Fontes de dados: através da PubMed, realizou-se a pesquisa bibliográfica, utilizando como chave os termos “Unicompartmental Knee Arthroplasty” e limitando a pesquisa a artigos publicados entre 1 de janeiro de 2010 e 31 de dezembro de 2012, escritos em inglês ou português. Foram selecionados 94 artigos, sendo incluídos na presente monografia aqueles que se revelavam pertinentes. Síntese de Resultados: nos últimos 20 anos, tem-se assistido a um aumento do interesse na artroplastia unicompartimental do joelho, devido à evolução dos materiais, às técnicas cirúrgicas e aos critérios de seleção dos pacientes que levaram a uma melhoria da sobrevida das próteses e dos resultados obtidos, sendo possível atingir, hoje em dia, resultados sobreponíveis aos verificados com a artroplastia total do joelho. Conclusões: a artroplastia unicompartimental do joelho, quando na mão de um cirurgião experiente, pode ser uma opção viável no tratamento da osteoartrose medial do joelho, mesmo em pacientes jovens e ativos, revelando-se estas próteses duradoras e capazes de proporcionar bons resultados funcionais.]]></p></abstract>

Avan&ccedil;os nas t&eacute;cnicas cir&uacute;rgicas, tecnologia dos materiais e engenharia dos implantes continuam a aumentar as expectativas em rela&ccedil;&atilde;o ao desgaste das superf&iacute;cies de contacto, &agrave; fixa&ccedil;&atilde;o das pr&oacute;teses e &agrave; sua longevidade. Estes avan&ccedil;os t&ecirc;m permitido uma expans&atilde;o das indica&ccedil;&otilde;es cir&uacute;rgicas para pacientes mais jovens, mais ativos e mais pesados [1, 2].<br />A artroplastia unicompartimental do joelho (AUJ) surgiu na d&eacute;cada de 1970, mas estudos iniciais relatavam maus resultados, com taxas de revis&atilde;o, a m&eacute;dio prazo, na ordem dos 15% a 28%, devido &agrave; dificuldade t&eacute;cnica e sele&ccedil;&atilde;o errada de pacientes que levavam a liberta&ccedil;&atilde;o da pr&oacute;tese, desgaste do polietileno e progress&atilde;o da artrose nos outros compartimentos, o que deixou esta t&eacute;cnica em segundo plano, quando comparada com a artroplastia total do joelho (ATJ) e Osteotomia Tibial Alta (OTA)[3, 4].<br />Apesar da ATJ continuar a ser o gold standard do tratamento cir&uacute;rgico da osteoartrose (OA) do joelho[5], com taxas de sobrevida sobrevida a 10 anos entre 94% a 100%, a AUJ tem mostrado grandes melhorias, nos &uacute;ltimos anos, com resultados excelentes publicados em v&aacute;rios estudos[6-8]. Juntamente com estes bons resultados, a melhor rela&ccedil;&atilde;o custo-efic&aacute;cia verificada com AUJ[9] e as muitas vantagens que lhe s&atilde;o atribu&iacute;das em rela&ccedil;&atilde;o aos outros m&eacute;todos cir&uacute;rgicos, fazem com que esta pare&ccedil;a, cada vez mais, uma alternativa vi&aacute;vel no tratamento da OA do joelho.</p></font>    <p>&nbsp;</p>    ]]></body>
<body><![CDATA[<p><b><font face="Verdana" size="2">MATERIAL E MÉTODOS</font></b></p><font face="verdana" size="2">    <p>Acedendo &agrave; base de dados MEDLINE, atrav&eacute;s da PubMed, realizou-se a pesquisa bibliogr&aacute;fica, utilizando como chave os termos &ldquo;Unicompartmental Knee Arthroplasty&rdquo; e limitando a pesquisa a artigos publicados entre 1 de janeiro de 2010 e 31 de dezembro de 2012, escritos em ingl&ecirc;s ou portugu&ecirc;s. Foram encontrados 252 artigos, realizando-se a leitura integral ou do resumo dos mesmos e sendo selecionados os artigos que abordavam os temas: indica&ccedil;&otilde;es cir&uacute;rgicas, sobrevida, resultados funcionais, vantagens e desvantagens, rela&ccedil;&atilde;o custo-efic&aacute;cia, complica&ccedil;&otilde;es e revis&atilde;o em casos de fal&ecirc;ncia. Esta triagem resultou na sele&ccedil;&atilde;o de 94 artigos, sendo inclu&iacute;dos na presente monografia aqueles que se revelavam pertinentes.</p></font>    <p>&nbsp;</p>    <p><b><font face="Verdana" size="2">VANTAGENS</font></b></p><font face="verdana" size="2">    <p>Est&atilde;o hoje descritas na literatura v&aacute;rias vantagens da AUJ sobre a ATJ, sendo, talvez, a mais preponderante, a possibilidade de atua&ccedil;&atilde;o apenas no compartimento degenerado, poupando o restante joelho[10]. Esta abordagem mais conservadora, permite a manuten&ccedil;&atilde;o das estruturas saud&aacute;veis, incluindo o ligamento cruzado anterior (LCA), e permite uma cin&eacute;tica muito semelhante &agrave; de um joelho normal. Est&aacute; descrita uma melhoria funcional, verificada tanto em termos objetivos, baseada em provas funcionais e na amplitude de movimento, como em question&aacute;rios colocados aos pacientes, que descrevem uma melhoria significativa em rela&ccedil;&atilde;o &agrave; fun&ccedil;&atilde;o pr&eacute;-operat&oacute;ria[11, 12]. Num estudo em que se avaliaram pacientes com uma pr&oacute;tese unicompartimental, enquanto subiam escadas, verificou-se que foi preservada a simetria entre os membros inferiores durante a marcha[13].<br />Apesar dos bons resultados que se verificam atualmente, continuam a haver casos em que &eacute; necess&aacute;ria uma revis&atilde;o da pr&oacute;tese, muitas vezes para ATJ. V&aacute;rios estudos relatam este procedimento realizado com sucesso, contrariando a ideia de que esta &eacute; uma cirurgia complicada e que necessita muitas vezes de enxertos &oacute;sseos e com dificuldades de implanta&ccedil;&atilde;o da nova pr&oacute;tese[14, 15]. Esta facilidade em rever uma AUJ falhada deve-se, em parte, &agrave; natureza poupadora de osso desta cirurgia. No entanto, h&aacute; ainda controv&eacute;rsia sobre os resultados de uma AUJ revista para ATJ, estando publicados estudos que relatam resultados inferiores aos de uma ATJ prim&aacute;ria[16, 17].<br />A AUJ &eacute; uma cirurgia menos invasiva do que a ATJ e como tal tem menos complica&ccedil;&otilde;es e taxa de mortalidade associadas[18]. Atualmente, h&aacute; a possibilidade de utilizar t&eacute;cnicas de cirurgia minimamente invasiva (CMI), podendo ser obtidos resultados ainda mais favor&aacute;veis. As perdas hem&aacute;ticas s&atilde;o menores e h&aacute; risco reduzido de eventos tromboemb&oacute;licos[19]. &Eacute; poss&iacute;vel, tamb&eacute;m, um melhor controlo da dor peri-operat&oacute;ria[20]. Isto leva a uma redu&ccedil;&atilde;o significativa no tempo de hospitaliza&ccedil;&atilde;o e a um menor risco de transfer&ecirc;ncia para unidades de cuidados continuados[21]. Estudos comparativos entre AUJ e ATJ revelam que os pacientes apresentam maiores &iacute;ndices de satisfa&ccedil;&atilde;o e bem estar mental no seguimento de uma AUJ, em grande parte devido &agrave; maior semelhan&ccedil;a com o joelho normal, o chamado natural feeling, proporcionado por estas pr&oacute;teses[22]. Devido &agrave; pequena incis&atilde;o realizada, com menor les&atilde;o muscular, &eacute; poss&iacute;vel obter resultados funcionais equipar&aacute;veis aos pr&eacute;-operat&oacute;rios no espa&ccedil;o de um m&ecirc;s, per&iacute;odo muito inferior ao verificado com ATJ[23, 24], podendo ser esperados os resultados finais entre os 6 e os 12 meses[25]. A AUJ permite, ainda, que uma grande maioria dos pacientes mais jovens regressem &agrave;s suas atividades profissionais, em m&eacute;dia, 3 meses ap&oacute;s a cirurgia[1] e que cerca de 90% dos pacientes relatem manuten&ccedil;&atilde;o ou melhoria da sua capacidade de participar em atividades f&iacute;sicas recreativas ao fim de 18 meses[2].<br />Est&aacute; ainda descrito que esta &eacute; uma cirurgia mais barata do que a ATJ[9].</p></font>    <p>&nbsp;</p>    <p><b><font face="Verdana" size="2">INDICAÇÕES E CONTRAINDICAÇÕES</font></b></p><font face="verdana" size="2">    <p>Numa revis&atilde;o da literatura feita em 2010, comparando a AUJ com OTA, Dettoni identificou como indica&ccedil;&otilde;es ideais para AUJ medial: A) a presen&ccedil;a de OA unicompartimental medial ou necrose avascular do c&ocirc;ndilo femural, com compartimento lateral e patelo-femural intactos; B) idade superior a 60 anos; C) baixa atividade f&iacute;sica; D) aus&ecirc;ncia de obesidade; E) dor m&iacute;nima em repouso; F) arco de movimento superior a 90&ordm; com menos de 5&ordm; de contratura em flex&atilde;o; G) deformidade com menos de 10&ordm;, que possa ser passivamente corrigida[26].<br />Na presente revis&atilde;o bibliogr&aacute;fica, a maioria dos autores que relatam bons resultados utilizaram indica&ccedil;&otilde;es sobrepon&iacute;veis ou muito semelhantes &agrave;s supramencionadas, apresentando como principais diferen&ccedil;as os crit&eacute;rios utilizados em rela&ccedil;&atilde;o &agrave; idade e peso. <br />Existe atualmente uma grande controv&eacute;rsia em rela&ccedil;&atilde;o &agrave; idade ideal para a realiza&ccedil;&atilde;o deste procedimento. Muitos cirurgi&otilde;es n&atilde;o imp&otilde;em um limite m&iacute;nimo, obtendo bons resultados. Num estudo realizado em pacientes com idade inferior a 60 anos, foi obtida uma sobrevida de 93,5% aos 10 anos[8], e noutro com as mesmas condi&ccedil;&otilde;es a sobrevida foi de 94% aos 12 anos[11]. Os autores referem ainda que em 90% dos casos &eacute; obtida muito boa satisfa&ccedil;&atilde;o por parte dos doentes. No entanto, a idade &eacute; considerada por alguns um preditor de satisfa&ccedil;&atilde;o, tendo os pacientes com idade inferior a 60 anos um maior risco de insatisfa&ccedil;&atilde;o em rela&ccedil;&atilde;o &agrave; sua pr&oacute;tese[27, 28]. Os autores sugerem como explica&ccedil;&atilde;o o facto dos doentes mais novos esperarem maior melhoria funcional do que aquela que a pr&oacute;tese pode oferecer, o que pode levar a um aumento da taxa de revis&atilde;o para este grupo de pacientes.<br />O excesso de peso &eacute; globalmente aceite como um fator de risco de fal&ecirc;ncia precoce na AUJ. No entanto n&atilde;o est&aacute; estabelecido um valor limite a partir do qual se deve optar por outro procedimento. Alguns autores consideram obesidade m&oacute;rbida uma contraindica&ccedil;&atilde;o[8], outros n&atilde;o imp&otilde;em limite e obt&ecirc;m bons resultados[27]. Num inqu&eacute;rito realizado no Reino Unido, apenas 8% dos cirurgi&otilde;es participantes consideraram a obesidade uma contraindica&ccedil;&atilde;o[29]. Este alargamento dos crit&eacute;rios pode ser explicado pelo aparecimento de estudos a relatar bons resultados em pacientes obesos. Kuipers ficou surpreendido ao verificar que no seu estudo os pacientes obesos n&atilde;o apresentavam um risco aumentado de fal&ecirc;ncia[28], especulando que isto se possa dever ao maior sedentarismo muitas vezes apresentado por pacientes obesos, que poupa a sua pr&oacute;tese aos efeitos delet&eacute;rios da sobrecarga ponderal. Outro autor refere n&atilde;o ter encontrado uma diferen&ccedil;a estatisticamente significativa entre o &iacute;ndice de massa corporal (IMC) e o resultado funcional obtido[30]. No entanto refere que de um modo geral, os melhores resultados foram encontrados em pacientes com menor IMC. Em contrapartida, num estudo desenhado especificamente para avaliar o resultado da AUJ estratificado por IMC, os autores conclu&iacute;ram que os pacientes com IMC&gt;35 Kg/m2 est&atilde;o em maior risco de fal&ecirc;ncia, pelo que a AUJ nestes pacientes deve ser abordada com precau&ccedil;&atilde;o[31].<br />A artrose deve estar numa fase avan&ccedil;ada, com apagamento da interlinha articular, o que corresponde a um grau 2 ou superior na escala de Ahlback[11]. Num estudo publicado em 2011, Niinimaki sugere que a AUJ s&oacute; deve ser realizada em casos em que a espessura da cartilagem do compartimento medial seja inferior a 40% da cartilagem do compartimento lateral[32]. O autor explica que esta cirurgia n&atilde;o deve ser realizada se houver evid&ecirc;ncia de desgaste incompleto da cartilagem, uma vez que em fases iniciais da artrose os sintomas s&atilde;o principalmente causados por inflama&ccedil;&atilde;o sinovial, justificando-se, por exemplo, infiltra&ccedil;&atilde;o com corticosteroides. Em casos mais avan&ccedil;ados, com perda total da cartilagem, &eacute; mais comum que os sintomas sejam de origem mec&acirc;nica, com dor proveniente do osso, fazendo com que o doente beneficie mais com uma AUJ.<br />Les&atilde;o do ligamento cruzado anterior &eacute; tida como uma contraindica&ccedil;&atilde;o para AUJ[26], por causar maiores for&ccedil;as sobre os componentes, levando a fal&ecirc;ncia precoce do implante. Ainda assim, existem estudos em que foram realizadas AUJ em joelhos com defici&ecirc;ncia do LCA tendo sido obtidos bons resultados. Um grupo de autores utilizando a pr&oacute;tese Oxford fase-3, obtiveram bons resultados em pacientes com LCA insuficiente, conseguindo uma sobrevida de 94% aos 5 anos [33]. Apesar dos bons resultados os autores recomendam que se continue a ter a presen&ccedil;a LCA intacto como crit&eacute;rio para a realiza&ccedil;&atilde;o de AUJ, apenas ignorando a les&atilde;o do mesmo em casos muito espec&iacute;ficos, em que o doente est&aacute; ciente do problema e do risco associado. <br />Estudos recentes demonstram que &eacute; poss&iacute;vel realizar AUJ combinada com reconstru&ccedil;&atilde;o do LCA, em pacientes com instabilidade articular, obtendo-se resultados bastante promissores[34, 35]. Os autores referem que este &eacute; um procedimento complexo, e que deve ser realizado por cirurgi&otilde;es experientes, sob pena de serem obtidos maus resultados.<br />Doen&ccedil;as inflamat&oacute;rias, como a artrite reumatoide, s&atilde;o consideradas uma contraindica&ccedil;&atilde;o formal[36]. Oste&oacute;fitos e condrocalcinose ligeira podem estar presentes sem contraindicarem a AUJ. No caso de OA patelofemural de grau avan&ccedil;ado, com osso-em-osso, deve optar-se por outro procedimento[8].</p></font>    <p>&nbsp;</p>    <p><b><font face="Verdana" size="2">IMPLANTES</font></b></p><font face="verdana" size="2">    ]]></body>
<body><![CDATA[<p>Est&aacute; descrito que os modelos fixos com alta conformidade produzem for&ccedil;as de contacto reduzidas, mas h&aacute; maior tens&atilde;o no componente tibial, podendo levar a liberta&ccedil;&atilde;o do mesmo. Por sua vez, um modelo fixo com baixa conformidade causa menos tens&atilde;o no componente tibial, mas produz maior for&ccedil;as de contacto, levando a um aumento do desgaste do polietileno. Todos os modelos fixos apresentam um conflito de cin&eacute;tica, com rota&ccedil;&atilde;o limitada. Surgem assim os modelos m&oacute;veis, numa tentativa de conciliar congru&ecirc;ncia e mobilidade, teoricamente reduzindo a tens&atilde;o no componente tibial e as for&ccedil;as de contacto ao mesmo tempo que resolvem o conflito de cin&eacute;tica, permitindo a livre rota&ccedil;&atilde;o da articula&ccedil;&atilde;o[37].<br />N&atilde;o est&atilde;o ainda esclarecidas as diferen&ccedil;as entre os modelos de pr&oacute;teses unicompartimentais. Num estudo em que foram comparados dois modelos fixos e um m&oacute;vel n&atilde;o foram detetadas diferen&ccedil;as significativas no que toca a resultados funcionais, sobrevida e modos de fal&ecirc;ncia, apesar do tempo decorrido at&eacute; se verificar um dado modo de fal&ecirc;ncia se alterar para os diferentes modelos[3]. Os autores sugerem que a AUJ com pr&oacute;tese m&oacute;vel &eacute; uma cirurgia que permite menor margem de erro, o que pode explicar as fal&ecirc;ncias precoces por vezes descritas para estes modelos.<br />Estudos in-vitro realizados para determinar diferen&ccedil;as em termos de desgaste do polietileno, utilizando simuladores em que a pr&oacute;tese &eacute; fixada a um bloco met&aacute;lico, revelam uma maior tend&ecirc;ncia para ocorrer desgaste nos modelos m&oacute;veis[38, 39]. Esta diferen&ccedil;a pode dever-se ao facto de, nos modelos m&oacute;veis, haver uma maior liberdade da movimenta&ccedil;&atilde;o em qualquer dire&ccedil;&atilde;o e uma maior &aacute;rea de contacto, com a adi&ccedil;&atilde;o da face inferior do polietileno. Noutro estudo in-vitro foram usados dois m&eacute;todos de fixa&ccedil;&atilde;o, sendo obtidos os mesmos resultados acima referidos quando foi usado o mesmo m&eacute;todo. No entanto, quando se fixaram as pr&oacute;teses a um f&eacute;mur sint&eacute;tico, numa tentativa de melhor reproduzir as condi&ccedil;&otilde;es in-vivo, os resultados inverteram-se, demonstrando um desgaste mais elevado nos modelos fixos[37], mas sendo a diferen&ccedil;a encontrada menor em rela&ccedil;&atilde;o aos resultados anteriores. Foi realizada uma an&aacute;lise a n&iacute;vel molecular, que revelou uma maior degrada&ccedil;&atilde;o da estrutura do polietileno para os modelos m&oacute;veis, em ambos os m&eacute;todos de fixa&ccedil;&atilde;o. Esta degrada&ccedil;&atilde;o da estrutura pode ter resultados delet&eacute;rios a longo prazo.<br />Outro autor conclui que a utiliza&ccedil;&atilde;o de modelos fixos com componente tibial met&aacute;lico, em detrimento de componentes totalmente de polietileno, produz uma melhor distribui&ccedil;&atilde;o das for&ccedil;as na superf&iacute;cie proximal da t&iacute;bia, reduzindo assim os casos de colapso, descritos para este modelo[40].</p></font>    <p>&nbsp;</p>    <p><b><font face="Verdana" size="2">SOBREVIDA</font></b></p><font face="verdana" size="2">    <p>Um dos grandes debates em rela&ccedil;&atilde;o a AUJ &eacute; a sua sobrevida. Ser&aacute; que podem ser obtidos resultados t&atilde;o bons ou melhores do que os que se verificam com a ATJ? Surgem assim muitos estudos a relatar taxas de sobrevida, obtendo-se resultados mistos e variados modos de fal&ecirc;ncia para estas pr&oacute;teses. <br />Num estudo realizado com a pr&oacute;tese unicompartimental m&oacute;vel Oxford fase-3, Clement obteve uma sobrevida de 91,2% aos 9 anos[27]. O autor atribui estes bons resultados a uma escolha cuidadosa dos pacientes indicados e &agrave; boa t&eacute;cnica dos cirurgi&otilde;es. Foran descreve resultados igualmente satisfat&oacute;rios com a pr&oacute;tese unicompartimental fixa Miller-Galante, obtendo uma taxa de sobrevida aos 15 anos de 93% e de 90% aos 20 anos[36]. Um achado interessante neste estudo foi que todos os pacientes que realizaram controlo radiogr&aacute;fico durante o follow-up revelaram progress&atilde;o da artrose no compartimento lateral, mas apenas em 2 casos esta progress&atilde;o obrigou a uma revis&atilde;o. Os autores sublinham o grande esfor&ccedil;o demonstrado pelos cirurgi&otilde;es em evitar a sobrecorre&ccedil;&atilde;o e o sobredimensionamento dos componentes da pr&oacute;tese, facto que decerto contribuiu para os resultados obtidos.<br />Numa an&aacute;lise da base de dados Medicare, foram selecionadas 2848 AUJ realizadas entre 2001 e 2007, sendo obtida uma sobrevida de 94% aos 5 anos[6]. Os autores atribuem o risco aumentado de revis&atilde;o da AUJ &agrave; experi&ecirc;ncia do cirurgi&atilde;o, sendo detetada uma diferen&ccedil;a significativa aos 2 anos entre cirurgi&otilde;es menos experientes que realizam menos de 40 AUJ por ano, quando comparados com os que realizam mais do que este n&uacute;mero de AUJ por ano. Esta diferen&ccedil;a pode ser explicada pelas fal&ecirc;ncias catastr&oacute;ficas atribu&iacute;veis a erros cir&uacute;rgicos.<br />Dervin relata experi&ecirc;ncia inicial de 545 casos dum grupo de cirurgi&otilde;es com a Oxford fase-3, descrevendo uma taxa de revis&atilde;o de 5% aos 2 anos, atingindo depois um plateau com um n&uacute;mero muito reduzido de revis&otilde;es[41]. A causa mais comum de fal&ecirc;ncia encontrada foi a progress&atilde;o da artrose no compartimento lateral. Mais uma vez, os autores atribuem estas fal&ecirc;ncias precoces a erros na sele&ccedil;&atilde;o dos pacientes e a erros cir&uacute;rgicos, nomeadamente a les&atilde;o do ligamento colateral medial e a defeitos de corte, resultando em erros de alinhamento e casos de overstuffing do compartimento lateral. <br />O alinhamento tibiofemural est&aacute; descrito como tendo um grande impacto no resultado obtido na AUJ. &Eacute; aconselhado um alinhamento neutro (5&ordm; a 8&ordm; em valgo) ou ligeiramente subcorrigido, pois a sobrecorre&ccedil;&atilde;o acarreta um risco aumentado de progress&atilde;o da artrose no compartimento lateral, enquanto a subcorre&ccedil;&atilde;o excessiva pode levar a desgaste aumentado do polietileno. Kim atingiu os melhores resultados quando se obteve um alinhamento de 4&ordm; a 6&ordm; em valgo[42].<br />O declive posterior do prato tibial no compartimento medial desempenha tamb&eacute;m um papel importante no resultado da AUJ. Um aumento deste declive pode levar a instabilidade da articula&ccedil;&atilde;o, com les&atilde;o do LCA, que podem levar a fal&ecirc;ncia precoce da pr&oacute;tese[43]. Est&aacute; descrito que o declive tibial posterior deve variar entre os 3&ordm; e 7&ordm;[44].<br />Apesar dos bons resultados obtidos por alguns autores com a CMI, continua a haver o receio por parte de muitos cirurgi&otilde;es em usar esta t&eacute;cnica devido &agrave; visualiza&ccedil;&atilde;o limitada proporcionada pela incis&atilde;o reduzida, o que pode p&ocirc;r em risco a prepara&ccedil;&atilde;o das superf&iacute;cies articulares e do alinhamento dos implantes[45]. Numa tentativa de diminuir este risco de mal posicionamento, tanto na CMI como na cirurgia convencional, surgiram sistemas de navega&ccedil;&atilde;o intraoperat&oacute;rios que, teoricamente, levariam a um melhor posicionamento das pr&oacute;teses. Estudos recentes n&atilde;o t&ecirc;m conseguido demonstrar esta melhoria no posicionamento com as t&eacute;cnicas de navega&ccedil;&atilde;o[30, 46]. Estes sistemas t&ecirc;m ainda o inconveniente de ser caros e levarem a cirurgias mais longas. No entanto, os autores referem poder haver vantagens da utiliza&ccedil;&atilde;o dos mesmos por cirurgi&otilde;es menos experientes, que ainda n&atilde;o desenvolveram totalmente a sua t&eacute;cnica. <br />O modo mais comum de fal&ecirc;ncia da pr&oacute;tese relacionada com o cirurgi&atilde;o, &eacute; a progress&atilde;o da artrose no compartimento lateral, devendo-se esta, muitas vezes, &agrave; sobrecorre&ccedil;&atilde;o acima descrita[47], que resulta de uma tentativa, por parte dos cirurgi&otilde;es, de atingir a estabilidade do prato de polietileno m&oacute;vel, utilizando o maior tamanho poss&iacute;vel.<br />Outros modos de fal&ecirc;ncia comuns, atualmente, s&atilde;o o desgaste do polietileno[11], dor medial persistente[48], luxa&ccedil;&atilde;o do prato de polietileno e liberta&ccedil;&atilde;o de componentes da pr&oacute;tese[49].<br />Com vista a evitar estes mecanismos de fal&ecirc;ncia est&atilde;o a ser concebidos novos equipamentos, como pratos de polietileno mais anat&oacute;micos e novos m&eacute;todos de fixa&ccedil;&atilde;o das pr&oacute;teses. &Eacute; o caso da pr&oacute;tese Twin Peg Oxford, que tem dois pinos de fixa&ccedil;&atilde;o femural, ao contr&aacute;rio do anterior modelo que tinha apenas um pino, com vista a reduzir poss&iacute;veis liberta&ccedil;&otilde;es deste componente. White descreve os primeiros 100 casos com esta pr&oacute;tese, obtendo 100% de sobrevida aos 2 anos[50].<br />Apesar de estarem descritos todos estes modos de fal&ecirc;ncia e dos maus resultados obtidos em alguns estudos[28, 47], est&atilde;o tamb&eacute;m descritos resultados muito favor&aacute;veis a longo-prazo. Price descreve a sua experi&ecirc;ncia na segunda d&eacute;cada com a pr&oacute;tese Oxford fase-3, tendo obtido uma sobrevida de 91% aos 20 anos[14]. O autor sugere que se a pr&oacute;tese sobreviver at&eacute; aos 10 anos, &eacute; de esperar que chegue aos 20 anos. Sendo assim, e dado que uma grande parte dos doentes em que se realizam AUJ tem perto de 70 anos, o autor acredita que n&atilde;o h&aacute; necessidade de considerar a AUJ um procedimento pr&eacute;-ATJ.</p></font>    <p>&nbsp;</p>    <p><b><font face="Verdana" size="2">CONCLUSÃO</font></b></p><font face="verdana" size="2">    <p>Nos &uacute;ltimos anos tem sido observado um aumento da popularidade da AUJ na comunidade ortop&eacute;dica devido ao avan&ccedil;o das t&eacute;cnicas e tecnologias cir&uacute;rgicas, &agrave; engenharia dos implantes e a procedimentos minimamente invasivos.<br />Apesar de ser uma t&eacute;cnica com uma curva de aprendizagem longa, em que muitas vezes s&atilde;o observados resultados menos bons no in&iacute;cio da aprendizagem do cirurgi&atilde;o, quando nas m&atilde;os de um perito e seguindo as indica&ccedil;&otilde;es adequadas, a AUJ parece ser um procedimento eficaz no tratamento de osteoartrose do compartimento medial do joelho, apresentando um risco de revis&atilde;o da pr&oacute;tese compar&aacute;vel ao verificado com a ATJ. &Eacute; poss&iacute;vel obter excelentes resultados funcionais e sobrevidas dos implantes igualmente satisfat&oacute;rias. Esta sobrevida prolongada das pr&oacute;teses, que chegam &agrave; 3&ordf; d&eacute;cada, faz com que a AUJ possa ser vista como uma medida definitiva na popula&ccedil;&atilde;o mais envelhecida. Com a inclus&atilde;o de pacientes mais jovens surge a necessidade de revis&atilde;o de algumas destas pr&oacute;teses, procedimento que, devido &agrave;s pr&oacute;teses modernas, pode ser realizado com dificuldades t&eacute;cnicas m&iacute;nimas.
