The present report intends to show a successful root canal treatment of a maxillary second molar with six root canals, of which four were present in the mesiobuccal root. After being diagnosed with pulp necrosis and symptomatic apical periodontitis, a molar was endodontically treated. It also had an uncommon paracone cusp. A careful inspection of the pulp chamber floor revealed a mesiopalatal sulcus, which, after exploration with ultrasonic tips under an operating microscope magnification, showed three additional mesiobuccal root canals. After proper mechanical debridement, disinfection with sodium hypochlorite, and warm gutta-percha obturation, a favorable outcome was observed at the 12-month follow-up.]]></p></abstract>
<abstract abstract-type="short" xml:lang="pt"><p><![CDATA[Resumo De entre as raízes dos molares maxilares, as mesiovestibulares são as que mais frequentemente apresentam um sistema canalar complexo e variável, podendo conduzir a uma maior dificuldade na obtenção de uma adequada preparação químico-mecânica. O presente trabalho pretende reportar o tratamento endodôntico bem-sucedido de um segundo molar maxilar com seis canais radiculares, em que quatro deles se encontravam na raiz mesiovestibular. Depois de ter sido diagnosticado com uma necrose com periodontite apical sintomática, o molar, que também apresentava uma incomum cúspide paracone, foi tratado endodonticamente. Uma cuidadosa análise da câmara pulpar revelou um sulco mesiopalatino, que, depois de devidamente explorado com pontas de ultrassons sob a magnificação de um microscópio operatório, revelou três canais mesiovestibulares adicionais. Depois de um apropriado desbridamento mecânico, desinfeção com hipoclorito de sódio e obturação com gutta-percha aquecida, foi possível perceber um resultado favorável no controlo dos 12 meses.]]></p></abstract>
