O conhecimento da fisiopatologia subjacente a estas patologias tem alterado a conduta clínica. Contudo, ainda não há consenso na abordagem. O objetivo deste trabalho é avaliar a concordância entre obstetras quanto às orientações na pré-eclâmpsia/eclâmpsia e síndrome de HELLP, publicadas por centros clínicos nacionais e organismos internacionais.  Materiais e Métodos:  Incluíram-se dois protocolos nacionais (protocolos) e quatro orientações internacionais (guidelines). Obteve-se a concordância de três obstetras após dividir as recomendações em oito categorias, do diagnóstico à gestão da doença, usando o método de Delphi modificado. Na comparação das recomendações os especialistas atribuíram um índice de concordância por categoria de &#8220;0&#8221; (discordância, diferenças com significado clínico), &#8220;0,50&#8221; ou &#8220;1&#8221; (concordância). A análise incluiu o cálculo das medianas dos índices de concordância, individualmente e globalmente.  Resultados:  Na maioria das categorias a discordância entre orientações foi parcial (medianas de &#8220;0,50&#8221;). As categorias menos concordantes foram &#8220;critérios de internamento na pré-eclâmpsia&#8221;, &#8220;critérios de gravidade da pré-eclâmpsia&#8221; e &#8220;orientação no síndrome de HELLP&#8221; entre protocolos e guidelines (medianas de &#8220;0,00&#8221;). A falta de concordância entre obstetras relativamente às orientações clínicas é reportada noutros estudos. As divergências detetadas poderão acarretar impacto nos desfechos materno, perinatal e neonatal.  Conclusão:  Os resultados sugerem a necessidade em elaborar orientações nacionais de conduta clínica nas doenças hipertensivas próprias da gravidez, incluindo rastreio e prevenção, obtidas através do consenso de um grupo de obstetras provenientes das diversas maternidades do país.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract  Overview and Aims:  The hypertensive disorders of pregnancy are important causes of maternal and fetal morbidity and mortality. Recent advances on the physiopathology have changed the management of these diseases but consensus on the best approach has not yet been reached. The main goal of this study is to evaluate obstetricians&#8217; agreement regarding the management of pre-eclampsia/eclampsia and HELLP syndrome as recommended by some Portuguese institutions and international organizations.  Methods:  Two national protocols (protocols) and four international guidelines (guidelines) were included. The agreement of three obstetricians was obtained by using the modified Delphi method. The recommendations were divided into eight categories, from diagnosis to disease management. Experts assigned an agreement index to compare each category of the recommendations based on a 3-score-scale: &#8220;0&#8221; (disagreement with clinical significance), &#8220;0.50&#8221; or &#8220;1&#8221; (agreement). The medians of the agreement indexes were calculated, individually and globally.  Results:  Partial disagreement (medians of &#8220;0.50&#8221;) was obtained in most categories. The categories with less agreement were &#8220;pre-eclampsia hospital admission criteria&#8221;, &#8220;pre-eclampsia severity criteria&#8221; and &#8220;HELLP syndrome management&#8221; with a disagreement considered as of clinical significance when comparing protocols and guidelines (medians of &#8220;0.00&#8221;). The lack of obstetricians&#8217; agreement regarding clinical management recommendations has been reported in other studies. Detected divergencies may have an impact on maternal, perinatal, and neonatal outcomes.  Conclusion:  The results point to the need to develop national guidelines for the management of hypertensive disorders of pregnancy, including screening and prevention, obtained through the consensus of a group of obstetricians, from different maternities in the country.]]></p></abstract>
