<body><![CDATA[<p>Os idosos     internados em institui&ccedil;&otilde;es de sa&uacute;de e sob o risco de infe&ccedil;&atilde;o a <i>influenza</i> podem ser contagiados pelos     profissionais de sa&uacute;de. Sobre estes &uacute;ltimos n&atilde;o existem dados comprovados     laboratorialmente acerca da taxa de infe&ccedil;&atilde;o a <i>influenza.</i> Assim, a vacina&ccedil;&atilde;o desta popula&ccedil;&atilde;o poder&aacute; reduzir o     n&uacute;mero de infe&ccedil;&otilde;es a este v&iacute;rus. Como os sinais e sintomas associados s&atilde;o     similares a outras patologias respirat&oacute;rias, &eacute; importante identificar     laboratorialmente a etiologia para comprovar o efeito da vacina&ccedil;&atilde;o na     preval&ecirc;ncia e incid&ecirc;ncia deste v&iacute;rus.</p>       <p>Foram     selecionados tr&ecirc;s estudos randomizados e controlados num total de 5.896     participantes e que apresentaram resultados que v&atilde;o ao encontro dos objetivos     propostos para esta revis&atilde;o.</p>       <p>Risco de     enviesamento: randomiza&ccedil;&atilde;o com baixo risco de enviesamento em dois estudos e     pouco clara em um; oculta&ccedil;&atilde;o e aloca&ccedil;&atilde;o pouco claras em todos os tr&ecirc;s ensaios;     baixo risco de resultados incompletos num ensaio cl&iacute;nico e alto em dois; baixo     risco de descri&ccedil;&atilde;o seletiva em todos os tr&ecirc;s estudos; alto risco de vi&eacute;s de     ades&atilde;o em todos os tr&ecirc;s ensaios (vacina&ccedil;&atilde;o de profissionais de sa&uacute;de contra a     gripe incompleta nos grupos de interven&ccedil;&atilde;o). N&atilde;o houve relato de eventos     adversos em qualquer estudo.</p>       <p>A vacina&ccedil;&atilde;o     dos profissionais de sa&uacute;de n&atilde;o demonstrou qualquer efeito na infe&ccedil;&atilde;o a <i>influenza</i> em utentes internados com 60     anos ou mais ou suas complica&ccedil;&otilde;es (infe&ccedil;&atilde;o do trato respirat&oacute;rio inferior,     hospitaliza&ccedil;&atilde;o ou morte devido a doen&ccedil;a do trato respirat&oacute;rio inferior).</p>       <p>Esta revis&atilde;o     n&atilde;o encontrou informa&ccedil;&otilde;es sobre outras interven&ccedil;&otilde;es utilizadas em associa&ccedil;&atilde;o &agrave;     vacina&ccedil;&atilde;o (higiene das m&atilde;os, utiliza&ccedil;&atilde;o de m&aacute;scaras, dete&ccedil;&atilde;o laboratorial     precoce de <i>influenza,</i> quarentena,     evitar novos internamentos, terap&ecirc;utica precoce com antivirais e redu&ccedil;&atilde;o do     contacto entre utentes e profissionais de sa&uacute;de com sintomas sugestivos de     gripe).</p>       <p>N&atilde;o h&aacute;     evid&ecirc;ncia que a vacina&ccedil;&atilde;o de profissionais de sa&uacute;de previna a infe&ccedil;&atilde;o por <i>influenza</i> ou suas complica&ccedil;&otilde;es (infe&ccedil;&otilde;es     do trato respirat&oacute;rio inferior, hospitaliza&ccedil;&atilde;o ou morte devido a infe&ccedil;&atilde;o do     trato respirat&oacute;rio) em indiv&iacute;duos internados com 60 anos ou mais, n&atilde;o existindo     indica&ccedil;&atilde;o para a vacina&ccedil;&atilde;o obrigat&oacute;ria destes trabalhadores. Outras     interven&ccedil;&otilde;es, como higiene das m&atilde;os, utiliza&ccedil;&atilde;o de m&aacute;scaras, dete&ccedil;&atilde;o precoce de <i>influenza</i> com zaragatoa nasal,     terap&ecirc;utica precoce com antivirais, quarentena, restri&ccedil;&atilde;o de visitas e do     contacto entre utentes e profissionais de sa&uacute;de com sintomas sugestivos de     gripe, podem proteger os indiv&iacute;duos internados com 60 anos ou mais.</p>       <p>S&atilde;o     necess&aacute;rios ensaios cl&iacute;nicos randomizados de alta qualidade para testar a     combina&ccedil;&atilde;o entre as v&aacute;rias interven&ccedil;&otilde;es dispon&iacute;veis.</p>       <p><b>COMENT&Aacute;RIO</b></p>     <p>Peter Doshi,     ao analisar a realidade americana, levanta quest&otilde;es importantes sobre a     influ&ecirc;ncia do marketing e das campanhas de sa&uacute;de p&uacute;blica no aumento exponencial     nos &uacute;ltimos 20 anos do n&uacute;mero de doses de vacina contra <i>Influenza</i> administradas.<sup>1</sup> Segundo dados publicados pela <i>World Health Organization</i> (WHO), a     comercializa&ccedil;&atilde;o a n&iacute;vel global da vacina atingiu em 2011 os $2,9 bili&otilde;es, sendo     que as estimativas para 2018 estimam um aumento para $3,8 bili&otilde;es. Ainda no     mesmo documento, as raz&otilde;es apontadas para este crescimento prendem-se com o     padr&atilde;o de consumo nos pa&iacute;ses desenvolvidos, a aposta em novos mercados de     pa&iacute;ses em desenvolvimento e o pre&ccedil;o crescente das novas vacinas     comercializadas.<sup>2</sup> De facto, institui&ccedil;&otilde;es como a WHO, o <i>Centers for Disease Control and Prevention</i> (CDC)<sup>3</sup> e o <i>National Institute     for Health and Care Excellence</i><sup>4</sup> recomendam a vacina&ccedil;&atilde;o de grupos     de risco e dos profissionais de sa&uacute;de envolvidos nos seus cuidados. Estas     recomenda&ccedil;&otilde;es baseiam-se em artigos que apontam para uma redu&ccedil;&atilde;o na transmiss&atilde;o     da infe&ccedil;&atilde;o dos profissionais de sa&uacute;de para utentes internados.<sup>5</sup> Mas     uma revis&atilde;o sistem&aacute;tica, publicada em 2008 pela <i>Cochrane,</i> veio identificar diferen&ccedil;as significativas entre a     efic&aacute;cia reportada em estudos financiados por ind&uacute;strias farmac&ecirc;uticas e     estudos financiados por entidades p&uacute;blicas, com maior efic&aacute;cia nos primeiros.<sup>6</sup> Por outro lado, Juan G&eacute;rvas, num documento intitulado &laquo;Algumas raz&otilde;es para     dizer &laquo;N&atilde;o, obrigado!&raquo; &agrave; vacina da gripe&raquo;, explora as diferen&ccedil;as entre a     reduzida ou inexistente evid&ecirc;ncia cient&iacute;fica apresentada em publica&ccedil;&otilde;es da <i>Cochrane,</i> <i>British Medical Journal</i> ou <i>Lancet</i> e as <i>guidelines</i> internacionais     defendidas por entidades como o CDC ou a <i>European     Center for Disease Prevention and Control</i> para este assunto.<sup>7</sup> As     conclus&otilde;es retiradas desta revis&atilde;o realizada pela <i>Cochrane</i> e de outros artigos<sup>8</sup> p&otilde;em em causa as     indica&ccedil;&otilde;es sobre a import&acirc;ncia da vacina&ccedil;&atilde;o dos profissionais de sa&uacute;de. Outro     dado interessante e transversal a v&aacute;rios pa&iacute;ses &eacute; a fraca ades&atilde;o &agrave; vacina&ccedil;&atilde;o     dentro deste grupo de pessoas. Num estudo de 2011, realizado em Floren&ccedil;a     (It&aacute;lia), que envolveu estudantes e profissionais de sa&uacute;de de tr&ecirc;s hospitais     universit&aacute;rios, verificou-se que a perce&ccedil;&atilde;o do risco de cont&aacute;gio da infe&ccedil;&atilde;o aos     doentes por parte dos indiv&iacute;duos questionados era baixa e que a maior parte dos     que aceitaram ser vacinados o fizeram para prote&ccedil;&atilde;o pessoal.<sup>9</sup></p>     <p>Tendo em     conta que grande parte destes profissionais se encontra fora dos grupos de     risco definidos para a vacina&ccedil;&atilde;o e em simult&acirc;neo a vantagem para os utentes n&atilde;o     se apresenta clara, urge construir novos estudos que supram esta lacuna.     Enquanto o conhecimento cient&iacute;fico n&atilde;o comprovar assertivamente os benef&iacute;cios     cl&iacute;nicos desta interven&ccedil;&atilde;o para os utentes e profissionais de sa&uacute;de, devemos     pautar a nossa pr&aacute;tica segundo o princ&iacute;pio de <i>primum non nocere</i> e agir com cautela.</p>     ]]></body>
