Although local estrogens are considered the first line treatment for severe VVA in postmenopausal women, a significant proportion of patients do not respond, do not adhere or have contraindications to hormonal treatments. In these situations, fractional CO2 laser may represent a good alternative. Aims: To analyze the efficacy and safety of fractional CO2 laser treatment for VVA. Study design: Retrospective observational study Population: 27 postmenopausal women treated for VVA with 2 or 3 sessions of fractional microablative CO2 laser system. Methods: We inquired patients about VVA symptoms (vaginal dryness, dyspareunia, vaginal burning, postcoital bleeding and urinary symptoms) before and after the treatment and a scale of 0-4 was used. Overall satisfaction was classified with a 5-point Likert scale. All patients had a previous colposcopic evaluation, which revealed VVA and a repeat colposcopy was performed after the treatment. Data were analyzed with SPSS® V24. Results: Mean age was 57.7 years ± 4.5 years old and 11% (3 women) had a background of breast cancer. We obtained a significant improvement in all VVA symptoms except for urinary symptoms. Mean overall satisfaction was 4.0 ± 0.92, with only one patient very dissatisfied with the treatment due to a persistent vulvodynia. Follow up colposcopy showed absence of vaginal atrophy signs in all women. No complications were reported and the treatment was painless in all cases, with no need of any kind of anesthesia. Conclusions: This study suggests that this new treatment modality is safe and effective in the treatment of VVA and may be considered for women with severe VVA who do not respond or have contraindications to hormonal therapy. However the duration of therapeutic effects is not clear and further research is needed.]]></p></abstract>

<body><![CDATA[<p><b>Introdução</b></p>     <p>O Síndrome genito-urinário (SGU) da menopausa foi um termo criado em Maio de 2013 por uma reunião de consenso entre a ISSWSH (Sociedade internacional para o estudo da saúde sexual da mulher) e NAMS (Sociedade de Menopausa Norte-Americana) que define um conjunto de sintomas e sinais associados a um défice de estrogénios envolvendo alterações nos orgãos genito-urinários<sup>1</sup>. É um conceito muito abrangente que engloba os sintomas resultantes do défice de estrogénios, bem como os resultantes do envelhecimento e outras situações que afectam a bexiga e o pavimento pélvico<sup>2</sup>. Assim vamos ter sintomas como a secura vaginal, a dispareunia, o prurido vulvovaginal, a leucorreia e, por outro lado, sintomas urinários como a frequência urinária, a urgência, a nictúria, a disúria e a incontinência (que ocorre em 15 a 35% dos casos em mulheres com mais de 60 anos). As infeções urinárias recorrentes ocorrem em 20% dos casos<sup>2</sup>.</p>     <p>O SGU afecta pelo menos 50 % de todas as mulheres na pós-menopausa. Contudo o receio e o pudor levam a que a maioria destas pacientes não manifeste espontaneamente estas queixas. Por isso o papel do médico é fundamental e este deve tomar a iniciativa de questionar sobre esta sintomatologia, uma vez que o tratamento é simples, seguro e pode transformar a qualidade de vida da mulher e do casal<sup>3</sup>.</p>     <p>Muito se tem escrito sobre o tratamento do SGU. A estrogenioterapia, quer tópica quer sistémica é eficaz para mulheres com atrofia vaginal grave a moderada e as duas vias podem ser utilizadas simultaneamente<sup>4</sup>. Contudo, existem contraindicações específicas para terapêutica hormonal como é o caso de mulheres com antecedentes de carcinoma da mama hormono-dependente. Apesar de estudos recentes demonstrarem segurança com estrogénios em doses mínimas, a maioria destas doentes sofre de atrofia vulvovaginal (AVV) grave que dificilmente responde a esta dosagem de estrogénios e, em muitos casos, são as próprias pacientes que recusam fazer um tratamento hormonal<sup>5,6</sup>.</p>     <p>O tratamento com Laser CO2 fracionado da atrofia vulvovaginal (AVV) é um tratamento inovador, mas com resultados promissores, segundo estudos muito recentes<sup>7-18</sup>. De acordo com a literatura, o protocolo de tratamento consiste em 3 sessões de Laser com intervalo mínimo de 30 dias, não requerendo qualquer tipo de anestesia. Os autores têm demonstrado de forma consistente que este tratamento induz melhorias ao nível de todos os sintomas da AVV como a secura vaginal, o desconforto vaginal e a dispareunia, refletindo-se numa melhoria da vida sexual e da qualidade de vida. Também o Laser Erbium:YAG tem sido referenciado na literatura como eficaz para o tratamento do SGU, em particular nas situações de incontinência urinária de esforço ligeira a moderada e até em prolapsos ligeiros, por induzir uma remodelação do colagénio do tecido da mucosa vaginal que persiste durante 6 meses após o tratamento<sup>19-23</sup>. Portanto, somos cada vez mais confrontados com o tratamento LASER do SGU, como alternativa às terapêuticas tradicionais, quer hormonais, quer não hormonais. </p>     <p>Na nossa unidade de colposcopia, utilizamos o Laser CO2 fracionado para tratamento da atrofia vulvovaginal grave em mulheres pós-menopausa, na ausência de resposta a outros tratamentos, ou quando existem contraindicações para terapêuticas hormonais. Assim o objectivo do nosso estudo foi avaliar a eficácia e segurança do Laser CO2 fracionado no tratamento da atrofia vulvovaginal. </p>     <p><b>Métodos</b></p>     <p>Estudo retrospectivo e observacional de 27 pacientes tratadas com 2 ou 3 sessões de Laser CO2 fracionado (Monalisa Touch, DEKA®) para AVV, durante o período de Novembro de 2015 até Outubro de 2016. Foram tratadas mulheres na pós-menopausa com sintomas graves de AVV resistentes a outros tratamentos ou com contraindicações para terapêutica hormonal. As especificações para o tratamento foram uma potência de 30-40 Watts com uma duração de 1000 microsegundos de duração de pulso e 1000 micras de intervalo, já descritas originalmente por Salvatore et al<sup>7</sup>. Utilizamos uma sonda vaginal para tratamento da vagina e uma sonda específica para tratamento da região vulvar (<a href="#f1">Figura 1</a>). O intervalo mínimo entre sessões é de 30 dias. Antes de cada sessão é recomendada a não colocação de cremes vaginais durante o período mínimo de 1 semana e após a sessão é recomendada abstinência sexual durante pelo menos 5 (cinco) dias.</p>     <p>&nbsp;</p>     <p align="center"><a name="f1"></a><img src="/img/revistas/aogp/v12n3/12n3a02f1.jpg"/></p>

<body><![CDATA[<p>&nbsp;</p>     <p>Foi realizado um inquérito às pacientes sobre os sintomas de AVV (secura, ardor, prurido, dispareunia, coitorragias e sintomas urinários) antes e após o tratamento, tendo sido utilizada uma escala de 0-4, em que o 0 significa ausência de sintoma e o 4 significa sintoma muito grave. A satisfação global com o tratamento foi classificada com uma escala de 1-5 (1 indica muito insatisfeita e 5 indica totalmente satisfeita com o tratamento). Todas as pacientes tiveram uma avaliação colposcópica prévia que objectivou a presença de AVV, tendo sido repetida após o tratamento. </p>     <p>Os dados foram analisados com o <i>software</i> SPSS ® V24, tendo sido utilizado o teste não paramétrico <i>Wilcoxon</i> para comparar os resultados antes e após o tratamento. Foi considerado um nível de significância de 5%. </p>     <p>Foi obtido o consentimento informado e o estudo foi aprovado pela comissão de ética da nossa instituição. </p>     <p><b>Resultados</b></p>     <p>A idade média da nossa amostra foi 57,7 ± 4,5 anos. A maioria das pacientes tiveram uma menopausa espontânea (n=24). Relativamente aos antecedentes de cancro da mama, 3 pacientes (11%) encontravam-se nesta situação, sendo que uma delas estava a fazer terapêutica com tamoxifeno e as outras com letrozol. </p>     <p>No que diz respeito aos sintomas de AVV, observou-se uma melhoria significativa após o tratamento com Laser CO2 fracionado, quer após 2 sessões (n=11) quer após 3 sessões (n=16). Os sintomas com maior significado estatístico foram a secura vaginal (antes <i>versus </i>após tratamento: 3,78 ± 0,42 <i>versus</i> 1,89 ± 0,89; <i>p value</i> &lt;0,001), o ardor vaginal (antes <i>versus</i> após tratamento: 2,70 ± 1,44 <i>versus</i> 0,48 ± 0,80; p value &lt;0,001) e a dispareunia (antes <i>versus</i> após tratamento: 3,37 ± 1,08 <i>versus</i> 1,11 ± 1,19; p value 0,004) (<a href="#q1">Quadro I</a>). Relativamente aos sintomas urinários verificou-se uma ligeira melhoria, contudo esta não foi estatisticamente significativa (antes <i>versus</i> após tratamento: 0,70 ± 0,39 <i>versus</i> 0,30 ± 0,78; p value 0,109). </p>     <p>&nbsp;</p>     <p align="center"><a name="q1"></a><img src="/img/revistas/aogp/v12n3/12n3a02q1.jpg"/></p>

<body><![CDATA[<p><b>Discussão</b></p>     <p>A experiência da nossa unidade tem revelado uma elevada eficácia e bom nível de satisfação com o tratamento Laser CO2 no tratamento da AVV em mulheres na pós-menopausa. </p>     <p>Este estudo tem várias limitações, nomeadamente o facto de ser um estudo retrospectivo, de não ter um grupo controlo, de a amostra ser relativamente pequena, e de o período de <i>follow up</i> ser inferior a 12 meses. Por outro lado, o protocolo de tratamento não foi igual em todas as mulheres uma vez que 11 pacientes foram submetidas a 2 sessões Laser e as restantes 16 pacientes foram tratadas com 3 sessões Laser como descrito em estudos prévios<sup>7-18</sup>. Contudo, na nossa revisão foi possível demonstrar a eficácia mesmo com 2 sessões Laser, com bom grau de satisfação da paciente. </p>     <p>Tendo em conta que na nossa população a indicação <i>major</i> para o tratamento foi a AVV, os sintomas urinários basais eram os mais relacionados com a atrofia em particular a disúria. Estudos mais recentes têm explorado o tratamento Laser CO2 na disfunção do pavimento pélvico<sup>16,24</sup>.<sup> </sup>Na nossa unidade, esta vertente terapêutica do Laser CO2 ainda não foi explorada, embora a evidência científica aponte no sentido da sua eficácia em situações menos graves. </p>     <p>De facto, cada vez mais o Laser oferece uma multiplicidade de soluções terapêuticas que apenas são possíveis pelas suas propriedades físicas. O tratamento com Laser CO2 fracionado no líquen escleroso também foi publicado muito recentemente e demonstra a eficácia em situações de atrofia vulvar muito grave, muitas vezes com lesões cicatriciais da vulva<sup>25</sup>.</p>     <p>A grande vantagem deste tratamento consiste na sua ausência de complicações imediatas ou tardias e no facto de ser uma técnica indolor, praticada em ambulatório com uma duração média de 10 minutos, com um efeito quase imediato e bastante mais duradouro do que todas as outras terapêuticas existentes, evitando o risco acrescido da terapêutica hormonal. Tem evidentemente a desvantagem económica, embora ainda não esteja estabelecida a relação custo-eficácia deste tratamento, pelo facto de ser muito recente. Por outro lado, também não sabemos a durabilidade deste efeito terapêutico. Sokol <i>et al</i> publicaram já este ano um estudo com 1 ano de <i>follow up</i> em que as pacientes mantinham ausência de sintomas<sup>17-18</sup>. Na nossa unidade ainda não foi feita esta avaliação da duração do efeito terapêutico. No entanto, a nossa percepção com a presente revisão é que o efeito se manteve pelo menos durante 6 meses. </p>     <p>Em conclusão, este estudo inicial sugere-nos a eficácia e a segurança do tratamento Laser CO 2 fracionado na AVV em mulheres na pós-menopausa. Contudo a amostra ainda é pequena sendo necessário um estudo prospetivo e observacional de forma a validar a eficácia e a analisar a durabilidade do efeito terapêutico.</p>     <p>&nbsp;</p>     <p><b>REFERÊNCIAS BIBLIOGRÁFICAS</b></p>     <!-- ref --><p>1.
