Fontes dos dados: Foi realizada uma revisão sistemática de case-reports, case-series e estudo clínicos indexados à PubMed desde 1941 que descrevem os resultados do tratamento cirúrgico da rotura do Grande Peitoral. O outcome primário foi o resultado clínico segundo a classificação de Bak et al. Os outcomes secundários incluíram o timing da cirurgia, idade de rotura e tempo de follow-up. Comparam-se os resultados dos diversos métodos de reparação, dos diversos tipos de âncoras e do tratamento agudo versus crónico. Síntese dos dados: Vinte e sete estudos com 110 doentes e 9 métodos de reparação foram incluídos: suturas topo-a-topo, suturas transósseas, parafusos com anilhas, âncoras não especificadas, âncoras absorvíveis, âncoras de titânio, âncoras de sutura, âncora de titânio com sutura transóssea e botão cortical. A idade média foi de 29,6 anos e o tempo médio de follow-up foi de 28 meses. A maioria dos resultados (90,9 %) foi excelente ou bom não se verificando diferença estatisticamente significativa entre os diversos métodos de reparação. A análise do subgrupo das âncoras (11 casos) não demonstrou superioridade de qualquer tipo de âncoras. A reparação aguda (54 casos) esteve associada a melhores resultados que o tratamento crónico (56 casos). Conclusões: De acordo com esta revisão sistemática, o tratamento cirúrgico deverá ser realizado na fase aguda não existindo evidência suficiente para recomendar um determinado método de reinserção.]]></p></abstract>

Excellent or good results were observed in 90,9 % of the patients and no statistical difference between repair methods groups was detected. Anchors subgroup analysis (11 cases) revealed similar results for the different types of anchors. The acute repair (54 cases) had significantly better results than chronic repair (56 cases). <body><![CDATA[<p><b><font face="Verdana" size="2">INTRODUÇÃO</font></b></p><font face="verdana" size="2">    <p>A rotura completa do tend&atilde;o do grande peitoral (GP) foi descrita pela primeira vez por Patissier em 1822<sup><span class="grouped_elements">1-3</span></sup>.Resulta habitualmente de um traumatismo indireto com pelo menos 48% dos casos a ocorrerem com treino com pesos<sup>4</sup>. A atividade que mais frequentemente gera rotura &eacute; o supino<sup><span class="grouped_elements">5-7</span></sup>.</p>     <p>&Eacute; uma patologia rara com 365 casos reportados at&eacute; ao ano de 2010, 75 % dos quais nos &uacute;ltimos 20 anos<sup>4.</sup> Desde as publica&ccedil;&otilde;es iniciais que se discute o tipo de tratamento. Bons resultados foram apresentados quer com o tratamento conservador<sup>8</sup>, quer com o tratamento cir&uacute;rgico<sup>9-11</sup>. Nas &uacute;ltimas d&eacute;cadas o tratamento cir&uacute;rgico parece apresentar melhores resultados do que o tratamento conservador<sup>6,7,12,13</sup>.</p>     <p>Principalmente para os doentes que desejem recuperar a for&ccedil;a para o desporto ou para o trabalho<sup>9,11,14-16</sup>. A evid&ecirc;ncia &eacute; ainda maior quando se trata de atletas<sup>2,3,13,17,18</sup>.</p>     <p>O tratamento conservador &eacute; atualmente reservado para os idosos<sup>7,19</sup>.</p>     <p>Os resultados da cirurgia na fase aguda (inferior a 3 ou 6 semanas p&oacute;s-rotura, dependendo dos autores) s&atilde;o superiores aos resultados do tratamento cir&uacute;rgico na fase cr&oacute;nica<sup>6,20,21</sup>.</p>     <p>Existem diversas t&eacute;cnicas descritas para a reinser&ccedil;&atilde;o do tend&atilde;o do GP sendo as mais frequentes: a sutura topo-a-topo, a sutura trans&oacute;ssea, os parafusos com anilha, as &acirc;ncoras, o sistema de bot&atilde;o cortical, associadas entre si ou com a utiliza&ccedil;&atilde;o de auto/aloenxerto. At&eacute; ao momento, n&atilde;o existe evid&ecirc;ncia suficiente sobre a melhor t&eacute;cnica de reinser&ccedil;&atilde;o do GP. Os autores prop&otilde;em-se realizar uma revis&atilde;o sistem&aacute;tica da literatura que permita comparar os resultados cl&iacute;nicos dos diversos m&eacute;todos de reinser&ccedil;&atilde;o nas roturas tendinosas do grande peitoral.</p></font>    <p>&nbsp;</p>     <p><b><font face="Verdana" size="2">MATERIAL E MÉTODOS</font></b></p><font face="verdana" size="2">    <p>Foi realizada uma revis&atilde;o sistem&aacute;tica da literatura existente atrav&eacute;s de uma pesquisa na base de dados <i>PubMed</i> com as seguintes palavras-chave - <i>Pectoralis major; distal pectoralis major; rupture</i> - inclu&iacute;das no t&iacute;tulo ou resumo. Foram analisados 183 resumos tendo sido selecionados 133 artigos considerados relevantes. Destes, 20 foram exclu&iacute;dos por n&atilde;o serem escritos em l&iacute;ngua inglesa. Dos 113&nbsp;restantes, n&atilde;o foi poss&iacute;vel obter a vers&atilde;o integral de sete. No total foram revistos 106 artigos aos quais foram aplicados os seguintes crit&eacute;rios de inclus&atilde;o - doentes com rotura tendinosa do grande peitoral com/sem avuls&atilde;o &oacute;ssea, submetidos a tratamento cir&uacute;rgico com tempo de <i>follow-up</i> superior a 3 meses e com os seguintes dados individualmente publicados ou poss&iacute;veis de obter ap&oacute;s contacto com os autores: idade, <i>timing</i> do tratamento (superior ou inferior a 3 semanas), descri&ccedil;&atilde;o dos resultados suficiente para aplica&ccedil;&atilde;o da classifica&ccedil;&atilde;o Bak et al.<sup>6</sup> e tempo de <i>follow-up</i>.</p>     ]]></body>
<body><![CDATA[<p>Os resultados do tratamento foram classificados segundo a escala apresentada por Bak et al.<sup>6</sup>: <i>Excelente</i> - se o doente se apresentar sem dor, com arco de mobilidade completo, sem queixas est&eacute;ticas, com for&ccedil;a de adu&ccedil;&atilde;o sim&eacute;trica ou perda inferior a 10 % de for&ccedil;a isocin&eacute;tica, e retomar as atividades pr&eacute;vias sem restri&ccedil;&otilde;es. <i>Bom</i> - se o doente apresentar ligeira limita&ccedil;&atilde;o funcional do movimento ou da for&ccedil;a, sem queixas est&eacute;ticas, com for&ccedil;a de adu&ccedil;&atilde;o sim&eacute;trica ou menos de 10% de d&eacute;fice isocin&eacute;tico. <i>Aceit&aacute;vel</i> - se existir uma limita&ccedil;&atilde;o que afete o regresso &agrave; atividade desejada, isto &eacute;, com dor ou falta da for&ccedil;a na atividade, ou se o resultado est&eacute;tico for insatisfat&oacute;rio. <i>Mau</i> - nos casos de fal&ecirc;ncia do tratamento, isto &eacute;, no tratamento conservador se requerer cirurgia ap&oacute;s um m&iacute;nimode 16 semanas p&oacute;s-les&atilde;o, e nos casos cir&uacute;rgicos se ocorrerem complica&ccedil;&otilde;es significativas, se mantiver dor ou limita&ccedil;&atilde;o do arco de mobilidade, ou se existirem queixas est&eacute;ticas significativas atribu&iacute;veis a cicatriza&ccedil;&atilde;o ou repara&ccedil;&atilde;o inadequadas.</p>     <p>Os dados obtidos foram registados em programa inform&aacute;tico Microsoft Excel<sup>TM</sup> e analisados no programa IBM SPSS Statistics<sup>TM</sup> (vers&atilde;o 22). A correla&ccedil;&atilde;o entre idade, <i>timing</i> do tratamento cir&uacute;rgico, tipo de repara&ccedil;&atilde;o e resultados foi realizada atrav&eacute;s dos teste exato de Fisher, testes t e ANOVA.</p></font>    <p>&nbsp;</p>     <p><b><font face="Verdana" size="2">RESULTADOS</font></b></p><font face="verdana" size="2">    <p>Vinte e sete estudos* cumpriram os crit&eacute;rios de inclus&atilde;o representando um total de 110 doentes. A idade m&eacute;dia de repara&ccedil;&atilde;o foi de 29,6 anos, com 54 doentes (49,1%) a serem submetidos a cirurgia em fase aguda (inferior a 3 semanas p&oacute;s-les&atilde;o). Registaram-se 9 m&eacute;todos de repara&ccedil;&atilde;o (1-9) e 4 tipos de resultados (A-D): Excelente (A), Bom (B), Aceit&aacute;vel (C) e Mau (D) (<a href="/img/revistas/rpot/v25n1/25n1a02t1.jpg">Tabela 1</a>).</p>

<p>&nbsp;</p>     <p>Os nove m&eacute;todos de repara&ccedil;&atilde;o analisados foram: sutura topo-a-topo, sutura trans&oacute;ssea, parafusos com anilhas, &acirc;ncoras n&atilde;o especificadas (N/E), &acirc;ncoras absorv&iacute;veis, &acirc;ncoras de tit&acirc;nio, &acirc;ncoras de sutura, &acirc;ncoras de tit&acirc;nio com sutura trans&oacute;ssea e bot&atilde;o cortical. Destes, os mais frequentes foram a sutura trans&oacute;ssea (58,2 %), a sutura topo-a-topo (14,5 %) e as &acirc;ncoras N/E (12,7%) (<a href="/img/revistas/rpot/v25n1/25n1a02t2.jpg">Tabela 2</a>).</p>

<p>&nbsp;</p>     <p>No que respeita ao outcome prim&aacute;rio, n&atilde;o se verificou associa&ccedil;&atilde;o estatisticamente significativa entre os v&aacute;rios m&eacute;todos de repara&ccedil;&atilde;o e os resultados (teste exato de Fischer, p-value = 0.788) (<a href="/img/revistas/rpot/v25n1/25n1a02t3.jpg">Tabela 3</a>). Quando se agrupou as diferentes repara&ccedil;&otilde;es com &acirc;ncora (grupo - &Acirc;ncoras), tamb&eacute;m n&atilde;o se verificou nenhuma superioridade deste m&eacute;todo sobre os restantes (teste exato de Fischer, p-value=0.917) (<a href="/img/revistas/rpot/v25n1/25n1a02t4.jpg">Tabela 4</a>).</p>

<p>&nbsp;</p>     <p>Com os v&aacute;rios tipos de &acirc;ncoras especificados (absorv&iacute;veis, tit&acirc;nio e de sutura) obtiveram-se resultados excelentes (33%), bons (45,5%) e aceit&aacute;veis (18,2%) (<a href="/img/revistas/rpot/v25n1/25n1a02t5.jpg">Tabela 5</a>). Quando comparadas entre si, n&atilde;o se verificou superioridade um tipo de &acirc;ncoras sobre os outros (teste exato de Fisher, p-value=0,475).</p>

<p>&nbsp;</p>     <p>No tratamento agudo foram obtidos apenas resultados excelentes (34 casos) ou bons (20 casos). No tratamento cr&oacute;nico, para al&eacute;m de resultados excelentes e bons (46 casos), tamb&eacute;m foram reportados 8 casos aceit&aacute;veis e 2 maus (<a href="/img/revistas/rpot/v25n1/25n1a02t6.jpg">Tabela 6</a>). O tratamento agudo esteve associado a melhores resultados cl&iacute;nicos (teste exato de Fischer, p-value = 0.000163).</p>

<p>&nbsp;</p>     <p><b><font face="Verdana" size="2">DISCUSSÃO</font></b></p>     <p><b><font face="Verdana" size="2">Outcome primário</font></b></p><font face="verdana" size="2">    <p>Apesar do aumento da frequ&ecirc;ncia do tratamento cir&uacute;rgico nas roturas do GP, n&atilde;o existem revis&otilde;es sistem&aacute;ticas publicadas que permitam comparar os resultados dos diferentes m&eacute;todos de repara&ccedil;&atilde;o. Nesta revis&atilde;o sistem&aacute;tica, que avalia individualmente cada doente com resultados eleg&iacute;veis publicados nos &uacute;ltimos 51 anos, n&atilde;o foi encontrada diferen&ccedil;a estatisticamente significativa entre os resultados dos diferentes m&eacute;todos de repara&ccedil;&atilde;o. Ficou demonstrado que nenhum dos nove m&eacute;todos foi individualmente superior. Quando analisadas como um grupo &uacute;nico (i.e. - &acirc;ncoras N/E, &acirc;ncoras absorv&iacute;veis, &acirc;ncoras met&aacute;licas e &acirc;ncoras de sutura), as &acirc;ncoras n&atilde;o demostraram resultados cl&iacute;nicos superiores aos restantes m&eacute;todos de repara&ccedil;&atilde;o do GP.</p>     <p>Hart et al.<sup>41</sup> n&atilde;o encontraram diferen&ccedil;a estatisticamente significativa entre sutura trans&oacute;ssea e &acirc;ncoras em estudo biomec&acirc;nico. No entanto, neste estudo todos os cad&aacute;veres apresentavam idade igual ou superior a 80 anos e 83% eram mulheres, quando a maioria das roturas se d&aacute; em homens na 3&ordf; ou 4&ordf; d&eacute;cadas de vida. Mooers et al.<sup>40</sup> j&aacute; haviam demostrado previamente um resultado cl&iacute;nico semelhante entre a fixa&ccedil;&atilde;o com &acirc;ncoras e os outros m&eacute;todos cir&uacute;rgicos de repara&ccedil;&atilde;o do GP, no entanto estas conclus&otilde;es n&atilde;o foram suportadas por uma revis&atilde;o sistem&aacute;tica e individualizada. Thomas et al.<sup>42</sup> apresentaram resultados semelhantes para a t&eacute;cnica de sutura trans&oacute;ssea e o bot&atilde;o cortical. No entanto, este estudo biomec&acirc;nico foi realizado em &uacute;mero de porcino.</p>     ]]></body>
<body><![CDATA[<p>N&atilde;o foi observada diferen&ccedil;a na compara&ccedil;&atilde;o entre &acirc;ncoras. Este resultado poder&aacute; dever-se ao n&uacute;mero reduzido da amostra tratada com &acirc;ncoras (11 no total), ao facto de existir um n&uacute;mero substancial de doentes em que o tipo de &acirc;ncora n&atilde;o foi especificado (&acirc;ncoras N/E) ou ao facto do tipo de &acirc;ncora n&atilde;o ter influ&ecirc;ncia nos resultados.</p></font>    <p><b><font face="Verdana" size="2">Outcomes secundários</font></b></p><font face="verdana" size="2">    <p>Confirmou-se neste estudo, &agrave; semelhan&ccedil;a de outros, a superioridade do tratamento cir&uacute;rgico em fase aguda<sup>6,20,21</sup>.A justifica&ccedil;&atilde;o ainda n&atilde;o foi totalmente clarificada pelos estudos de maior n&uacute;mero de doentes, no entanto admite-se que quando a cirurgia &eacute; adiada mais do que 8 semanas ap&oacute;s a les&atilde;o, o risco de complica&ccedil;&otilde;es aumenta como resultado da necessidade de aumentar a exposi&ccedil;&atilde;o cir&uacute;rgica devido a fibrose e retra&ccedil;&atilde;o excessivas do m&uacute;sculo<sup>6,20,43</sup>.</p>     <p>N&atilde;o existiu correla&ccedil;&atilde;o entre a idade, tempo de <i>follow-up</i> e o <i>timing</i> do tratamento. Como tal, n&atilde;o &eacute; poss&iacute;vel apontar a idade ou o tempo de <i>follow-up</i> como causa dos melhores resultados do tratamento agudo.</p>     <p>N&atilde;o se encontrou nenhum m&eacute;todo de repara&ccedil;&atilde;o com melhores resultados na fase aguda. Do mesmo modo, n&atilde;o se encontrou nenhum m&eacute;todo de repara&ccedil;&atilde;o com melhores resultados s&oacute; para a fase cr&oacute;nica.</p>     <p>As principais limita&ccedil;&otilde;es encontradas neste estudo s&atilde;o: os artigos avaliados foram publicados exclusivamente na l&iacute;ngua inglesa e indexados &agrave; <i>PubMed</i>; n&atilde;o foi avaliada a influ&ecirc;ncia de esteroide anabolizantes no pr&eacute; e p&oacute;s-operat&oacute;rio; desconhecem-se o tipo de &acirc;ncoras inclu&iacute;das no grupo &acirc;ncoras N/E; desconhece-se a influ&ecirc;ncia dos 3 casos com avuls&atilde;o-&oacute;ssea; muitos doentes ficaram exclu&iacute;dos deste estudo por n&atilde;o terem sido publicados ou n&atilde;o terem sido facultados os seus resultados individualizados; n&atilde;o foi avaliado o m&eacute;todo de reabilita&ccedil;&atilde;o por Fisioterapia; n&atilde;o existe escala de avalia&ccedil;&atilde;o consensual entre os autores o que obrigou &agrave; adapta&ccedil;&atilde;o dos crit&eacute;rios de Bak et al.<sup>6</sup>; n&atilde;o foram comparadas complica&ccedil;&otilde;es e o tempo de <i>follow-up</i> m&iacute;nimo foi de apenas 3 meses.</p>     <p>Nesta revis&atilde;o sistem&aacute;tica das roturas tendinosas com/sem avuls&atilde;o &oacute;ssea do GP tratadas cirurgicamente, os resultados do tratamento agudo foram superiores ao tratamento cr&oacute;nico. A esmagadora maioria dos resultados s&atilde;o bons ou excelentes n&atilde;o tendo sido encontradas diferen&ccedil;as entre a reinser&ccedil;&atilde;o com &acirc;ncoras e a sutura topo-a-topo, sutura trans&oacute;ssea, parafusos ou bot&atilde;o cortical. Do mesmo modo, n&atilde;o foi encontrada superioridade atribu&iacute;vel a um tipo espec&iacute;fico de &acirc;ncora.
