, e   McHugh (1975), e adaptado à população portuguesa por Guerreiro et al. (1994). É   um instrumento conhecido e amplamente utilizado em rastreios de deterioração   cognitiva e de quadros demenciais, sendo a sua aplicação bastante rápida   (sensivelmente 15 min) e fácil (Folstein, Folstein, &amp; McHugh, 1975). O MMSE   abrange a avaliação de diferentes domínios cognitivos, nomeadamente: a   orientação, atenção, memória, linguagem e a habilidade visuoconstrutiva. A sua   pontuação máxima é de 30 pontos, sendo que valores mais elevados indicam um   melhor desempenho cognitivo. Foram estabelecidos os seguintes valores de corte,   tendo por base que existe défice cognitivo em: Analfabetos &#8804; 15; 1 a 11   anos de escolaridade &#8804; 22; Escolaridade superior a 11 anos &#8804; 27)   (Morgado, Rocha, Maruta, Guerreiro, &amp; Martins, 2009). O coeficiente de   consistência interna foi calculado utilizando o valor do <i>Alpha de Cronbach</i>,   tendo sido obtido o valor de 0.873, o que revela uma elevada consistência interna (Tabachnick &amp; Fidell, 1996). </font></p>     <p><font size="2" face="verdana"><i>Depressão</i></font></p>     <p><font size="2" face="verdana">A Escala de Depressão Geriátrica (GDS-27) foi desenvolvida por Yesavage   et al. (1983), e validada para a população portuguesa por Pocinho e   colaboradores (2009), sendo utilizada no rastreio da depressão e criada   especificamente para a população idosa (Pocinho et al., 2009; Yesavage et al.,   1983). A GDS-27 é composta por 27 itens de respostas dicotómicas (sim ou não) e   avalia o modo como o idoso se tem sentido ultimamente, em especial ao longo da   última semana. A identificação de 11 ou mais sintomas nesta versão é utilizada   como ponto de corte entre ausência e presença de depressão (Pocinho et al.,   2009). Foi calculada a consistência interna do teste através do <i>Alpha de     Cronbach</i>, tendo sido obtido o valor de 0.857, o que revela uma elevada consistência interna (Tabachnick &amp; Fidell, 1996). </font></p>     <p><font size="2" face="verdana"><i>Capacidade intelectual</i></font></p>     <p><font size="2" face="verdana">As Matrizes Progressivas Coloridas de Raven (MPCR) foram desenvolvidas   por John Raven em 1947 e avaliam a capacidade intelectual, na componente   não-verbal (Raven, 1956; Simões, 2000). Foram validadas para português por   Simões (2000, 2012), sendo compostas por 36 itens, divididos em 3 séries de 12   problemas, dispostos segundo uma ordem de dificuldade crescente (A, Ab e B). O   teste consiste na apresentação de uma imagem incompleta, à qual o indivíduo tem   de escolher uma das seis alternativas possíveis de modo a completar o desenho,   através da capacidade de raciocínio, por analogia. Cada item respondido   corretamente é cotado com 1, e 0 pontos quando incorretamente, variando a   cotação final entre 0-36, fornecendo três resultados: o bruto, o percentil e a   classificação qualitativa (Raven, 1956). Foi calculada a consistência interna   da escala através do <i>Alpha de Cronbach</i>, tendo sido obtido o valor de   0.531, o que revela uma consistência interna baixa (Tabachnick &amp; Fidell, 1996). </font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="verdana"><i>Questionário Sociodemográfico</i></font></p>     <p><font size="2" face="verdana">Para avaliação das variáveis sociodemográficas (idade, género, estado   civil, escolaridade, tempo de institucionalização, prática de exercício físico,   frequência e tempo semanal, histórico clínico básico) foi aplicado um questionário.</font></p>     <p><font size="2" face="verdana"><b>Procedimentos</b></font></p>     <p><font size="2" face="verdana">Inicialmente foi contactado o presidente da instituição com o intuito de   o informar acerca do estudo a efetuar, solicitando autorização para a   realização do mesmo. Após informados os participantes dos objetivos do estudo,   foi obtido o Termo de Consentimento Livre e Esclarecido por escrito, sendo-lhes   garantido o princípio de confidencialidade (podendo estes interromper a sua   participação a qualquer momento). Os testes/questionário foram aplicados   individualmente a cada idoso, em contexto de mesa, e preenchidos pelo investigador   responsável, devido à elevada iliteracia por parte da amostra. Todas as   avaliações foram realizadas por uma psicomotricista treinada e acompanhada pelo psicólogo da instituição.</font></p>     <p><font size="2" face="verdana"><b>Análise estatística</b></font></p>     <p><font size="2" face="verdana">Para o tratamento estatístico dos dados foi utilizado o programa SPSS   19.0 (<i>Statistical Package for the Social Sciences</i>). Para testar a   normalidade das distribuições recorremos ao teste de <i>Kolmogorov-Smirnov</i>.   Foi aplicado o modelo geral linear (GLM), de forma a verificar a interação   entre variáveis, introduzindo como covariável o tempo de institucionalização, e   como variáveis fator a idade (&lt; 85 anos; &#8805; 85 anos), a escolaridade   (analfabetos; 1º ciclo) e exercício físico (praticantes e não praticantes). O nível de significância foi de 5% (<i>p </i>&#8804; 0.05).</font></p>     <p>&nbsp;</p>     <p><font size="3" face="verdana"><b>RESULTADOS</b></font></p>     <p><font size="2" face="verdana">Relativamente ao estado civil dos idosos que integraram a amostra, 66.7%   eram viúvos, 20.5% solteiros e os restantes 12.8% casados. Quanto ao nível de   escolaridade, 48.7% dos participantes eram analfabetos e 51.3% detentor do   ensino primário. O tempo de institucionalização variou entre 1 e 108 meses   (32.21 ± 32.48). No que diz respeito à prática de exercício, apenas 51.3% eram praticantes regulares com uma frequência bissemanal, 120 min/semana. </font></p>     <p><font size="2" face="verdana">A <a href="/img/revistas/mot/v12n2/12n2a06t1.jpg">Tabela 1</a> apresenta a análise descritiva das variáveis dependentes   deste estudo. Na amostra total (<i>N </i>= 39), a maioria dos idosos apresentou   défice cognitivo (82.1%). No respeitante à capacidade intelectual, apenas 5.1%   apresentaram um desempenho cognitivo “intelectualmente médio”, os restantes   situaram-se entre “abaixo da média” (66.7%) e “intelectualmente deficiente”   (28.2%). Os resultados da GDS indicaram evidências da presença de depressão em 69.2% dos idosos. </font></p>

<body><![CDATA[<p><font size="2" face="verdana">A <a href="/img/revistas/mot/v12n2/12n2a06t3.jpg">Tabela 3</a> apresenta os efeitos principais e as interações obtidos pela   GLM. Tal como se pode constatar, os resultados demonstraram um efeito   significativo da escolaridade na função cognitiva (<i>p</i>= 0.030); e do tempo   de institucionalização (<i>p</i>= 0.003) na depressão. Relativamente aos   resultados da interação escolaridade x idade, estes foram significativos na   função cognitiva (<i>p</i>= 0.010), e na capacidade intelectual (<i>p</i>= 0.032).</font></p>     
<p><font size="2" face="verdana">Verificou-se ainda que os indivíduos mais velhos (&#8805; 85 anos) e com   nível de escolaridade superior (1º ciclo) revelaram desempenhos superiores na função cognitiva e na capacidade intelectual geral (ver <a href="/img/revistas/mot/v12n2/12n2a06t2.jpg">Tabela 2</a>; <a href="/img/revistas/mot/v12n2/12n2a06t4.jpg">Tabela 4</a>). </font></p>     
<p>&nbsp;</p>     <p><font size="3" face="verdana"><b>DISCUSSÃO</b></font></p>     <p><font size="2" face="verdana">O presente estudo revelou que a maioria dos idosos institucionalizados   apresentou comprometimento das habilidades cognitivas, assim como uma elevada   incidência de sintomatologia depressiva, resultados merecedores de atenção face   ao atual panorama de envelhecimento da população, em Portugal (Instituto   Nacional de Estatística [INE], 2012). O nosso estudo evidenciou um efeito   significativo do tempo de institucionalização na depressão, o que corrobora   resultados de outras investigações as quais ressalvam que este é um problema   comum que acomete indivíduos que vivem permanentemente numa unidade   institucional (Pimentel, Afonso, &amp; Pereira, 2012; Runcan, 2012). Sabemos   que idosos institucionalizados são mais suscetíveis a doenças psiquiátricas, e   vulneráveis à depressão (Runcan, 2012), apresentando de igual modo um maior   declínio cognitivo comparativamente aos que permanecem na comunidade (Akdag,   Telci, &amp; Cavlak, 2013; Harmand et al., 2014). Estas condições são comuns e   ocorrem frequentemente em conjunto (Korostiy &amp; Dubenko, 2012), sendo   notório o impacto negativo sobre a saúde (Ford, 2014). Os nossos resultados,   confirmam esta realidade, à semelhança de um estudo levado a cabo por Oliveira   e colaboradores, e demonstram que a grande maioria das instituições apresenta um   ambiente empobrecido (Oliveira et al., 2014), responsável pelo agravamento do   declínio cognitivo e da depressão, devido ao número reduzido de atividades   cognitivas, físicas e sociais, e ao predomínio de hábitos sedentários (Fiske, Wetherell, &amp; Gatz, 2009; Rolland, Kan, &amp; Vellas, 2008). </font></p>     <p><font size="2" face="verdana">Apesar de a nossa amostra apresentar uma escolaridade baixa, da análise   dos resultados obtidos podemos referir um efeito do nível de escolaridade na   pontuação do MMSE, confirmando a tendência de estudos anteriores (Huang &amp;   Zhou, 2013; Meng &amp; D’Arcy, 2012). É de notar o seu papel como um dos   agentes responsáveis no desempenho da função cognitiva, sendo esperado que,   quanto mais elevado, melhor a pontuação em tal teste neuropsicológico (Yao,   Zeng, &amp; Sun, 2009). Nesta linha de ação e pensamento, alguns autores   sublinham que é um fator a ter em conta pelas múltiplas experiências que   proporciona, afirmando ainda que determinadas experiências de vida (i.e.   educação, ocupação e lazer) se encontram associadas com um menor declínio cognitivo   (taxas mais lentas de declínio da memória), e a uma redução do risco de   demência (Deary et al., 2009; Stern, 2009, 2012). Existem evidências robustas   de que uma educação de alto nível no início de vida está relacionada com uma   redução significativa na prevalência e incidência de demência (Meng &amp;   D’Arcy, 2012), sendo de salientar que a conclusão do 1º ciclo aumenta significativamente os valores de cognição (Huang &amp; Zhou, 2013).</font></p>     <p><font size="2" face="verdana">Paúl e colaboradores (2010) afirmaram que com o avançar da idade é quase   universal um certo grau de comprometimento cognitivo, sendo este notório,   maioritariamente, em idosos-velhos. A capacidade intelectual, designada também   por inteligência geral ou fator <i>g</i>, apresenta menores desempenhos em   idade avançada (Van der Elst et al., 2013), vista esta, como o mais importante fator de risco (Robertson, Savva, &amp; Kenny, 2013). </font></p>     <p><font size="2" face="verdana">Constatámos uma interação idade x escolaridade, retratando que os   indivíduos mais velhos e com nível de escolaridade superior, apresentaram   resultados mais elevados nas habilidades cognitivas, avaliadas pelos MMSE e   MPCR, respetivamente. Tal ponto, poderá ser justificado devido ao impacto do   papel da educação de acordo com a hipótese de reserva cognitiva. Baseado nesta   conceção, uma variedade de autores tem suportado que a educação mostra efeitos   protetores sobre a reserva cognitiva (Huang &amp; Zhou, 2013; Meng &amp;   D’Arcy, 2012), sendo esta eficaz na vitalidade cognitiva na idade adulta tardia   (Kramer et al., 2004). É assim expectável, que níveis educacionais mais   elevados se reflitam numa maior eficiência e capacidade neural (Scarmeas,   Albert, Manly, &amp; Stern, 2006; Tucker &amp; Stern, 2011), sendo de salientar   que indivíduos cognitivamente ativos, desenvolvem formas mais eficientes   relativamente à execução de tarefas mentais (Fjell, McEvoy, Holland, Dale, &amp; Walhovd, 2014). </font></p>     <p><font size="2" face="verdana">Pesquisas recentes comprovam que a prática de exercício físico está   claramente associada à preservação da cognição durante o processo de   envelhecimento, assim como a um decréscimo dos sintomas depressivos (Miller,   Taler, Davidson, &amp; Messier, 2012; Teixeira et al., 2013). Os nossos   resultados parecem entrar em discordância com a literatura existente, podendo   ressalvar-se ainda que apesar da maioria destes idosos (51.3%) apresentarem uma   prática bissemanal (120 min/semana), estes resultados não cumprem os requisitos   das <i>guidelines</i> do <i>American College of Sports Medicine</i> e da <i>American     Heart Association</i>, preconizando tais diretrizes um mínimo de 30 minutos de   intensidade moderada de exercício aeróbio, 5 dias por semana (Nelson et al.,   2007; WHO, 2010). Estes não cumprem as recomendações, sendo considerados   segundo Ciucurel e Iconaru (2012), como a faixa etária com hábitos de vida mais sedentários comparativamente à restante população.</font></p>     <p><font size="2" face="verdana">Sugerimos, de acordo com a atual realidade, a realização de mais estudos   no âmbito desta temática, dada a inconsistência de alguns dos resultados   alcançados comparativamente às investigações existentes. É ainda de salientar   que não podemos generalizar os nossos resultados dada a limitação da amostra, enfatizando ainda o reduzido número de sujeitos do género masculino.</font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="verdana">Perante os atuais resultados, consideramos de suma importância a   urgência de novas pesquisas, devendo as mesmas concentrar esforços que sirvam   de base ao desenvolvimento de futuros programas de intervenção que visem prevenir ou retardar o declínio cognitivo e os níveis de depressão.</font></p>     <p>&nbsp;</p>     <p><font size="3" face="verdana"><b>CONCLUSÕES</b></font></p>     <p><font size="2" face="verdana">Como síntese geral, estes resultados permitem-nos concluir que, a   escolaridade é a variável mais determinante da função cognitiva, em idosos mais   velhos, enquanto o tempo de institucionalização foi a única variável que   diferenciou os níveis de depressão. Face ao aumento da esperança média de vida,   e de forma a prevenir a demência, será importante dotar os indivíduos de uma reserva   cognitiva, através de um nível de escolaridade superior.
