São vários os fatores que condicionam o seu aparecimento. Como consequência destes fatores a prevalência da desnutrição varia entre 5-20% em idosos residentes na comunidade e superior a 60% em idosos institucionalizados. São vários os estudos que apontam para um aumento de risco de infeção em cirurgia ortopédica associada à desnutrição. Neste trabalho os autores efetuaram uma revisão da literatura sobre o tema entre 1980 e 2016. Na literatura científica têm sido utilizados vários métodos para identificar a desnutrição: valores serológicos laboratoriais, medidas antropométricas ou a utilização de sistemas de score. Entre estes o “Rayney-MacDonald nutritional índex“ é um dos mais utilizados e usa os valores séricos da albumina e da transferrina para efetuar o calculo que permite identificar o estado nutricional do doente. É importante que os serviços de ortopedia desenvolvam uma metodologia de avaliação do estado nutricional dos doentes, em especial os mais idosos como é o caso dos doentes com fratura da extremidade proximal do fémur de forma a corrigir deficiências e prevenir complicações.]]></p></abstract>

As a result of these factors the prevalence of malnutrition varies between 5-20% in elderly community residents and more than 60% in institutionalized elderly. In this work the authors carried out a review of the literature on the subject between 1980 and 2016. Several studies point to an increased risk of infection in orthopedic surgery associated with malnutrition. In scientific literature, several methods have been used to identify malnutrition: serological laboratory values, anthropometric measurements or the use of score system. Among these the “Rayney-MacDonald nutritional index” is one of the most used and uses the serum levels of albumin and transferrin to make the calculation for identifying the patient nutritional status. It is important for orthopedic services to develop a methodology for assessing the nutritional status of patients, especially the elderly as in the case of patients with hip fractures in order to correct deficiencies and prevent complications.]]></p></abstract>

Neste artigo abordam-se os fatores respons&aacute;veis pela desnutri&ccedil;&atilde;o no idoso, complica&ccedil;&otilde;es e m&eacute;todos de diagn&oacute;stico.</p></font>    <p>&nbsp;</p>    ]]></body>
<body><![CDATA[<p><b><font face="Verdana" size="2">MATERIAL E MÉTODOS</font></b></p><font face="verdana" size="2">    <p>Os artigos cient&iacute;ficos que serviram de base para a elabora&ccedil;&atilde;o deste artigo foram selecionados a partir da pesquisa de artigos publicados em l&iacute;ngua inglesa, na&nbsp; PubMed Medline contendo as palavras de pesquisa&nbsp; &ldquo;malnutrition&rdquo;, &ldquo;orthopaedic sugery&rdquo;, &ldquo;aging&rdquo; em diferentes combina&ccedil;&otilde;es entre 1980 e 2016.</p></font>    <p>&nbsp;</p>    <p><b><font face="Verdana" size="2">RESULTADOS</font></b></p><font face="verdana" size="2">    <p>O envelhecimento &eacute; com frequ&ecirc;ncia acompanhado de diminui&ccedil;&atilde;o do metabolismo basal<sup>1,2</sup>. Tal circunst&acirc;ncia resulta da redu&ccedil;&atilde;o da massa magra, em especial das fibras musculares metabolicamente ativas<sup>3</sup>.</p>
    <p>Com o envelhecimento existe uma redu&ccedil;&atilde;o da ingest&atilde;o alimentar. Os fatores apontados para esta diminui&ccedil;&atilde;o s&atilde;o: perda de apetite; redu&ccedil;&atilde;o do olfato e paladar; patologia oral; saciedade precoce<sup>4,5,6</sup>. S&atilde;o tamb&eacute;m reconhecidos como elementos importantes, os fatores psicossociais, econ&oacute;micos e associados &agrave; toma de medicamentos<sup>7</sup>. Os idosos tamb&eacute;m alteram os seus h&aacute;bitos alimentares com predile&ccedil;&atilde;o por alimentos menos energ&eacute;ticos como os vegetais e frutas em lugar dos altamente energ&eacute;ticos como os doces e os nutrientes ricos em prote&iacute;nas<sup>8</sup>. Como consequ&ecirc;ncia destes fatores a preval&ecirc;ncia da desnutri&ccedil;&atilde;o varia entre 5-20% em idosos residentes na comunidade e superior a 60% em idosos institucionalizados<sup>9</sup>.</p>
    <p>O termo desnutri&ccedil;&atilde;o tem sido associado a uma nutri&ccedil;&atilde;o insuficiente na grande maioria dos estudos ortop&eacute;dicos. Contudo, a defini&ccedil;&atilde;o utilizada para este termo pela Organiza&ccedil;&atilde;o Mundial de Sa&uacute;de (OMS) &eacute; a de excesso ou insufici&ecirc;ncia nutritiva<sup>10</sup>. Na realidade v&aacute;rios estudos demonstraram que mesmo no obeso existem insufici&ecirc;ncias nutricionais, pelo que o termo desnutri&ccedil;&atilde;o parece bem adaptado para os doentes com peso insuficiente ou excessivo<sup>11</sup>. A desnutri&ccedil;&atilde;o tem sido associada a um aumento da morbilidade na cirurgia ortop&eacute;dica, com um aumento do n&uacute;mero de infe&ccedil;&otilde;es<sup>12,13,14</sup>. Esta associa&ccedil;&atilde;o entre desnutri&ccedil;&atilde;o e infe&ccedil;&atilde;o p&oacute;s cir&uacute;rgica pode ser explicado pela menor capacidade de cicatriza&ccedil;&atilde;o da ferida operat&oacute;ria e ainda pelo processo inflamat&oacute;rio prolongado, com repercuss&atilde;o sobre v&aacute;rios mecanismos<sup>15,16,17</sup>. Entre eles a redu&ccedil;&atilde;o da capacidade de prolifera&ccedil;&atilde;o do fibroblasto e a s&iacute;ntese do colag&eacute;nio<sup>18</sup>. A obesidade &eacute; tamb&eacute;m fator de risco para infe&ccedil;&atilde;o por estar associada a maior possibilidade de ocorrer necrose dos adip&oacute;citos, com a forma&ccedil;&atilde;o de espa&ccedil;o morto, o que naturalmente condiciona a velocidade de cicatriza&ccedil;&atilde;o da ferida operat&oacute;ria<sup>18</sup>. A obesidade aumenta na maioria dos casos a dificuldade da t&eacute;cnica cir&uacute;rgica, com o aumento do tempo operat&oacute;rio, o que &eacute; s&oacute; por si um risco de infe&ccedil;&atilde;o<sup>19</sup>. Isto &eacute; particularmente importante no paciente idoso<sup>20</sup>. Na literatura cient&iacute;fica t&ecirc;m sido utilizados v&aacute;rios m&eacute;todos para identificar a desnutri&ccedil;&atilde;o: valores serol&oacute;gicos laboratoriais<sup>21,22</sup>; medidas antropom&eacute;tricas<sup>16</sup>; ou a utiliza&ccedil;&atilde;o de sistemas de pontua&ccedil;&atilde;o<sup>23</sup>. Os valores serol&oacute;gicos mais utilizados para identificar desnutri&ccedil;&atilde;o s&atilde;o o valor de linf&oacute;citos &lt;1500 c&eacute;lulas mm3 e uma albumina s&eacute;rica &lt; 3,5gr/dL<sup>24</sup>. Valores de transferrina s&eacute;rica &lt; 200mg/dl s&atilde;o tamb&eacute;m considerados como indicador de desnutri&ccedil;&atilde;o<sup>15</sup>. Embora a pr&eacute;-albumina tenha sido utilizada em alguns estudos como indicador de desnutri&ccedil;&atilde;o, n&atilde;o existe uma defini&ccedil;&atilde;o do valor abaixo do qual se deve considerar a sua exist&ecirc;ncia<sup>25</sup>. Os valores considerados normais variam entre 16-35 mg/dL. Embora seja ainda motivo de controv&eacute;rsia, alguns trabalhos sugerem que doentes submetidos a artroplastia com n&iacute;veis s&eacute;ricos de zinco baixos (&lt; 95&mu;g/dL) apresentam com maior frequ&ecirc;ncia atraso da cicatriza&ccedil;&atilde;o da ferida cir&uacute;rgica<sup>26</sup>. Em outros estudos foram ainda utilizados como indicadores de desnutri&ccedil;&atilde;o a rela&ccedil;&atilde;o valores s&eacute;ricos albumina/globulina &lt;1,5 (valor normal 1,5-2,3) e a rela&ccedil;&atilde;o do n&ordm; de linf&oacute;citos / mon&oacute;citos inferior a 5<sup>27</sup>.</p>
    <p>A desnutri&ccedil;&atilde;o tamb&eacute;m pode ser avaliada recorrendo a m&eacute;todos antropom&eacute;tricos. Os mais utilizados s&atilde;o o per&iacute;metro da perna<sup>28</sup>, o per&iacute;metro do bra&ccedil;o<sup>29</sup> e prega cut&acirc;nea tricipital<sup>30</sup>. A desnutri&ccedil;&atilde;o em adultos foi definida pela presen&ccedil;a do per&iacute;metro do bra&ccedil;o &lt;22 cm, o per&iacute;metro da perna &lt;31 cm<sup>31</sup>. Em rela&ccedil;&atilde;o &agrave; prega cut&acirc;nea tricipital n&atilde;o est&aacute; definido o valor abaixo do qual se considera existir desnutri&ccedil;&atilde;o, contudo valores baixos s&atilde;o indicadores de desnutri&ccedil;&atilde;o grave<sup>32</sup>.</p>
    <p>As ferramentas de pontua&ccedil;&atilde;o padronizadas tamb&eacute;m podem ser utilizadas para avaliar a desnutri&ccedil;&atilde;o. O &ldquo;Rayney-MacDonald nutritional &iacute;ndex &ldquo; (RMNI), o &ldquo;Mini Nutritional Assessment&rdquo; (MNA) e o &ldquo;Schwarzopf nutritional &iacute;ndex&ldquo; s&atilde;o os mais conhecidos. O RMNI tem sido usado em muitos estudos, utiliza os valores s&eacute;ricos da albumina e da transferrina para efetuar o calculo (<a name="topq1"></a><a href="#q1">Equa&ccedil;&atilde;o 1</a>)<sup>16,33</sup>.</p>    <p>&nbsp;</p><a name="q1"></a>     ]]></body>

    <p>Um valor de zero ou negativo &eacute; indicador de desnutri&ccedil;&atilde;o.</p>
    <p>O MNA &eacute; um question&aacute;rio que abrange diferentes t&oacute;picos, entre os quais par&acirc;metros antropom&eacute;tricos e h&aacute;bitos alimentares entre outros<sup>34</sup>. Tem um question&aacute;rio curto que permite fazer o rastreio do estado nutricional ou um question&aacute;rio longo que permite efetuar a avalia&ccedil;&atilde;o do estado nutricional.</p>
    <p>O &ldquo;Schwarzkopf nutritional Index&ldquo; foi desenvolvido no &ldquo;New York University Hospital for Joint Diseases&rdquo; e tem uma aplica&ccedil;&atilde;o especifica para avalia&ccedil;&atilde;o do estado nutricional de doentes que v&atilde;o ser submetidos a cirurgia artropl&aacute;stica<sup>35</sup>.</p></font>    <p>&nbsp;</p>    <p><b><font face="Verdana" size="2">CONCLUSÃO</font></b></p><font face="verdana" size="2">    <p>O estado nutricional pode influenciar os resultados das interven&ccedil;&otilde;es cir&uacute;rgicas ortop&eacute;dicas, em especial em doentes mais idosos como &eacute; o caso dos doentes com fraturas da extremidade proximal do f&eacute;mur. Torna-se indispens&aacute;vel que os servi&ccedil;os de ortopedia tenham um protocolo de avalia&ccedil;&atilde;o do estado nutricional dos doentes de forma a poder corrigir defici&ecirc;ncias nutricionais e prevenir complica&ccedil;&otilde;es.<br /><br /></p></font>    <p>&nbsp;</p>    ]]></body>
