Am J Prev Med.     2014;47(3):330-40.</p>       <p>A vacina&ccedil;&atilde;o     obrigat&oacute;ria dos profissionais de sa&uacute;de contra o v&iacute;rus <i>Influenza</i> &eacute; uma realidade cada vez mais frequente; no entanto, o     real impacto cl&iacute;nico desta medida permanece por esclarecer. Neste estudo     procedeu-se a uma revis&atilde;o sistem&aacute;tica da literatura publicada sobre este tema     com o objetivo de esclarecer sobre os benef&iacute;cios e riscos da vacina&ccedil;&atilde;o neste     grupo.</p>       <p>A recolha     bibliogr&aacute;fica resultou da pesquisa em seis bases de dados internacionais <i>(MEDLINE, Embase, The Cochrane Library,     Cumulative Index to Nursing and Allied Health Literature, Science Citation     Index Expanded, and Conference Proceedings Citations Index)</i> de artigos que     avaliassem o efeito da vacina&ccedil;&atilde;o mandat&oacute;ria dos profissionais de sa&uacute;de contra o     v&iacute;rus Influenza, sem restri&ccedil;&atilde;o de outcome, desenho do estudo ou l&iacute;ngua; tamb&eacute;m     n&atilde;o foi considerada, como crit&eacute;rio de exclus&atilde;o, a aus&ecirc;ncia de grupo comparador.     Foram selecionados 12 estudos observacionais (oito relatos de caso, tr&ecirc;s     estudos transversais e um estudo de coorte) em que apenas tr&ecirc;s eram     multic&ecirc;ntricos. Quando avaliada a qualidade da defini&ccedil;&atilde;o da amostra, dos grupos     pr&eacute; e p&oacute;s interven&ccedil;&atilde;o e a presen&ccedil;a de co-interven&ccedil;&otilde;es, a maioria dos estudos     demonstrou um elevado risco de vi&eacute;s.</p>       ]]></body>
<body><![CDATA[<p>Do total de     estudos inclu&iacute;dos, oito reportaram <i>outcomes</i> relativos &agrave; taxa de vacina&ccedil;&atilde;o dos profissionais de sa&uacute;de (ap&oacute;s implementa&ccedil;&atilde;o da     obrigatoriedade dessa medida), tendo-se verificado um aumento das taxas de     vacina&ccedil;&atilde;o que ultrapassava os 94%. Em tr&ecirc;s desses estudos foram demonstradas     taxas de vacina&ccedil;&atilde;o significativamente superiores em hospitais, em que a     vacina&ccedil;&atilde;o foi de implementa&ccedil;&atilde;o obrigat&oacute;ria, quando comparados com institui&ccedil;&otilde;es     em que a mesma medida n&atilde;o foi aplicada (<i>p</i> &lt; 0,001). Em dois dos estudos foi analisada a taxa de absentismo dos     profissionais com resultados inconclusivos. Outros dois estudos avaliaram a     presen&ccedil;a de efeitos adversos da interven&ccedil;&atilde;o, tendo obtido dados limitados e     heterog&eacute;neos. Um dos estudos avaliou a opini&atilde;o dos profissionais de sa&uacute;de     relativamente &agrave; obrigatoriedade da vacina&ccedil;&atilde;o, tendo-se verificado que 96% dos     participantes consideraram a medida importante para a prote&ccedil;&atilde;o de pacientes e     profissionais e 72% consideraram que a forma de aplica&ccedil;&atilde;o mandat&oacute;ria era     coerciva. N&atilde;o foram estudados <i>outcomes</i> cl&iacute;nicos relativos aos pacientes nem os custos associados &agrave; interven&ccedil;&atilde;o.</p>       <p>Verifica-se,     assim, que a implementa&ccedil;&atilde;o da vacina&ccedil;&atilde;o obrigat&oacute;ria dos profissionais de sa&uacute;de     contra o v&iacute;rus <i>Influenza</i> conduz a um     aumento da taxa de vacina&ccedil;&atilde;o neste grupo mas n&atilde;o foram estudados os benef&iacute;cios     cl&iacute;nicos associados a esta medida. S&atilde;o necess&aacute;rios estudos prospetivos e     multic&ecirc;ntricos que avaliem <i>outcomes</i> eminentemente cl&iacute;nicos para que se possam tomar decis&otilde;es informadas     relativamente &agrave;s vantagens desta interven&ccedil;&atilde;o.</p>       <p><b>COMENT&Aacute;RIO</b></p>       <p>A primeira     vacina contra o v&iacute;rus <i>Influenza</i> come&ccedil;ou a ser produzida em 1938, mas aplicada em larga escala &agrave; popula&ccedil;&atilde;o     americana apenas em 1979, para &nbsp;preven&ccedil;&atilde;o da gripe Su&iacute;na.<sup>1</sup> Na     procura de evid&ecirc;ncia sobre a efic&aacute;cia e efetividade da vacina&ccedil;&atilde;o, uma revis&atilde;o     sistem&aacute;tica de 2012 identificou e analisou 17 ensaios cl&iacute;nicos randomizados e     aleatorizados e 14 estudos observacionais que englobavam estes <i>outcomes</i> em casos de gripe comprovados     laboratorialmente. Nestes casos, que raramente fazem parte da pr&aacute;tica cl&iacute;nica,     a efic&aacute;cia da vacina&ccedil;&atilde;o apenas provou ser moderada, sendo drasticamente     reduzida ou mesmo ausente em algumas esta&ccedil;&otilde;es do ano. Os mesmos autores     identificaram que em adultos acima dos 65 anos a evid&ecirc;ncia escasseia.<sup>2</sup></p>       <p>Quando se     analisa o racional de vacinar profissionais de sa&uacute;de surgem argumentos como     preven&ccedil;&atilde;o da doen&ccedil;a e consequente diminui&ccedil;&atilde;o do absentismo laboral, mas tamb&eacute;m     preven&ccedil;&atilde;o do cont&aacute;gio a doentes. Como fica claro no resumo da revis&atilde;o     sistem&aacute;tica, os dados relativos a este “grupo de risco” foram inconclusivos.</p>       <p>Contudo, a     aus&ecirc;ncia de estudos e a incerteza quanto &agrave; efic&aacute;cia da vacina&ccedil;&atilde;o destoam do     atual panorama normativo internacional e nacional que insiste em recomendar a     vacina a grupos de risco, nomeadamente a profissionais de sa&uacute;de.<sup>3-4</sup> De forma ainda mais incompreens&iacute;vel, assiste-se j&aacute; em alguns pa&iacute;ses a uma     crescente press&atilde;o no intuito de aumentar a taxa de cobertura vacinal<sup>5-8</sup> atrav&eacute;s da vacina&ccedil;&atilde;o laboral mandat&oacute;ria ou fortemente recomendada.</p>     <p>Devemos, por     isso, investir na procura e na divulga&ccedil;&atilde;o de conhecimento cient&iacute;fico robusto e     de qualidade e alertar para a fragilidade de fundamenta&ccedil;&atilde;o de algumas normas de     orienta&ccedil;&atilde;o e <i>guidelines.</i></p>       <p>&nbsp;</p>       <p><b>REFER&Ecirc;NCIAS BIBLIOGR&Aacute;FICAS</b></p>       <!-- ref --><p>1.
